[Repair of two cases of partial anomalous pulmonary venous return by draining total right pulmonary venous blood to the left atrium through dilated atrial septal defect].

Two cases of partial anomalous pulmonary venous return were reported in whom right pulmonary veins were totally drained to the coronary sinus and to the right atrium respectively. Re-routing of pulmonary veins was done by dilated atrial septal defect and making intra-atrial tunnel using equine pericardial patch. Postoperative courses were uneventful. MRI was useful to identify the pathway from right pulmonary veins to the left atrium.

[Effect of aortic valve replacement on left ventricular function in adult patients with aortic stenosis].

Echocardiographic and hemodynamic studies were obtained in 16 consecutive adult patients who underwent aortic valve replacement (AVR) with St. Jude Medical valve for aortic stenosis (AS). Three cases of congenital AS was included and two of them had undergone aortic valvotomy in childhood. One of 16 patients died due to late cardiac tamponade six weeks after AVR. Postoperative studies showed improved left ventricular (LV) functions. LV end diastolic and end systolic diameter (LVDs and LVDs) fell from 50.3 and 38.2 to 44.6 and 31.6 mm respectively (p less than 0.05). Fractional shortening (%FS) rose from 26.5 to 32.2% (p less than 0.05). End systolic wall stress (ESWS) fell from 126.2 to 69.6 k dynes/cm2 (p less than 0.01). Cardiac index and pulmonary arterial wedge pressure improved from 3.4 and 14.4 to 3.6 l/min/m2 and 10.5 mmHg respectively (ns). Preoperatively, six were functional class II, eight were class III and one was class IV (New York Heart Association classification). Postoperative improvement was as follows, eight: class I, seven: class II. In four cases, preoperative echocardiography revealed most depressed LV function in %FS (smaller than 21%) and ESWS (greater than 140). Postoperatively they improved from 18.3 and 164 to 26.0% and 72.8 k dynes/cm2 respectively. These results suggested that depressed LV function in the patients with longstanding AS was largely related to limited preload reserve due to LV enlargement and mechanical unloading of LV (correction of afterload mismatch) resulted in improvement of LV function. In conclusion, LV dysfunction owing to AS alone is reversible and AVR results in great clinical improvement.

A sequence preference for nucleation of alpha-helix--crystal structure of Gly-L-Ala-L-Val and Gly-L-Ala-L-Leu: some comments on the geometry of leucine zippers.

The synthetic peptide Gly-L-Ala-L-Val (C10H19N3O4.3H2O; GAV) crystallizes in the monoclinic space group P21, with a = 8.052(2), b = 6.032(2), c = 15.779(7) A, beta = 98.520(1) degree, V = 757.8 A3, Dx = 1.312 g cm-3, and Z = 2. The peptide Gly-L-Ala-L-Leu (C11H21N3O4.3H2O; GAL) crystallizes in the orthorhombic space group P212121, with a = 6.024(1), b = 8.171(1), c = 32.791(1) A, V = 1614 A3, Dx = 1.289 g cm-3, and Z = 4. Their crystal structures were solved by direct methods using the program SHELXS-86, and refined to an R index of 0.05 for 1489 reflections for GAV and to an R index of 0.05 for 1563 reflections for GAL. The tripeptides exist as a zwitterion in the crystal and assume a near alpha-helical backbone conformation with the following torsion angles: psi 1 = -150.7 degrees; phi 2, psi 2 = -68.7 degrees, -38.1 degrees; phi 3, psi 32 = -74.8 degrees, -44.9 degrees, 135.9 degrees for GAV; psi 1 = -150.3 degrees; phi 2, psi 2 = -67.7 degrees, -38.9 degrees; phi 3, psi 31, psi 32 = -72.2 degrees, -45.3 degrees, 137.5 degrees for GAL. Both the peptide units in both of the tripeptides show significant deviation from planarity [omega 1 = -171.3(6) degrees and omega 2 = -172.0(6) degrees for GAV; omega 1 = -171.9(5) degrees and omega 2 = -173.2(6) degrees for GAL]. The side-chain conformational angles chi 21 and chi 22 are -61.7(5) degrees and 175.7(5) degrees, respectively, for valine, and the side-chain conformations chi 12 and chi 23's are -68.5(5) degrees and (-78.4(6) degrees, 159.10(5) degrees) respectively, for leucine. Each of the tripeptide molecule is held in a near helical conformation by a water molecule that bridges the NH3+ and COO- groups, and acts as the fourth residue needed to complete the turn by forming two hydrogen bonds. Two other water molecules form intermolecular hydrogen bonds in stabilizing the helical structure so that the end result is a column of molecules that looks like an alpha-helix.

