Distinguishing the roles of the two different cGMP-binding sites for modulating phosphorylation of exogenous substrate (heterophosphorylation) and autophosphorylation of cGMP-dependent protein kinase.

The role of each of the two different cGMP-binding sites (referred to as slow and fast sites) of type I cGMP-dependent protein kinase (PKG) in altering the rate of catalysis of phosphorylation of exogenous substrates (heterophosphorylation) or the rate of autophosphorylation has not been resolved. In the present study, the cGMP concentration required for half-maximal activation (A(50)) of wild-type PKG type Ibeta (WT) was 5-fold higher for heterophosphorylation than for autophosphorylation. cGMP occupation of the slow site was associated with an increase in the autophosphorylation rate, whereas occupation of the fast and slow site together was associated with a decrease in the autophosphorylation rate compared with the rate observed with occupation of the slow site alone. The contributions of each cGMP-binding site were investigated using PKG mutants containing substitutions of an invariant threonine residue that is critical for high affinity cGMP-binding in each site. Site-directed mutagenesis of Thr-317 of the fast site (T317A) increased the cGMP A(50) for heterophosphorylation 4-fold at 30 degrees C, with nominal effect on cGMP A(50) for autophosphorylation compared with WT. The analogous slow site mutation (T193A) increased the cGMP A(50) for heterophosphorylation and autophosphorylation 32- and 64-fold, respectively. Compared with WT, the cGMP A(50) of the double mutant (T193A/T317A) for heterophosphorylation was increased 300-fold, whereas the cGMP A(50) for autophosphorylation was similar to that of T193A. Thus, occupation of both cGMP-binding sites of PKG is required for maximal stimulation of heterophosphorylation, whereas occupation of the slow site alone is sufficient for stimulation of the rate of autophosphorylation, and additional occupation of the fast site reduces this rate.

The development of clinical indicators--the impact on midwifery practice in Queensland in the future.

Perineal trauma is the most frequently reported complication of labour and delivery in the Qld Perinatal Data Collection. It is documented that some individual accoucheurs seem to be particularly skillful in assisting at birth in a way that minimises perineal trauma. Recent trends within the Qld health industry have emphasised the importance of quality improvement activities focussing on health outcomes. Midwives accoucheur most of the "normal" deliveries in public hospitals, thus it is important that midwives play a leading part in the development and improvement of tools which identify quality outcomes and pinpoint areas for improvement. This study aims to benchmark current practices regarding perineal outcomes. Moreover, the authors aim to initiate discussion towards the development of the first clinical indicators appropriate to midwifery care.

Ligand-induced conformational changes in cyclic nucleotide phosphodiesterases and cyclic nucleotide-dependent protein kinases.

Three methods have been used to assess the conformational effects associated with ligand binding to two unrelated cyclic nucleotide receptor proteins: the cGMP-binding, cGMP-specific phosphodiesterase (cGB-PDE or PDE5A) and the cGMP-dependent protein kinase (PKG). The methods should be applicable to other proteins and to other types of modification such as phosphorylation. The procedures use either ion-exchange chromatography, size-exclusion chromatography, or native gel electrophoresis of these proteins in the absence and presence of regulatory ligands. Measurements from these respective approaches allow documentation of changes in the quaternary structure, surface electronegativity, and relative compactness (Stokes radius) of the protein molecule. The combined data allow the changes in protein conformation to be quantitated in terms of alterations in the axial ratio or length/width dimension of the molecule. The methods can be applied to partially purified proteins and to proteins that are available in limited quantities. Conformational changes due to stable modifications of proteins can be potentially examined in crude extracts of intact cells. Each of the methods can be tailored to optimize resolution of a particular protein under a variety of conditions. Activity measurements, Coomassie brilliant blue or silver staining of gels, radioautography, or Western blot analysis can be used for detection of the protein.

Continuous quality improvement: status of Canadian health care organizations.

The Canadian Council on Health Services Accreditation (CCHSA) surveyed its client groups to determine how involved they had become in continuous quality improvement (CQI) activities. This was a follow up to a 1992 survey. Most of the participating organizations indicated they had adopted a CQI philosophy. A major factor influencing this decision was the use of the CCHSA's client-centred standards. This article outlines the gains organizations have realized by implementing CQI, and the extent to which organizations have implemented CQI principles, methods and tools. The information from this survey has been used to develop the accreditation program for the year 2000--"The AIM Project: Achieving Improved Measurement."

Arginine 75 in the pseudosubstrate sequence of type Ibeta cGMP-dependent protein kinase is critical for autoinhibition, although autophosphorylated serine 63 is outside this sequence.

