[Secondary intraocular lens implantation].

PURPOSE: To analyse results of secondary intraocular lens implantation in aphakic eyes. MATERIAL AND METHODS: Sixty five eyes of 63 patients (23 females and 40 males) had secondary intraocular lens (IOL) implantation, from December 1996 to April 1997. Follow-up was from 1 to 21 months. An anterior chamber lens was used in 51 eyes (78.5%) and a posterior chamber lens in 14 eyes (21.5%); these included 6 cases of trans-scleral fixated lenses. The most common reason for secondary implantation was to relieve the discomfort caused by spectacles or contact lenses. RESULTS: Final postoperative visual acuity of 20/40 or better was achieved in 77% of the eyes. The most common postoperative complications were: pupil deformation, transient corneal edema and hyphema. One eye developed endophthalmitis requiring pars plana vitrectomy and one eye had cystoid macular edema.

[Vitrectomy for treatment of ocular complications from diabetes. II. Vitrectomy complications].

UNLABELLED: The aim of this study was to present complications of pars plana vitrectomies (ppV) for diabetic retinopathy. MATERIAL: The intraoperative and postoperative complications were described on the basis of results of 168 procedures performed in the period 1991-1993. RESULTS: The most frequent intraoperative complications were bleeding from the retina vessels and iatrogenic tears while the membrane peeling was done. The main postoperative complications were: detachment of the retina, hemorrhages to the vitreous cavity, secondary glaucoma with rubeosis iridis and cataract. The authors present their own experience in treating complications of ppV and postoperative care.

[Vitrectomy for treatment of ocular complications in diabetes. III. Factors influencing results of vitrectomy].

UNLABELLED: The aim of this study was to find factors influencing results of pars plana vitrectomy for proliferative diabetic retinopathy. MATERIAL AND METHODS: 25 general and local factors which could influence results of pars plana vitrectomy (ppV) were selected and compared with functional results of surgery of 168 eyes with diabetic retinopathy. RESULTS: The most significant general factor was the age during ppV and the age at diagnosis of diabetes. Glycosylated hemoglobin blood level had little influence on ppV results. The most important local factor was the morphological state of the retina before the surgery. Better functional results were also achieved in eyes treated with panretinal photocoagulation before ppV and treated with vitrectomy without silicone oil tamponade when visual acuity was better than 5/50. CONCLUSIONS: General state of patients before ppV has little influence on functional results of ppV. Local factors are the main factors influencing this results.

[Long-term results of vitrectomy in children and adolescents].

PURPOSE: Assessment of vitrectomy in the treatment of children and adolescents. MATERIAL AND METHODS: We evaluated the treatment of 44 children between the ages of 8 and 17 who underwent vitrectomy from January 1991 to January 1996. In 35 eyes indication for vitrectomy was complicated intraocular trauma, in 6 eyes Toxocara canis infection, in 2 eyes idiopathic vitreous hemorrhage and in eye complication due to diabetes. The mean follow-up period of study was 15 months. RESULTS: The anatomic success category included eyes with attached retinas and a final visual acuity above 1/50 which was achieved in 85% of operated eyes. Within this group 50% of the eyes had a final visual acuity of 5/50 or better. Anatomic and functional success were stable for a period of 12 months after vitrectomy. CONCLUSIONS: 1. Vitrectomy is the primary method of treatment for complicated intraocular trauma in children. 2. Through vitrectomy we achieved functional success which facilitated visual rehabilitation after ocular trauma. 3. Vitrectomy in children and adolescents have fewer complications when compared to vitrectomy in adults.

[Vitrectomy in ocular complications of diabetes. I. Early and late results of vitrectomy].

Results of 168 pars plana vitrectomies (ppV) of 144 diabetics are presented. In 26.2% of eyes before surgery, vitreous hemorrhage without detachment of the retina was present. In the remaining eyes, different stages of tractional detachment occurred. Visual acuity worse than 1/50 was observed in 83.9% of eyes before surgery. Results of ppV were evaluated on the 7th day and after the 3rd, 6th, 12th and 18th month following the procedure. Anatomical and functional success was achieved in about 80% and 67% of treated eyes, respectively, over the whole follow-up period.

[Vitrectomy in Terson's Syndrome].

AIM OF THE STUDY: The authors present results of vitrectomy in the treatment of a 50 year old woman with Terson's Syndrome. MATERIAL: Pars plana vitrectomy was performed in two eyes of a 50 year old woman with vitreous hemorrhage caused by the rupture of cerebral arteries aneurysms. The vitreous hemorrhage occurred two days after the neurosurgery procedure. Visual acuity in both eyes was hand movement and did not change after pharmacotherapy and cryotherapy. Pars plana vitrectomy was performed in the right eye 3, 5 months after the hemorrhage and in the left eye 5 months after the hemorrhage. Visual acuity 5/12 in the right eye and 5/7 in the left eye was achieved after the vitrectomy and 5/50 (the decrease caused by cataract) and 5/7 after 11 and 7 months of follow-up. CONCLUSION: Pars plana vitrectomy is a method of choice in the cases of binocular Terson's Syndrome and accelerates returning of useful visual acuity.

The antibiotic edeine. XII. Isolation and structure of edeine F.

