[Adenoacanthoma of the pancreas (author's transl)].

Case reports are given of 5 patients with adenoacanthoma of the pancreas. Clinical aspects of this syndrome are discussed taking into account the own cases and cases reported in the literature; there are no differences of the clinical picture as compared to pancreatic carcinoma of ductal type. Surgery of this type of malignancy does not improve life expectancy, however in many cases surgery is justified because it may alleviate pain. Only 1,1-4,4% of all patients with pancreatic carcinoma do harbour adenoacanthoma - thus further case reports may contribute to better describe the clinical and pathological picture of this disease and possibly to further elucidate the pathogenesis.

[Long-term follow-up study of C-cell-function after partial duodenopancreatectomy (author's transl)].

Late results were obtained from the follow up of 48 patients with chronic pancreatitis, who underwent partial duodenopancreatectomy. We measured the rest function of the remaining B-cells after resection by daily glucose profile, i.v.-gtt, measurements of the glucagon stimulated C-peptide-output and the amount of C-peptide in the 24-h-urine. In 9% of the cases the operation induced diabetes in addition to the already existing 31%. 3/4 of the nondiabetics showed a latent diabetic metabolism (K value < 1.0). The cause of this, as shown by the C-peptide-analysis, was the loss of the endocrine functional reserve following pancreas resection because of chronic pancreatitis. Therapeutically great differences resulted in reaching and equilibrium of serum glucose in the pancreas resected insulin-dependent patients, because they were dependent on carbohydrates for energy. The tendency to hypoglycaemia represented an additional endangerment.

[The effect of exocrine glandular tissue on the transplantation results of isolated islands of Langerhans].

Islet cells were transplanted into isologous rat strains with exocrine pancreatic tissue as contaminant. The relationship of transplant function to the degree of contamination was tested. No influence on transplant success was found despite a five fold amount of exocrine tissue in one group after transplantation into livers of diabetic animals. But a 12-fold contamination of the 2000 isolated islets of Langerhans did not allow a normalization of blood sugar content. The glucose assimilation coefficient was constantly below 1.0. Purification of islet cell transplants was estimated by determination of amylase activity per islet according to SUTHERLAND.

[Function of the residual pancreas following partial duodeno-pancreatectomy].

After Whipple operations, follow-up examinations were conducted under hospital conditions in order to investigate the function of the remainder of the pancreas and the extent to which general health was adversely affected. General parameters such as vocational rehabilitation, history of pain, and weight were analyzed, as well as chemistry related to the severity of pancreatic malassimilation, e.g., stool weight, stool fat contents, fat utilization, chymotrypsin in stool, and PABA test. An exocrine pancreatic insufficiency was found in 80% of patients, but this was easily manageable using medications, sometimes in combination with a MCT fat diet. Subclinical diabetes mellitus was shown in 80% of patients using glucose tolerance tests. However, clinical manifestations of diabetes did not occur.

[Transplantation of isolated islands of Langerhans for the treatment of diabetes mellitus].

The historical development of research on islets transplantation is briefly reviewed followed by a description of a technique using Ficoll gradient separation to obtain islets of Langerhans from the rat pancreas. It was possible to increase the number of islets obtained with this procedure by modifying it in several ways. The islets were then transplanted in an isologue manner into rats with streptocotocin-induced diabetes. The effect of transplantation on glucose metabolism in these rats was evaluated both by determination of the general symptoms typical for diabetes as well as by the performance of such functional tests as IVGTT, GAC and tolbutamide test. The islets could be transplanted into various locations, but it was found that positive results were obtained only if the liver was the site of application and if than 2100 islets were used. Three hours after transplantation normalization of blood sugar levels and serum insulin could be observed; these levels remained stable over 18 months. It was possible to transplant in the same animal several times. This had an effect on the metabolism which was equivalent to the sum of the separate transplants. By means of both light and electron microscope examination the morphological changes which the transplanted islets underwent at the site of transplantation were observed.

[Behaviour of serum phosphorus level during the postoperative period (author's transl)].

