Pharmacokinetics of the antimicrobial agents rifampicin and miconazole released from a loaded central venous catheter.

The time course of rifampicin and miconazole concentrations after insertion of a polyurethane catheter loaded with these antibiotics were studied. Data from controlled release experiments in vitro were used, and the concentration time courses of the antimicrobials in serum were calculated by pharmacokinetic simulations. Systemic therapy using typical dosages (rifampicin 600 mg/day iv, miconazole 3 x 200 mg/day iv) results in rifampicin concentrations between 54 and 8424 microg/L, and miconazole concentrations between 3567 and 4676 microg/L. After insertion of a polyurethane catheter loaded with these antibiotics, the maximal concentrations after catheter placement were determined as 6 microg/L at 10.7h for rifampicin, and 13 microg/L at 28.6 h for miconazole. Assuming that the total amount of antibiotics incorporated in the catheter matrix were bioavailable ('worst case'), the resulting maximal concentrations calculated by simulation are 10 microg/L for rifampicin and 65 microg/L for miconazole. Maximal concentrations of rifampicin or miconazole resulting from the insertion of a polyurethane catheter loaded with these antibiotics are, therefore, far below the concentrations resulting from a systemic therapy with the same antimicrobial agents. Even in the worst case, the danger of selecting resistant bacterial strains seems remote because the systemic drug levels are magnitudes of order below subinhibitory concentrations.

Studies on the role of superoxide anion radicals for the cardiotoxicity of adrenochrome.

Adrenochrome is an oxidative product of adrenaline and possesses cardiotoxic properties. As oxygen free radicals play a role in the cytotoxic effects of catecholamines, the role of superoxide anion radicals, as mediators of adrenochrome toxicity, was investigated using electrically-driven Langendorff rabbit hearts with depleted catecholamine stores. Repetitive regional myocardial ischemia (MI) was induced by coronary artery branch ligature, and MI was quantitated from epicardial NADH-fluorescence photography. Adrenochrome (10(-6) mol/l) was added to the perfusion solution after a reperfusion period of 20 min, 30 min before the 2nd coronary occlusion, with or without the additional application of SOD (30 U/ml). Left ventricular pressure was significantly enhanced by adrenochrome (p < 0.05), but it fell rapidly down below its initial value (p < 0.05). Coronary flow was significantly decreased by adrenochrome (p < 0.05). Whereas epicardial NADH-fluorescence was similar after repetitive coronary occlusions in untreated controls, it was significantly enhanced by adrenochrome (p < 0.05). The deleterious effects of adrenochrome on MI were not inhibited by SOD. Thus, there is no evidence for superoxide anion radicals as mediators of the deleterious effects of adrenochrome on MI in isolated rabbit hearts. The deleterious effects of adrenochrome on MI in isolated rabbit hearts might be caused by functional effects, impairing the oxygen consumption/oxygen supply balance.

[The antitoxic action of triamterene in cardiac glycoside poisoning].

The antagonistic effect of triamterene on the toxicity of cardiac glycosides was investigated in conscious rabbits using the infusion method. Pretreatment with triamterene (5 mg/kg and 10 mg/kg) significantly reduces the ouabain toxicity indicated by an increase of the dose producing arrhythmia (from 61 +/- 14 to 121 +/- 17 and 178 +/- 22 micrograms/kg, resp.) and lethality (from 114 +/- 18 to 236 +/- 5 and 329 +/- 11 micrograms/kg, resp.). The triamterene induced increase of plasma potassium concentration may contribute to the antitoxic effect, however, the effect persisted after the decrease of plasma potassium concentration (by addition of NaHCO3) to pretreatment values. Under the influence of digitoxin the antitoxic effect of triamterene (10 mg/kg) is also demonstrated by the delayed appearance of arrhythmias (113 +/- 3 min compared to 78 +/- 5 min) and of lethality (125 +/- 4 min compared to 92 +/- 6 min). Triamterene is not only a prophylactic but also a curative antitoxic agent in the digitalis intoxicated rabbit.

The mechanical atrioventricular time of isolated hearts as a correlate of the atrioventricular conduction time.

The time difference between onset of contraction in right atrium and left ventricle of isolated guinea pig hearts (mechanical AV time) was measured continuously by an impulse-triggered interval counter and taken as an equivalent of the electrophysiological AV conduction time derived from the epicardial ECG. Excellent correlation between these two parameters was established over a broad range of experimentally modified conduction times. The method was used to analyze the influence of electrical pacing, epinephrine, Ca2+ ions, and adenosine both on the AV conduction and on functional parameters of the hearts and revealed advantages in comparison to separate tests of single parameters in different experimental arrangements.

Influence of carbocromen on some metabolic parameters of isolated working guinea pig hearts.

The influence of carbocromen (chromonar) on both functional and some metabolic parameters of isolated guinea pig hearts was tested over a broad concentration range and with different exogenous substrates. As revealed from a shift of the myocardial respiratory quotient, carbocromen (in the upper coronary active concentration range) induced an increased glucose utilization at the expense of FFA oxidation. Theoretically, this may indicate a relative reduction of myocardial oxygen demand, but the therapeutic relevance of this observation still remains obscure, as an additional exogenous FFA supply may override this biochemical carbocromen effect.

Heat production and oxygen consumption of the isolated rabbit heart: their relation to mechanical function.

A method is described for simultaneous determination of the heat production and the oxygen consumption of the isolated, isovolumetrically beating rabbit heart. The perfusion of the heart was performed via the aorta at a constant flow rate with carbogen saturated Tyrode's solution at a temperature of 37.0 degrees C. Heart function was varied by stepwise augmentation of the left intraventricular volume (LVV) by means of a balloon catheter. The following mechanical parameter of heart function were determined: enddiastolic pressure (EDP), peak pressure (PP), developed pressure (DP), max. contraction and relaxation velocity (dP/dt-A and dP/dt-B), enddiastolic tension (Tens-EDP), peak tension (Tens-PP), developed tension (Tens-DP), and circumferential tension (Tens-Cir). DP, dP/dt-A and dP/dt-R showed a maximum response to changes of the LVV at 2.0 ml LVV and 19.3 mm Hg EDP. Heat production (H) and oxygen consumption (Q) were correlated closely to mechanical function and to each other (r = 0.89, n = 8). The ratio H/Q was 4.9 cal/ml O2 and remained constant during the experiment. The myocardial energy consumption was significantly correlated toall contraction parameters with the best fit to DP and Tens-Cir (r = 0.934 and 0.933 resp.). On the basis of the calculated mean regression lines, the function-independent and the function-dependent energy consumption were calculated.
