[MR tomography versus CT arthrography in glenohumeral instabilities].

In a prospective study the diagnostic value of MRI compared to CT arthrography (CT-A) was evaluated in 26 patients with 27 instable shoulder joints. Surgical and/or arthroscopic correlation was available in all cases. Both methods showed a high accuracy (96% CT-A, 94% MRI) in the evaluation of the glenoid labrum. CT-A was significantly superior to MRI in the detection of capsular lesions (sensitivity 96% vs. 44%, accuracy 96% vs. 72%, negative predictive value 96% vs. 67%). CT-A and MRI results regarding humeral head fractures were similar; fractures of the glenoid rim were better detected by CT-A, the difference, however, was statistically not significant. Overall, CT-A proved to be superior to MRI, as lesions of the joint capsule were shown with greater certainty.

[Cystic space-occupying lesions of the seminal vesicles. Their demonstration with MRT].

Between October 1990 and March 1991, 8 patients with cystic lesions in the region of the posterior bladder wall, the seminal vesicles and the prostate were examined by MRI. In all cases there was accurate characterisation of the lesion and of its anatomy. In 3 patients with abnormalities in the retrovesical space there was very good agreement with the operative findings. 4 patients were treated conservatively. In these patients there was good agreement with the findings on cystoscopy, CT and sonography.

[The value of MR tomography in acute shoulder dislocations].

24 patients up to two weeks after primary traumatic shoulder dislocation were examined at 0.5 and 1.5 T. Surgical and/or arthroscopic correlation was available in 13, CT-arthrographic correlation in 16 patients. A joint effusion allowing sufficient evaluation of the capsulolabral complex was present in 21/24 (87.5%) cases. 11/14 patients with combined dislocated detachments of the glenoid labrum and capsular lesions were subsequently operated upon. Marrow edema of the humeral head was found in 16/19 Hill-Sachs lesions and in 4/5 fractures of the greater tuberosity. Two patients presented with a lesion of the long biceps tendon associated with rotator cuff tears and were also subsequently operated upon. MRI performed shortly after primary traumatic shoulder dislocation allows a comprehensive evaluation of the intraarticular lesions and decisively influences further therapy.

[Measurement error estimation in MR tomographic volumetry of the left ventricle with the multisection technic].

A multi-slice technique for MRT measurements of the left ventricular volume is much faster than the use of single-slice methods and is therefore better tolerated, leaving time for additional measurements. The end-diastolic left ventricular volume can be reliably measured by this method (123.3 +/- 13.5 ml vs. 124.1 +/- ml). The end-systolic volume is consistently overestimated by 23.7 +/- 18.3% compared with the reference value obtained by single slice measurements (47.9 +/- 8.9 ml vs 39.1 +/- 7.9 ml). Correspondingly, stroke volume and ejection fraction is underestimated on average by 10.6 +/- 9.7% and 10.6 +/- 7.6% respectively).

[Bone marrow changes following radiotherapy. Results of MR tomography].

Magnetic resonance imaging (MRI) offers a new approach in the morphologic evaluation of the bone marrow. Physiologic and pathologic changes can be assessed with very high sensitivity. Radiotherapy induces acute depletion of the hematopoietic bone marrow, resulting in fatty degeneration. With MRI it is possible to evaluate the changes during irradiation and it also discloses the long-term fatty degeneration after radiotherapy. The irradiated bone marrow mostly exhibits a homogeneous hyperintense pattern on T1-weighted images. This allows clear recognition of the former target volumes. Our quantitative studies based on chemical shift imaging data reveal a lack of recovery of hematopoiesis after radiotherapy with 30 Gy or more. These results are independent of patients' age and of the interval after radiotherapy.

[The value of imaging techniques in the diagnosis of non-specific abdominal pain in young patients].

In a retrospective study the use of imaging modalities in the diagnosis of non-specific abdominal pain in patients under 31 years was evaluated. 253 patients with primarily non-specific abdominal complaints were analysed. For further investigation one of the following methods had to be applied: abdominal sonography and/or upper gastrointestinal tract roentgenograms and/or enteroclysis and/or barium enema. In 66% (167/253) of all cases abdominal pain remained non-specific. Clinical history and physical examination led to the final diagnosis in 66/86 (76.7%) of patients with pathologic findings. 21/561 (3.7%) radiographic examinations revealed abnormalities of clinical importance. In 10/253 (4%) patients the final diagnosis could be established only with the help of radiologic and/or endoscopic examinations. The low efficacy of conventional radiology justifies the demand for a stricter indication in the young patient suffering from non-specific abdominal pain.

[Intestinal contrast imaging in abdominal computed tomography: water or contrast medium?].

The suitability of water as an oral or rectal contrast medium for abdominal CT was studied in 56 patients and compared with an iodine-containing water-soluble contrast medium (ioxital amino acid). In some cases it was impossible to differentiate gastrointestinal structures from extraluminal fluid collections (cystic tumours, ascites, abscesses) and there was poor filling of distal small bowel and colon. The routine use of water can, therefore, not be recommended. In some cases, however, water can result in improved image quality by reducing artifacts and improving the demonstration of the mucosa.

[CT arthrography of the shoulder after surgical repair for ventral instability].

