The management of metformin overdose.

Only two cases of deliberate self-poisoning with metformin have been described in the literature. We report the management of three cases, all of which presented with severe lactic acidosis. Two cases had a fatal outcome. The options for treatment and previous cases reported in the literature are reviewed. Recommendations for the management of metformin overdose include early recognition, intensive support of the cardiovascular system, the correction of metabolic acidosis and the maintenance of body temperature. The use of venovenous haemofiltration against a non-lactate containing fluid may improve outcome when sodium bicarbonate alone fails to reverse severe systemic acidosis.

Tension pneumothorax during ventilation via Ayre's T-piece.

The occurrence of a pneumothorax in a 4-week-old infant is described. The cause of the problem is believed to have been a kink in the silastic reservoir tube. The failure of the monitors to detect the event and methods of improving safety are discussed. Difficulties may occur even with familiar equipment; the value of full monitoring and constant vigilance are stressed.

General anaesthesia in accident and emergency departments.

Potential problems with the provision of general anaesthesia in accident and emergency (A&E) departments were investigated by sending consultants in A&E a postal questionnaire asking for their views on the quality of the anaesthetic service available to their department. Of the 129 consultants replying, 31% had problems in obtaining an anaesthetist, 17% thought the first on-call anaesthetist was too inexperienced and 5% had experienced various other problems. Overall, 20% were unhappy with the service.
