[A modified procedure for repositioning shoulder dislocation].

A new method is presented for the reduction of shoulder dislocations and dislocation fractures, together with the follow-up results. This procedure is a modification of a method for reduction of the hip. Examination of our patients 1-3 years after the accident showed no sign of nerve lesions and little residual discomfort.

[Aggressive fibromatoses. Treatment concept and results with special reference to ultrasound diagnosis].

Therapeutic tactics and control regime for aggressive fibromatosis are described in this paper, with reference being made to the authors' own patients. Locally delimited, wide excision without supporting radiotherapy is recommended also for recurrent cases. Anti-oestrogenic treatment has now been adopted to cope with locally delimited inoperability. The polytopic, usually congenital form of aggressive fibromatosis must be interpreted as a particularly unfavourable course with potential malignancy. Sonography so far has proved to be a suitable approach to pre-operative and postoperative screening.

Adult

[Therapy of acute poisoning with orthograde intestinal lavage].

Orthograde intestinal lavage was successfully used in the treatment of eight patients with acute poisoning. Three had mushroom (Amanita pantherina) poisoning, five acute drug intoxication (attempted suicide: diazepam, dihydropyridine, crotylbarbital, phenobarbital, amitriptyline and glibenclamide). Within 5-18 hours the clinical signs of poisoning regressed in four patient in stage IV (after Reed), so that the patients became responsive. Detoxication by orthograde intestinal lavage is achieved by mechanical removal of nonabsorbed compounds from the gastro-intestinal tract and via intestinal dialysis across the intestinal mucosa.

Amitriptyline

[Preoperative preparation of the colon with special reference to orthograde intestinal lavage].

The effectiveness of the orthograde lavage of the colon with isotonic saline and 153-electrolyte solution, resp., was checked by means of a comparative prospective study. The most important factor in the pre-operative preparation of the colon is mechanical cleaning which is best performed by way of orthograde lavage leading in 90% of the cases to a complete emptying of the colon. Reduction of the intestinal flora can be improved in addition to lavage by a short prophylaxis with large doses of antibiotics. This method was applied to 100 operations performed in the colorectal region and only in 6% of the cases was an infectious complication observed. Lavage of the colon raises no problems and is well tolerated even by elderly patients. Ileus and the risk of perforation in the gastrointestinal tract are absolute contra-indications. Because of its almost iso-ionic composition the authors prefer the 153-electrolyte solution to isotonic saline. Due to the increase of the extracellular space some blood parameters tend to decrease.

[Nailing of pertrochanteric fractures after Lezius and Herzer (author's transl)].

Out of a total of 297 patients (1967 to 1973) suffering from pertrochanteric fractures, 122 patients underwent operative treatment and were followed up by a questionnaire. The late results do not differ significantly from patients treated conservatively. There is only a decrease of the mortality rate during hospitalisation.