The peptide antibiotic edeine F produced by Bacillus brevis Vm4, one of the components of edeine antibiotics complex, was isolated from a fermentation broth and was also obtained by amidination of edeine D. Edeine F is composed of amino acids: (S)-beta-phenyl-beta-alanine, (S)-isoserine, (S)-2,3-diaminopropionic acid, (2R,6S)-diamino-(7R)-hydroxyazelaic acid, glycine and a polyamine guanidylspermidine. Enzymatic degradation of antibiotic with carboxypeptidase B, dinitrophenylation of edeine and of its enzymatic degradation products and synthesis of edeine F from edeine D of known structure permitted to postulate the chemical structure for edeine F.

Vitreoretinal surgery in diabetic patients on hemodialysis.

The present paper reports our first results after pars plana vitrectomy in patients with diabetic retinopathy and hemodialysis with a follow-up of 6 to 24 months. Between January 1992 and October 1994 we performed vitreoretinal surgery with silicone oil tamponade in nine eyes of seven patients with diabetic nephropathy on hemodialysis. All patients had had type I diabetes for 19-32 years. Over the observation period the retina was completely attached in eight eyes. Final visual acuity of 0.1-0.7 was attained in four eyes, 0.06 two, hand movements in one eye. Two eyes had no useful final visual acuity because of redetachment of the retina or secondary glaucoma with rubeosis iridis. The small number of complications shows that pars plana vitrectomy can be done in diabetic patients with nephropathy on hemodialysis. This significantly improves their quality of life.

[Current views on the pathogenesis and treatment of central serous choroidopathy].

The majority of authors as a cause of formation of a leakage in central serous choroidopathy considers the disturbances of function of the pigmentary layer. According to Spitznas it is based on transformation of these cells from absorbing to extracting towards the choroid and retina under the influence of unknown factors (immunological processes, circulatory, inflammatory factors or stress). Marmor sees the lesion of the pigmentary epithelium as a disturbance of a cyclic action of the AMP neeth the neurosensory layer of the retina. There exist contemporary 2 methods of treatment of the central serous retinopathy. One part of the authors thinks that one should wait till the spontaneous remission, the other one--that in cases with the leakage situated beyond the risk area laser therapy can be applied.

[The role of laser therapy in central serous choroidopathy].

The authors present early results of treatment of central serous choroidopathy by means of argon laser. Additionally they compare the fate of the patients with this condition treated conservatively in the last years. The results obtained confirm the world reports that application of laser in therapy of central serous choroidopathy shortens the course of the disease.

[Role of intraocular tamponade in treatment of complications of proliferative diabetic retinopathy].

The authors presented results of pars plana vitrectomy performed in 80 eyes of 70 patients with proliferative diabetic retinopathy. Endo-tamponade with silicone oil was applied in cases complicated by intraocular haemorrhages and tractional or rhegmatogenous retinal detachment. A complete attachment of the retina was achieved in 70 eyes, in 5 a limited area of flat retinal detachment persisted and in 5 the results were negative. The functional results were evaluated in two groups: 1) 26 eyes in which silicone oil was not used or was removed after several months, 2) with permanent silicone oil tamponade. Statistically significant better visual acuity was achieved in the first group. The authors concluded that for better results vitrectomy should be performed in less advanced stages of proliferative diabetic retinopathy, in which permanent silicone oil tamponade is not necessary.

[Vitrectomy in treatment of retinal detachment with proliferative vitreoretinopathy].

The authors presented their experiences concerning the treatment of 26 eyes (in 25 patients) with retinal detachment complicated by PVR in which pars plana vitrectomy and silicone oil tamponade were applied. The patients have been previously operated with traditional methods however without success. Positive results, it is retinal attachment completely or in the superior and central part of the fundus with residual detachment inferiorly were achieved in 19 eyes (73%). An useful visual acuity (> 1/50) was achieved in 12 eyes and in 7 (27%) it was better than 5/50. Our results confirmed that this method (pars plana vitrectomy with silicone oil tamponade) is a method of choice in complicated retinal detachment, in which traditional methods failed.

[Retinotomy--early personal experience].

PURPOSE: In selected cases of retinal detachment complicated by advanced proliferative vitreoretinopathy, despite vitrectomy and silicone oil tamponade, we observe reproliferations on the anterior retina. These reproliferations cause retinal redetachment. Extensive peripheral retinotomy is performed to treat these eyes. This paper shows our results after relaxing retinotomy. MATERIAL AND METHOD: Between November, 1991 and December, 1994 retinotomy was performed in 12 eyes. All eyes were treated with scleral buckling methods and vitrectomy, membrane peeling and silicone tamponade without success. Preoperative visual acuity was light perception or hand movements in 10 eyes and 0.5/50 in 2 eyes. RESULTS: Retinal reattachment was achieved in 10 cases postoperatively. Final visual acuity 5/50-5/10 was observed in 3 eyes, 1/50-4/50 in 5 eyes and light perception or hand movements in 4 eyes. CONCLUSION: Our results show that retinotomy allows successful treatment of severe proliferative vitreoretinopathy in cases previously treated with scleral buckling methods and vitreous surgery without success.