Behaviour of serum phosphorus and phosphate-excretion in urine has been observed during the postoperative period in 40 patients who obtained different amounts of phosphate-substitution. The findings were compared with 8 patients who had a phosphate-depletion syndrome. It was shown, that the appearance of hypophosphataemia during the postoperative period can be prevented by a daily supply of at least 25 mmol phosphate. If a phosphoate depletion syndrome has occurred, the substitution of 40 to 50 mmol phosphate and simultaneous reduction of caloric supply is recommended. Furthermore, routine control of serum phosphorus level and, if necessary, measurement of phosphate excretion in urine should be performed in any type of postoperative parenteral nutrition.

[Methods of evaluating the function of transplanted islets of Langerhans (author's transl)].

The various groups who are working on islet-transplantation used different parameters in determining the success or failure of their experiments. In our own transplantation-trials we tested the value of using the symptoms typical of diabetic humans for rats. These are: change in bodyweight, water intake, urine-volume and glucose content as well as the glucose-level in the blood. Special diabetic provocation-tests were also carried out with rats. It was shown, that the i.v. glucose-tolerance-test (IVGTT) and the glucose assimilation-coefficient (GAC) were able to demonstrate the reversal of the diabetic state after islet-transplantation. The tolbutamid-test gave no additional evidence about the success of the transplantation. Histological examinations should always be carried out in addition to the biochemical tests, because a decrease in the islet's-function is shown to depend on a higher rate of morphological regressions.

[The loss of exocrine pancreas-function following operation for pancreatitis (author's transl)].

In a follow-up-series on 37 patients, who had been operated on for pancreatitis, the effect of pancreas insufficiency on the well-being of resected and non-resected patients was examined. Analyses of fatcontent in feces, collected over a three day-period, revealed that all patients with chronic pancreatitis now had a maldigestion, in particular patients with duodenopancreatectomie. Pseudocyst-patients showed no maldigestion. Increased in the bodyweight in most cases could not only be explained by drug-substituion of encymes but was also due to the resection of the pain-producing area of the pancreas.

Multiple transplantation of islets of Langerhans.

In 6 diabetic rats about 1,000 isolated islets of Langerhans were transplanted into the liver with little effect on the uninsulinism and hyperglycemia. Subsequently, a second transplantation resulted in a decrease of blood sugar to normal and in a reversal of the loss in body weight. Finally, in animals who had reversed to diabetes again, a third islet transplantation was performed. These rats with a total of 3,100 islets showed a decrease in the glucose levels from 250-305 to 110 +/- 27 (X +/- SEM) mg/100 ml. The insulin level after glucose stimulations was measured at 27 less than X less than 35 micromicron/ml.

[Progress in isolation technic of islets of Langerhans for transplantation].

An improved technique is described which yields up to 6,000 isolated islets of Langerhans from the pancreas of rats. The quotients for insulin/amylase and insulin/protein were estimated in vitro. For in vivo testing isologous transplantations of islets were performed. Several parameters such as total body-weight, blood sugar levels, insulin i.s., glucose-tolerance and glucose-assimilation tests demonstrated that the islets still retained their normal function despite the isolation procedure.

[Long term results following surgical therapy for chronic pancreatitis (author's transl)].

A report is presented on the results of the follow-up examination of 43 patients who had been previously operated on chronic pancreatitis. Clinical data such as the patients' well being, their ability to work were collected and specific tests for the function of the pancreas such as fat-content in faeces and glucose-tolerance-test were made. It was shown, that the pancreas-resected patients felt better in themselves. Moreover, their clinical picture was superior to that of non-resected patients although they had initially required a longer postoperative rehabilitation phase. Pseudocysts, which had been drained, showed no diminution of the pancreas function, nor any tendency to rest-pancreatitis.

[Transplantation of vital islands of Langerhans in diabetic animal recipients].

Pancreatic islets of 300 g-Lewis-rats have been transplanted to 200 g streptozotocin-diabetic rats after preparation by manual-dissection method and by the Ficoll-method. The transplantation of more than 500 intact pancreatic islets into the peritoneal cavity of streptozotocin-diabetic-Lewis-rats results in a significant reduction of hyperglycemia, an increase of insulin output and a restoration of weight gain. It is suggested, that it is best to use the glucose-tolerance-test for interpretation of insulin-output and insulin-effect.
