[Level of fibrinogen/fibrin degradation products (F/FDP) in certain internal diseases].

Serum FDPs were investigated in 30 healthy and 95 patients with pulmonary thrombembolia, not-stabilized angina pectoris, myocardial infarction, rheumatism, rheumatoid arthritis, lupus erythematodes and dermatomyositis. FDPs are determined by hemagglutination inhibition according to Merskey. They are found in the sera of the healthy in average values of 3.73 mkgr/ml. The highest average values in the first 24 h were found in case of pulmonary thrombembolia up to 106.64 mkgr/ml, followed by rheumatoid arthritis 26.3 mkgr/ml, myocardial infarction with complication 22.4 mkgr/ml, rheumatism +5.58 mkgr/ml, not-stabilized angina pectoris 5.5 mkgr/ml; and noncomplicated myocardial infarction 4.3 mkgr/ml. By the third day of the disease FDP in pulmonary thrombembolia decreased, whereas a negligible elevation was observed in case of non-complicated myocardial infarction. The results were interpreted as well as the cause for the presence of the mentioned products in those groups of diseases. FDP determination is recommended as a routine method in case of: diagnosis of pulmonary thrombembolia, differentiation of myocardial infarction with or without complications, differentiation of pulmonary thrombembolia from myocardial infarction in emergency states, progressing with chest pain, collapse phenomena, dyspnea and establishment of the activity of the process of rheumatoid arthritis. FDP determination in stenocardia and rheumatism is not expedient.

[Phase analysis of the normal and postectopic left ventricular contractions in nonejection click-telesystolic apical murmur syndrome].

ECG, PCG, CSG and ACG were synchronously recorded in 25 patient with non-expulsion click syndrome -- telesystolic murmur as well as in 62 cardiac healthy subjects. With the investigation of the systolic and diastolic intervals, a prolongation of the transformation period was established (Q--1 = 72 +/- 12 msec) and a shortening of the isovolumetric relaxation (A2--0 = 63 +/- 10 msec). The ratio PPE/LVET (period prior to expulsion/left venticular expulsion time) is normal in patients with nonexpulsion click -- telesystolic apical murmur but the comparison of PPE/LVET with a normal contraction with that of the first post extrasystolic contraction shows a statistically significant shortening. The ratio PPE/LVET in prolapse of the mitral valve prior to extrasystole is 0.39 +/- 0.02, and during the first contraction with intensified stroke after a compensatory pause is 0.31 +/- 0.03 (p less than 0.02). The possibility for explanation of the changes in phase analysis are discussed and a conclusion is drawn that they reflect the existence of deviation in the left ventricular mechanics and hemodynamics in case of mitral valve prolapse.

[Assessment of left ventricular function in hypertension by means of phase analysis].

Polygraphic records of ECG, PCG, carotissphygmogram and apexcardiogram were performed and 16 systolic and diastolic intervals were taken in 86 patients with hypertonic disease, distributed according to the stage of the disease according to Mjassnikov classification as well as in 62 subjects with intact hearts. A lengthening of asynchronous contraction was established as well as of the isovolumetric contraction, pre-blood driven period, isovolumetric relaxation and shortening of the left-ventricle time of blood driving, the ratio pre-blood driven peroid/left ventricle time of blood driving showing the most pronounced changes. Regression and correlation analysis in the comparison of those changes in the intervals with a means blood pressure in patients from the three stages of hypertonic disease, show that the best correlation is obtained in the patients from the III stage. The results obtained are discussed and a conclusion is drawn that the ratio pre-blood driven period/left ventricle time of blood driving is with the highest practical importance from the systolic and diastolic intervals in the assessment of the adequacy of the left-ventricle compensation.

[Changes in the level of alpha1-antitrypsin in bronchial asthma].

The alfa1-antitrypsin level was followed up in patients with bronchial asthma with different forms and severity of the disease. The latter was found to be elevated in all patient groups with an average of 335,38 mg% as compared with the controls 290.83 mg%, p less than 0,01. The elevated antitrypsin is explained with the allergic process and the secondary bacterial inflammatory reactions, triggering the formation of antienzyme substances, in order to inactivate the enzymes, playing a certain role in the pathogenesis of the first two processes--allergy and bacterial inflammation. In patients, both treated and not with cortisone, antitrypsin is elevated with no manifestation of significant difference for both patient groups. The cortisone preparations very likely do not supress the antienzyme systems in organism. As regards the pathogenetic role of alfa1-antitrypsin deficiency in asthma, the latter is admitted to play an insignificant role only in single cases.

[Serum immunoglobulin E level in bronchial asthma].

Serum immunoglobulin E level was determined in 50 patients with bronchial asthma, treated in the out-patients department and clinical conditions at the Faculty Hospital--Varna. The quantitative determination of immunoglobulin E was carried out by radial immunodiffusion according to Mancini with monospecific anti-IgE globulin serum from Behringswerke (GFR). A considerable elevation of immunoglobulin E in the patients' sera was found, at an average of 394 IU (control 124 IU). A discrepancy in serum immunoglobulin E level was established with the different clinical forms of asthma. The highest are the values with infectious-allergic astmha-424 IU. High are the values both in the treated and not-treated with corticosteroids, without an essential difference between the two patient groups. Some of the rest immunoglobulins showed also an elevationppIgG 2620 mg% and IgA 366 mg%.

[Some modern views of the role of the eosinophils in allergic reactions and bronchial asthma and a new method of detecting eosinophils in the bronchial secretion].

A review of certain contemporary opinions of eosinophil function in allergic reactions and bronchial asthma is presented in this report. Phagocytosis and processing of the complexes antigenantibody, histamine inhibition and a histamine elimination elimination by a specific inhibitor, isolated from eosinophilis (EDI), stimulation of prostaglandines E release, which also inhibit histamine and have a bronchial dilataion effect. The new method is recommended for eosinophil detection in sputa based on the fluorescent principle as faster and more efficient.