25 patients who had undergone a modified Lange repair were examined postoperatively after about four years, both clinically and by CT arthrography. Ventral redislocation was no longer seen postoperatively. As regards residual ventral instability, there was agreement between CT arthrography and the clinical findings in 20/25 patients (90%). Newly developed or residual dorsal instability could be accurately demonstrated by the presence of pathological changes of the dorsal labrum and by the observation of a wide dorsal joint recess. CT arthrography did not provide reliable evidence concerning loss of function and mobility of the treated shoulder. However, the presence of marked capsular folding suggests a poor clinical result.

[Interpretation of MR tomography findings in trauma surgery].

MR-Imaging is well established as the most sensitive method for the diagnosis of avascular necrosis of the femoral head. In this paper preliminary results are analyzed in an attempt to clarify whether as in a vascular necrosis of the femoral head. MR changes after trauma or surgery allow conclusions concerning the vitality of the bone. Comparison of clinical, MR and histological findings revealed that we cannot yet differentiate between fibrosis of the bone marrow with sustained vitality of the osteocytes and bone necrosis. Therefore, conclusions with implications for therapy should be drawn with extreme care.

[Color-coded Doppler sonography of primary and secondary liver tumors].

50 Patients with 78 focal liver lesions were examined via colour-coded Doppler system to study the vascularity of metastatic lesions and primary benign and malignant liver tumours. According to the amount of detectable colour Doppler signals, tumour vascularity was graded into 4 types. Metastases (n = 40) and cavernous haemangiomas (n = 12) seemed to be avascular with no evidence of increased vascularity (Type I/II). Conversely primary liver cancer (PLCA) (n = 7) and focal nodular hyperplasia (FNH) (n = 16) mainly produced arterial Doppler signals within the tumour (Type III/IV). AV shunts and partial portal vein thrombosis could be demonstrated in PLCA. Doppler colour flow imaging for detection of tumour vascularity may thus add to the specificity of ultrasonic tumour diagnosis.

[CT arthrography in the diagnosis of shoulder instability].

In the course of a prospective study, CT arthrography was carried out on 40 patients with shoulder instability. In 35 of these patients a lesion of the capsule and labrum was demonstrated, indicating glenohumeral instability; in three patients this was shown primarily by CT arthrography. In three patients, multidirectional instability was demonstrated. In four patients there was an isolated lesion of the labrum, whereas in one patient all the findings were normal. Compared with the capsulo-labral lesions, bone changes involving the head of the humerus and the glenoid were of lesser importance, although their severity affected the choice of operative treatment in four patients. Fifteen of the 40 patients have undergone surgery so far and in all cases the findings on CT arthrography could be confirmed.

[Spontaneous intramural hematoma of the intestinal wall--an unusual hemorrhagic manifestation of hemophilia A].

Gastrointestinal bleeding is a well known event in patients suffering from blood dyscrasias. In contrast spontaneous nontraumatic hematoma of the bowel is a rare complication, requiring a different therapeutic management, which depends on localization and extent of the hematoma. We report about two hemophiliac patients presenting with acute abdominal pain caused by spontaneous intramural hematomas.

[Possibilities of computerized arthrotomography in the diagnosis of shoulder instabilities].

Shoulder instability can usually be diagnosed by examination of the history and a careful physical examination. Additional diagnostic studies include radiographic investigations and arthroscopy as a functional and very comprehensive diagnostic procedure for shoulder lesions. Conventional radiographic investigations with special projections have proven to be reliable in revealing bony changes of the glenoid and of the humeral head, whereas CT arthrography is very helpful in evaluating capsulolabral lesions associated with shoulder instability. This kind of investigation is a highly sensitive and less invasive technique. Combined with intraarticular application of contrast medium the extent of pathologic changes associated with instability can be imaged and differentiated from other intraarticular causes of dysfunction. During a 1-year period, 42 patients were studied by double contrast arthrography followed by CT examination, and 33 were found to have a shoulder instability. The intention of this report is to describe a few CT arthrographic findings in the presence of pathologic anatomy of unstable shoulders.

[Duplex sonography of the portal vein. Procedure in healthy probands and patients with portal hypertension].

Duplex ultrasonography is a useful non-invasive means of investigating portal vein blood flow in cases of liver disease. A group of 50 selected consecutive patients with portal hypertension revealed a significant increase in portal vein diameter of 3.5 mm on average, and a significant decrease in mean flow rate of, on average, 3.1 cm/s, in comparison with a group of 50 healthy control subjects. In contrast, alcohol-induced portal hypertension appeared always to be associated with an elevated flow rate. On average, the flow volume increased by 200 ml/min. With the exception of vessel diameter, which remains virtually unchanged, both patients and controls revealed, postprandially, a comparable increase in the parameters measured.

[The value of radiation therapy in the management of adenoid-cystic carcinoma of the head and neck].

There are different opinions about the importance of radiotherapy in the treatment of adenoid-cystic carcinomas. A report is given on the experiences gained with highdose radiotherapy in twelve patients suffering from adenoid-cystic carcinomas. In spite of the unfavorable composition of the group of patients (6/12 were classified stage T3 or T4, 5/12 were only irradiated after a local recurrence had appeared), local tumor control was achieved in all patients by percutaneous irradiation with a focal dose of 80 Gy or interstitial irradiation with a focal dose of 70 Gy. On the basis of these results as well as of recent communications in literature, it is concluded that adenoid-cystic carcinomas can be locally cured by radiotherapy.