Autoinhibitory domains in many protein kinases include either a phosphorylatable substrate-like sequence or a pseudosubstrate sequence. This study shows that Ibeta cGMP-dependent protein kinase (cGK) autophosphorylates Ser-63, which is in an atypical cGK substrate sequence (-59AQKQAS-) that is amino-terminal to the pseudosubstrate motif (-74KRQAI-). cGMP increases the rate of autophosphorylation (approximately 0.8 phosphate/cGK monomer) approximately 3-fold. Autophosphorylation is an intramolecular process since it is independent of cGK concentration. cGMP activation of cGK enhances proteolysis within and near the pseudosubstrate site; treatment of dimeric cGK with three proteases produces three cGK monomers (approximately 67-70 kDa each). Their amino-terminal sequences are 75RQAISAEPT-, 76QAISAEPTAF-, and 86DIQDLSXV-, respectively. cGMP stimulates these kinases by 10-, 2.5-, and 1.4-fold, respectively, compared with a 10-fold effect on intact cGK. Increased basal activity accounts for the diminished stimulation. Thus, the primary autophosphorylation site of Ibeta cGK is well outside the pseudosubstrate site, but Arg-75 in the pseudosubstrate site is critical for autoinhibition. Autoinhibition also involves elements that are carboxyl-terminal to Arg-75.

Developmental pathways to schizophrenia: behavioral subtypes.

This study examined childhood behavior problems in schizophrenic patients and their healthy siblings. Childhood Behavior Checklist (T. Achenbach, 1991) ratings were obtained from retrospective maternal reports, for 4 age periods: birth to 4 years, 4 to 8 years, 8 to 12 years, and 12 to 16 years. The results indicated that the patients had a variety of childhood behavior problems when compared to their siblings and that the various types of problems differed in their developmental course. Cluster analysis was conducted on the childhood behavior ratings for the schizophrenic patients, and 2 subgroups emerged. Cluster I showed more pronounced behavioral problems than Cluster II, and some of these problems were apparent in early childhood and increased with age. Cluster I also demonstrated greater neuromotor abnormalities in childhood.

Childhood emotional expressions, educational attainment, and age at onset of illness in schizophrenia.

This study examined the relation between preschizophrenic subjects' facial expressions of emotion in childhood home movies and two criterion variables: educational level (highest grade completed) and age at onset of illness. Earlier research suggest that premorbid affective blunting is associated with an earlier onset of illness and poorer prognosis in schizophrenia. It was, therefore, predicted that lower rates of both positive and negative facial expressions would be associated with lower levels of educational attainment and earlier age at onset of illness. The results indicated that childhood emotional expressions were not associated with educational level but were linked with age at onset. Preschizophrenic subjects who showed lower rates of negative emotion during late childhood/adolescence were younger at illness onset. Findings are discussed in light of previous reports linking affective symptoms with better prognosis in schizophrenia.

Granulocyte colony-stimulating factor "mobilized" peripheral blood progenitor cells accelerate granulocyte and platelet recovery after high-dose chemotherapy.

Hematopoietic growth factors have been used to accelerate engraftment after bone marrow transplantation and to "mobilize" peripheral blood progenitor cells (PBPC). We report on the data in 85 consecutive patients with Hodgkin's disease who were treated in a single institution using different methods to obtain PB progenitor cells. Use of granulocyte colony-stimulating factor for mobilization resulted in a significantly accelerated time to recovery of granulocytes (10 days v 12 days, P < .01) when compared with "nonmobilized" PBPC recipients. Similarly, use of mobilized PBPC resulted in a significantly accelerated time to platelet engraftment (13 days v 30 days, P < .001) when compared with "nonmobilized" recipients. Moreover, there was a statistically significant difference in total costs in favor of the group receiving "mobilized" PBPC.

Convergence of DSM-III diagnoses and self-reported symptoms in child and adolescent inpatients.

A systematic study of the relationship between empirically-derived diagnoses and symptom patterns is reported in a sample of 160 preadolescents and adolescents consecutively admitted to a psychiatric inpatient setting. Results show that the DSM-III diagnoses generated using the child version of the Diagnostic Interview Schedule for Children (DISC-C) were significantly associated with several factor analytically-derived symptom scales of the Achenbach and Edelbrock Youth Self-Report. The results included the finding of significant associations for conduct disorders with the delinquency scale, for affective disorders with the depressed scale, and for both affective and anxiety disorders with the broad-band internalizing scales. This study suggests implications about diagnostic categorizations and construct validity as well as implications for clinical evaluation.

Comparison of DISC with clinicians' DSM-III diagnoses in psychiatric inpatients.

A sample of 163 children and adolescents, consecutively admitted to a large, private psychiatric teaching hospital, was interviewed using the child version of the Diagnostic Interview Schedule for Children (DISC-C). Patients, ages 12 to 16, were interviewed during the first month of admission. Kappa coefficients were obtained from cross-tabulated frequencies of DISC and clinician diagnoses. Agreement between clinical and DISC diagnoses was generally poor across diagnostic categories. In general, when algorithms of a higher threshold were used, the percentage of patients in a particular diagnostic group was closer to the percentage diagnosed by the clinician. Discussion focuses on factors that may contribute to the discrepancy between number and type of diagnoses that the DISC yields compared to those made by the clinicians.