A new method of the measurement of nasal secretion in guinea pigs.

A simple quantitative method to measure nasal secretion in guinea pigs is described. Nasal secretion was measured with a piece of cotton thread dyed with fluorescein at one end which was inserted into an anterior naris and kept there for 60 s. The stretch of color of a thread dyed with fluorescein was proportional to fluid volume and to increase in weight of a thread due to absorbed nasal secretion induced by nasal provocation. In addition, the stretch of color due to nasal secretion was associated with the score of rhinorrhea. Thus, it is considered that the amount of nasal secretion can be reflected to the length of the stretch of color. Each secretion on the ipsilateral and the contralateral sides induced by nasal provocation could be separately measured by this method. The amount of nasal secretion induced by allergen in passively sensitized guinea pigs could be reduced by pretreatment with ketotifen or flutropium. These results suggest that our method may serve as a quantitative test for nasal secretion in guinea pigs, which would be useful in the study of hypersecretory response in the allergic model or in evaluating the effect of antiallergic drugs on nasal allergy.

Nasal mucosal hypersensitivity in guinea pigs intermittently exposed to cold.

To develop an animal model for experimental nasal hypersensitivity and hyperreactivity, guinea pigs were subjected to intermittent exposure to cold temperature (intermittent cold stress, SART stress) for 5 consecutive days. In SART-stressed guinea pigs, nasal mucosal hypersensitivity to histamine evoking sneeze response and nasal hypersecretion in response to methacholine were observed. The hypersensitivity remained for further 7 days after being released from SART stress. On the other hand, such nasal mucosal hypersensitivity was not caused by a continuous cold stress alone, suggesting that intermittent exposure to cold may be of importance for the appearance of nasal mucosal hypersensitivity. In passively sensitized SART-stressed guinea pigs, the quantity of nasal secretion induced by an allergen was significantly increased compared with that of a group of normal animals. The expression of muscarinic acetylcholine receptor (m-ACh.R) became higher in SART-stressed guinea pigs. Thus, hypersensitivity and hyperreactivity in this system were found to be associated with an increase in density of m-ACh.R. SART-stressed guinea pigs will serve as an animal model for hypersensitivity in nasal mucosa, which would be useful for the study of nasal allergy.

[Auditory brain stem response in patients with consciousness disturbance].

To investigate the prognosis for patients with consciousness disturbance, we recorded auditory brain stem response (ABR) in 35 patients with primary or secondary brain damage. For patients with secondary brain damage, the first ABR recorded after admission could be used not only to differentiate consciousness level but also to predict prognosis. However, for the patients with primary brain damage, the first ABR could not be used for either differentiating consciousness level or predicting prognosis. In addition, in two patients with secondary brain damage, the series of ABR recordings indicated that brain stem damage was reversible.

Design of crystalline helices of short oligopeptides as a possible model for nucleation of alpha-helix: role of water molecules in stabilizing helices.

