Transplantation of mucosal tissue model composed of rabbit oral mucosal cells.

Since skin grafted into the oral cavity does not differentiate into mucosa, its original characteristics remain unchanged. This may cause discomfort to patients treated with such grafted skin, due to the skin's hair follicles and keratinization. This phenomenon is controlled by subepithelial connective tissues, and is thus referred to as the Epithelio-Mesenchymal Interaction. The grafting of mucosa itself would be preferable, but this procedure is severely limited due to the scarcity of suitable tissue. Accordingly, we have recently prepared artificial mucosa in vitro, using fibroblasts and epithelial cells derived from rabbit oral mucosa and collagen gel, and transplanted it into a donor animal. This paper is the first report of a method using artificial mucosa for the reconstruction of mucosal defects.

Evidence for existence of polyol pathway in cultured rat mesangial cells.

The accumulation of polyols has been previously found in renal glomeruli isolated from streptozocin-induced diabetic (STZ-D) rats, although the intraglomerular polyol pathway has not been exactly localized. Because we have previously observed mesangial cell dysfunction in STZ-D rats, we examined whether the polyol pathway exists in mesangial cells as a possible candidate of the cause of cellular dysfunction. The activities of two polyol pathway enzymes, aldose reductase and sorbitol dehydrogenase, were clearly detected in the crude homogenate of cultured mesangial cells at higher levels than those of whole glomeruli when DL-glyceraldehyde or D-fructose was used as substrate. When cells were incubated in medium containing 55 mM glucose or galactose, a large amount of sorbitol or galactitol was accumulated intracellularly. The accumulation of polyols was effectively blocked by an aldose reductase inhibitor, ICI 128436. These results suggest that the polyol pathway exists in mesangial cells of rat glomeruli and may play a role in the development of mesangial cell dysfunction found in STZ-D rats.

Diagnostic pattern for neuroses in China, Japan, and the United States.

To investigate the diagnostic patterns for neuroses in China, Japan, and the United States, the authors showed videotapes and brief written case histories of six Chinese patients to psychiatrists and psychiatrists-in-training in Beijing, Tokyo, and Honolulu. In cases with a well-distinguished clinical picture the diagnoses were congruent in the three countries. Diagnostic disagreement occurred in cases with symptoms of decline in mental function, which were overwhelmingly diagnosed as neurasthenia by the Chinese clinicians, and cases with situational stress, which were diagnosed as adjustment reaction by the Americans. This illustrates that different professional concepts and classification systems are used in different countries.

[A clinical study of radiotherapy with HCFU for advanced and recurrent breast cancers. Radiation and HCFU Study Group for breast cancer].

We have investigated the usefulness of combination therapy with radiation and HCFU for advanced and recurrent breast cancer according to a clinical controlled multicenter trial from 1982 to 1984. One hundred cases were registered and 82 of them were available. Treatment sites were the lymph nodes, skin, bone and lung, and the overall response rate was 58% in CR and 19% in PR, while the duration of remission was 18 weeks in CR. Side effects were found in 10% of the patients. Combination therapy with radiation and HCFU may be useful in multimodal treatment for advanced recurrent breast cancer.

[Diagnostic imagings of mediastinal mass].

Conventional postero anterior and lateral chest radiographs constitute the first examination of the mediastinum. A well-penetrating radiograph of 130KV or more is needed to optimally, demonstrate the lung-mediastinal borders as well as air-containing structures in the central thorax. Many mediastinal lesions that are obscure on conventional films are clearly demonstrated by CT, which clearly separates structures in the axial plane. The extent and localization of tumor can be more easily determined by CT than by other techniques. Conventional chest tomography has been popular in the past, but with the advent of CT scanning, this procedure has little in evaluating a mediastinal mass if CT is available. Nuclear scan using gallium 67 is fairly accurate in identifying inflammatory lesions and malignant tumors, but with the advent of CT such studies have little if any use in the mediastinum. The list of CT indications conforms somewhat to the report published by the Society of Computed Body Tomography, with some modifications. CT examination of the mediastinum has become one of the most useful complementary modalities in the diagnostic field and often not only is it complementary, but it may be an alternative to other X-ray studies, isotope scanning, or mediastinoscopy.

Coarse areae gastricae in the proximal body and fundus: a sign of gastric hypersecreton. Radiological and endoscopic correlation.

The clinical significance of coarse areae gastricae in the proximal body and fundus on the double-contrast radiograph was investigated in 60 patients. Radiographic findings were correlated with endoscopic features and biochemical analysis of gastric juice as well as with the clinical and endoscopic features of 98 controls with a regular mucosal pattern. The patients with the coarse pattern had more gastric secretion than the control group. Ulcers were seen in 65% of patients, particularly in the duodenum (48%); they were more commonly associated with a coarse pattern and closely related to gastric hyperacidity. Coarse areae gastricae in the proximal body and fundus may be a useful radiographic sign in assessment of gastric secretion.
