[Mechanical ileus (author's transl)].

UNLABELLED: 181 patients with mechanical bowel obstruction were treated between 1965 and 1974 at the First Surgical Clinic of the University of Vienna. 172 were operated, 7 had conservative treatment. RESULTS: 1. the mortality was 24,3% and infavorably influenced by a) advanced malignant tumors, b) age over 60 years, c) preexistent peritonitis. -2. Enterotomy for bowel-decompression was in no case followed by a complication.- 3. Nobles operation should be performed only totally and in cases of severe adhaesions. 4. Discussion of the one stage operation in large bowel obstruction, which is only indicated in patients with good general condition. The high letality of 47% after operations in cases with ileus due to carcinosis peritonei requires critical indications for these operations.

[Acute hemorrhage of the upper digestive tract with special reference to stress ulcer. Prevention and surgical therapy].

For the surgical therapy of an acute stress-ulcer bleeding there are three methods to someones disposal: 1.) the local care of a bleeding source 2.) the partial gastrectomy and 3.) vagotomy with pyloroplasty and local care of a bleeding source and as prophylaxis of an acute stomach bleeding the highly selective vagotomy with or without pyloroplasty. On judging the success of these methods, some has to consider the basal illness with its letality, because it is to differ, wether a patient died postoperative on his basal disease, or on the effect of the operation, or out of deficient control of the situation. Considering such points it comes out clear, that today the vagotomy with a drainage operation is the method of choice, because: 1.) through this intervention patients are tainted less as for instance through a partial gastrectomy, 2.) through a technical faultless done operation, especially through a vagotomy a success concerning a relapse bleeding is guaranteed. Beside that, the highly selective vagotomy without drainage operation presents itself as prophylactic measure, to prevent an acute stomach bleeding in a number of transplanted patients (kidney transplantation). The first experiences with this method show, that a improvement of the results is to be expected.

[New method for the long-term cannulation of the common bile duct in the dog].

A new method for long term cannulation of the common bile duct in dogs is reported. The procedure is carried out with a T-tube, which is brought out through the abdominal wall. There it is rolled up in the hole of an implanted Thomas cannula. - A reliable protection from removal by the animal. The result in 9 dogs was very good, the longest duration of cannulation was 9 months.

[Advances in the treatment of postoperative complications after abdominal operations. A comparative, retrospective analysis of two reported periods (author's transl)].

Period I (1958-64)included 227 complications, period II 257 complications. Altogether, 484 complications were evaluated in 436 cases. The by far higher lethality in period I (71,3 percent) compared to 41,1 per cent in period II can be explained by the lower frequency of relaparotomy of 24,6 percent (52,3 per cent in period II). The mortality after relaparotomy was almost equal in both periods. The central problem in the therapy of postoperative surgical complications can consequently be seen in the indications for relaparotomy. Therefore, to improve the results, a more active diagnosis and indication and adequate performance of the operation are necessary.

[Kinetics of bile acid in jejuno-ileal bypass].

The influence of jejunoileal bypass on bile salt kinetics and lithogenity of bile was investigated in 9 German shephard dogs. Total bile acid pool decreased 92% from initial values, bile acid turnover rate was enhanced markedly. High relative biliary cholesterol concentration lead to an increase of lithogenity of the bile.