We have designed, synthesized, crystallized, and performed x-ray analysis of several hydrophobic tripeptides that show an extended near alpha-helical structure in the crystalline state. All of the tripeptides that show this remarkably stable helix crystallize with two or three water molecules; they all have glycine at the N terminus and have increasing hydrophobicity as one moves from the N to C terminus. Even though three residues in the oligomer are not sufficient to complete a turn, one of the water molecules acts as an added residue and links up adjacent tripeptide segments along the helix axis so that in the crystal, the helix appears effectively as one long continuous helix. Two of these tripeptides are stabilized by two water molecules that enable the peptides to complete a turn of the helix and extend the helical structure throughout the crystal by linking translationally related peptides by hydrogen bonds. In two other peptides, these roles are played by three rather than two water molecules. Though these tripeptides have different crystal symmetry, they all show the basic pattern of hydrated helix and packing, indicating the strong conformational preference for a stable structure even for these tripeptides. Such conformationally stable hydrated structures for short specific related sequences illustrate their possible importance in nucleating protein folding and in the role water molecules play in such events.

Unilateral dermatomal superficial telangiectasia.

A three-year-old boy with unilateral dermatomal superficial telangiectasia (UDST) is the first reported case with skin lesions in two non-adjacent areas (dermatomal areas of Trigeminal2 and L4-5). In this case, onset occurred at an early age, the onset was eight months after an operation, and the liver function was normal; therefore we believe it is congenital, even though the patient had undergone an extensive right hepatectomy to remove a hamartoma in his liver at the age of four months.

[Effect of neurotropin on experimental osteoarthritis].

The effect of Neurotropin on osteoarthritis was investigated in comparison with those of prednisolone and indomethacin. 1) There were remarkable decreases in the staining intensity to safranin-O and in the contents of uronic acid, total hexosamine and hexose in the articular cartilage of rabbits in which experimental osteoarthritis was induced by the injection of papain into the knee joint. In the Neurotropin-treated group, the decrease in the staining intensity to safranin-O and the contents of uronic acid, total hexosamine and hexose were evidently recovered. On the other hand, in the prednisolone- or indomethacin-treated group, the degeneration of the cartilage was even more pronounced than in the control group treated with papain alone. 2) Neurotropin had no effect on the autolytic degradation of cartilage, but promoted the incorporation of 14C-acetate into the proteoglycan in the articular cartilage of rabbits. 3) Both prednisolone and indomethacin inhibited the autolytic degradation and the incorporation of 14C-acetate into the proteoglycan. These results suggested that the therapeutic effect of Neurotropin on osteoarthritis may be due to the improvement of decreased proteoglycan content in the matrix of articular cartilage; and in this respect, it is different from anti-inflammatory drugs such as prednisolone and indomethacin.

[Correlation between morphologies of mandibular vertical sections and linear measurements of the skull].

The purpose of this study is to evaluate the relationships between the sectional forms of the mandible and the craniofacial morphology. Vertical sections were made at the symphysis (SY) and at the second molar region (M2R). Materials were dried human skulls which were measured in three dimensional co-ordinate system. The correlation matrix of 24 measurements was examined in the male and the female. The difference of these measurements between the low angle group and the high angle group of gonial angle was also examined. Results obtained were as follows: 1. Gonial angle was negatively correlated with the height, width, breadth of ramus and the breadth of second molars. SY form was related to the facial height and size of the mandible. On the other hand, M2R form was related to the height, width and breadth of ramus. 2. The differences of measurements between the low angle group and the high angle group of gonial angle were as follows: 1) The ramal height, ramal width, bicondylar width and breadth of second molars in the low angle group were significantly larger than those in the high angle group. 2) The measurements of SY in the high angle group and the low angle group showed no significant difference. 3) The height and area of M2R in the low angle group were significantly larger than those in the high angle group. This study demonstrated that SY form might relate to vertical factors of the face (facial height), the size of mandible and the types of occlusions. On the other hand, M2R form being nearer to the attachment of masseter and medial pterygoid muscles than SY was related to the form of ramus and the gonial angle.

[Morphological changes in the dental arch and the palate during orthodontic treatment].

