[Dealing with AIDS in a North Rhine Westphalia large city: Bottrop].

The various Public Health offices in North Rhine Westphalia are making a substantial contribution towards AIDS prevention means of their relevant concepts. Regional and political individualities as well as local structures can exercise a positive or a negative effect on concrete AIDS prevention work. Cities like Düsseldorf, Köln, Essen etc., enjoy material and personal advantages. Smaller communities (like Bottrop) must rely entirely on creative working concepts. The AIDS prevention concept of Bottrop (a city situated in the northern Ruhr area with a population of 120,000), consisting of sexual education and paedagogical approaches, is presented. The information aims at specific groups, but has no ideological strings attached. The report refers to five main points of working: 1. AIDS prevention with special attention to adolescents in school and out-of-school contexts. 2. Medical advice by institutions. 3. Daily AIDS consulting hours in the Public Health office. 4. Planning and realisation of an action day 'Health Day'. 5. Working with drug addicts. In addition, there is an epidemiological approach. The overall aims are the prevention or infection and a human psychosocial spectrum of advice without excluding anyone.

[The cervical lymph node in violence to the neck (author's transl)].

In choking, throttling, and atypical hanging lymph nodes above and below the region of violence show dilatation and hyperemia of capillaries, unequally distributed. Moreover, irregularly distributed slight bleedings in the tissue of lymph nodes are to be found. Blood is to be seen in the intermediary sinuses and in the marginal sinus in spite of the short time of survival. Obviously, the intensity of violence is of importance. Hypostasis in a deep position of head during unconsciousness and reanimation procedures--both events usually last one or several hours--produce an almost equal dilatation of capillaries and veins of cervical lymph nodes. Slight bleedings are to be found especially near the follicles. The different microscopic pictures may be of significance with regard to differential diagnosis.

[Haemorrhages, directly and indirectly in origin, localised in the musculus sternocleidomastoideus. (Contribution towards the angio-architecture of the skeletal muscle) (author's transl)].

Haemorrhages in skeletal muscle--as in this case the musculus sternocleidomastoideus--caused by direct and local impact very often have a patchy form. They are produced not only by lesions of capillaries, small veins, and small arteries, but also by damage of muscle fibres. Indirect impact stretches muscle fibres and blood vessels very much, far away from the locality of direct impact. By this maximal stretching a protection mechanism is abolished, which exists by of the different angles of a blood vessel and its small vessel branches off, adapted to the distance of contraction and stretching and by the thickened connective tissue surrounding the vessels. Thus, blood vessels may be torn, especially such vessels that branch off nearly at a right angle. Blood will then flow into the small areas of connective tissue surrounding the vessels, and bleedings occur in form of stripes.

[Lethal intoxication by formalin during dialysis (author's transl)].

Systems for dialysis are disinfected, among other solutions, by formalin. Residues of formalin may remain in the interior of the system. A lethal intoxication with formalin by transvasal introduction of formalin during the dialysis of patients with nephritic disease cannot be observed very often. Two cases of death at the same time in one system for dialysis make evident that every case of death during dialysis should be investigated. The central symptom in both patients was dyspnea. It is probable that formalin affects the transportation and exchange of oxygen with lethal hemolysis.

[Administration of D-penicillamine instead of penicillin with fatal results: allergy of the delayed type with protracted shock as fatal complication of treatment with D-penicillamine for rheumatoid arthritis in a child (author's transl)].

Following one-week treatment with D-penicillamine for rheumatoid arthritis a seven-year-old girl died with the clinical features of protracted circulatory shock. Several doctors had mis-diagnosed the skin changes and mistakenly prescribed penicillamine, given for the rheumatoid arthritis, instead of plenicillin at the latter's dosage. By the administration of penicillamine a hypersensitivity reaction of the delayed type had been induced which ended fatally due to severe shock.

[Child abuse with fatal sequelae in an Essen autopsy sample].

In the Institute of Forensic Medicine in Essen/Germany 24 cases of lethal child battering or neglect have been observed over 17 years from 1973 to 1989. The medicolegal and morphologic findings in these cases are presented and compared to the literature. The patterns of abuse/neglect in Essen compare to those frequently reported in the literature: skull/brain traumas caused by blunt impact, multiple hematomas, bone fractures, and symptoms of malnutrition, vitamin deficiency, and general neglect. In many cases the abuse could be shown to have been chronic. In the seven years 1983-1989 the cases of lethal child mistreatment and neglect amounted to 0.18 percent of all autopsies. Five cases of lethal sexual assault in children during the same period are compared to the mistreated cases.
