The value of preoperative mammograms in nononcologic breast surgery.

It remains unclear whether preoperative mammograms are necessary in young patients having nononcologic breast surgery. Various authors have recommended mammography in all such cases, in conflict with guidelines from the American Cancer Society. We conducted a retrospective study to determine the utility of preoperative mammograms in young women. From 1989 through 1994, 208 patients under 35 years of age had elective breast surgery in our institution. Fifty-one (24.5%) had preoperative mammography. No malignancies were diagnosed. We believe mammograms are not routinely needed before elective breast surgery in patients under 35 years of age.

Pyogenic versus pseudopyogenic granulomas.

Surgeons are confronted daily with a myriad of skin pathology. Pyogenic and pseudopyogenic granulomas are certainly among the differential diagnoses to many nonhealing dermal lesions. This article revisits two types of vascularized lesions that are perhaps part neoplastic and part inflammatory/reparative. The differences clinically between these two types of lesions are few. The classical form generally is found along mucosal surfaces and upper extremities. The pseudopyogenic granuloma is more often found on the scalp and ears. Confirmation in their diagnoses is based strictly on their histological criteria. The management of these two types of lesions is identical. Excisional biopsies for all nonhealing lesions are warranted to rule out lesions with an ominous progression.

Dermal cylindroma (turban tumor). Case report.

We present an unusual case of massive dermal cylindroma (turban tumor), occupying the entire scalpand forehead. It was treated by a total scalping procedure and skin grafting in two stages. Additional tumors of the face, neck, chest, shoulders, and back were excised and closed. The nose was treated by shoving and dermabrasion, similar to a rhinophyma. The lip was treated by dermabrasion. There has been no evidence of recurrence in the scalp which was treated by excision and grafting. There is now a papular quality of the skin over the nose and on the uppler lip, indicating that regrowth may occur. The etiology, pathology, and a review of the literature are presented.