The changes of the dental arch form after the tooth movement and the sequential changes occurred on the palate during orthodontic treatment were examined in the coordinate system based on the standard point at the palatal vault. The subjects selected for this study were 20 patients of Angle Class II div. 1. Both left and right maxillary premolars were extracted and an edgewise appliance had been treated. The plaster models prepared before and after treatment were used for materials. Only suitable materials were measured as a result of an examination of the reproducibility of the palatal core at the palatal vault. Results obtained were as follows: 1. It was suggested that the dental arch and the palate was mutually effected on each other in the arch length and the arch width. 2. The morphological changes in the dental arch and the palate due to orthodontic treatment was discussed on the basis of the results of Fourier analysis. The dental arch and the area from F division to J division were significantly smaller after orthodontic treatment than before it. The shape became closer to a semicircle. 3. Values representing morphological changes in the dental arch and J division were similar and values representing morphological changes in the other division were getting smaller as the area being close to the palatal vault.

[Replacement of omniscience mitral valve in suboptimal opening].

This 57-year-old female of MS had been treated by MVR with a 25 mm Omniscience valve on October 26, 1983. She was doing well until November 1984 when she developed general malaise and dyspnea. She was hospitalized again on June 3, 1985. Studies including echocardiogram and cardiac catheterization revealed mitral stenosis. The echocardiogram showed rapid blood stream toward the interventricular septum. The maximum opening angle of the disc was measured 30 degrees on the cine film. The cardiac catheterization showed increased PA and PA wedge pressures. The Omniscience valve was replaced with a 25 mm SJM valve on July 19, 1985. The postoperative course was uneventful. The postoperative echocardiogram and cardiac catheterization showed significant improvements in flow pattern and intracardiac pressure. She is doing well as an instructor of Japanese dancing after discharge from the hospital.

[Enhancing effects on interferon-gamma production by combination of OK-432 and interferon-alpha in patients with head and neck cancer].

Interferon (IFN)-gamma, though having the same titer as that of IFN-alpha or -beta, is several time more effective against cancer. However, it is impossible to obtain IFN-gamma as a drug at present. OK-432 is known to be an inducer of IFN-gamma, but the titer of IFN-gamma is very low in the blood after injection of OK-432. Therefore, for the purpose of high titer IFN-gamma production, a combination of OK-432 and IFN-alpha was investigated. In 30 patients with head and neck cancer, OK-432 was administered singly or combined with IFN-alpha at intervals of 2 weeks, and the amount of IFN in the blood induced by OK-432 was compared. In the group of cases combined with a high titer of IFN-alpha (5 x 10(6) units or 3 x 10(6) units), enhancement of IFN production induced by OK-432 was noted in 9 out of 20 cases. In the group treated with a low titer of IFN-alpha (7 x 10(3) units), no enhancement effect was seen. IFN induced by OK-432 contained both gamma and alpha types, as did the IFN enhanced by the combination with IFN-alpha.

Modified steroid hormones. 7. 4-Fluoro-17 beta-estradiol: carbon-13 nuclear magnetic resonance, crystal and molecular structure, and biological activity.

The 4-fluoro analogue of 17 beta-estradiol was investigated for estrogenic action in rats by determination of uterine hyperemia and [3H]uridine incorporation and was found to be an active estrogen. Antitumor activity of this analogue was demonstrated against 7,12-dimethylbenz[a]anthracene (DMBA) induced rat mammary adenocarcinoma. Its 13C NMR spectrum was determined, and all signals were assigned. A detailed X-ray diffraction investigation of 4-fluoro-17 beta-estradiol was carried out, and it crystallized in triclinic space group P1 with two steroids and one methanol in a unit cell of a = 7.367 (1), b = 9.363 (6), c = 12.531 (1) A, alpha = 89.31 (3), beta = 93.38 (1), gamma = 109.62 (3) degrees, V = 812.8 A3, and Z = 2. The structure was solved and refined to an R index of 0.062 using 3045 reflections measured on an automated diffractometer. The oxygen atoms at C(3) and C(17) at either ends link the molecules together in a head to tail fashion. The hydroxy groups of the solvent molecules also take part in linking the molecules sideways through the hydroxy groups.
