Biochemical analysis of breast cyst fluid as a possible predictor of breast carcinoma development.

The occurrence of breast cancer in patients with gross cystic disease is 2-5 times higher as compared to control group of women. During 3 years, 183 cyst fluid samples were analyzed in 129 females, in 30 patients of them the samples were analysed repeatedly. The distribution of the Na+/K+ ratio, considered as the measure of cancer risk, was found to be bimodal. In repeated analyses the type I cyst fluid markedly predominated (Na+/K+ < or = 4.0). A direct dependence on this ratio was found in the concentration of glucose, albumin, carcinoembryonic antigen (CEA), tissue polypeptide antigen (TPA) and its specific form, TPS; an indirect dependence was found for the level of uric acid, phosphates, alkaline phosphatase (ALP) and alpha-amylase (AMS). The predominance of apocrine metaplasia cells released into the cyst fluid is characteristic of type I cysts. A definitive assessment of significance of these parameters will be enabled by a long-term follow-up of the disease in the respective patients.

Correlation of cytosolic concentrations of ER, PS2, Cath-D, TPS, TK and cAMP in primary breast carcinomas.

In a group of 391 patients with primary breast cancer the cytosolic concentrations of ER, PS2, Cath-D, TPS, TK and cAMP were determined. PS2 production was found to be significantly dependent not only on ER but also on cAMP. A similar dependence on ER and cAMP production was found also in Cath-D. When the production of these prognostic factors was correlated with the occurrence of metastases to axillary lymph nodes, three prognostically unfavorable groups of primary breast carcinomas were revealed. The first group was represented by tumors with negative PS2 values (< or = 2.5 ng/mg) and elevated TPS values (> or = 4.0 kU/mg). The second group comprised prognostically very unfavorable tumors with moderately to highly elevated PS2 values and positive Cath-D values (> or = 35 pmol/mg), or positive TPS values (> or = 8.0 kU/mg). The third group was constituted by tumors with elevated TK (> or = 5.0 U/mg) and ER values (> or = 40 fmol/mg). The possible role of PS2 in the metastasizing of primary breast carcinomas is discussed.

Early diagnosis of breast cancer dissemination by tumor markers follow-up and method of prediction.

A mathematical model of prediction of progression was tested in patients with breast cancer employing long-term monitoring of tumor markers CEA, CA 15-3, MCA and TPA, erythrocyte sedimentation rate (FW), and the enzymes gamma-glutamyl transferase (GGT), alkaline phosphatase (ALP) and lactate dehydrogenase (LD) in serum. At the same time, specificity, sensitivity, lead time and positive predictive value were evaluated along with false positivity for all these parameters and their combinations. A model was proposed for the follow-up of patients with breast cancer after the completion of basic therapy.

[Development and interrelations between certain respiratory and biochemical parameters in normal neonates].

The authors investigated the development and interrelations of respiratory and biochemical parameters of normal neonates during the adaptation period from 15 minutes to the 7th day after delivery. They found that the correlations of oxygen consumption, carbon dioxide output, respiratory volume and minute ventilation are positive and independent on the time of assessment. These respiration parameters are independent on the glucose, lactate and pyruvate levels, on the respiratory quotient and pH and pCO2. The authors characterize the similarity of the trend of individual parameters and the variability of normal values.
