Ursodeoxycholic acid treatment in primary biliary cirrhosis with the emphasis on late stage disease.

We studied the effect of ursodeoxycholic acid in 19 patients with primary biliary cirrhosis, mainly stages III and IV. The dose of UDCA employed was 10-15 mg/kg body weight per day. After 1 yr, 17 patients were still using UDCA, and the mean values of serum alkaline phosphatase, gamma-glutamyltranspeptidase and alanine-aminotransferase had fallen significantly. Serum bilirubin, initially elevated in 7 of the 13 late-stage (III and IV) patients, showed a further increase in 3 of the 7 patients. In 2 of these 3 patients, UDCA had to be withdrawn (dose reduction had no effect). One patient developed a decompensated cirrhosis in spite of UDCA withdrawal. Pruritus worsened in 4 patients, all of whom were late stage patients. Ten late-stage (III-IV) patients showed improvement in liver biochemistry and clinical findings as did all early-stage PBC patients. Thus, UDCA treatment is not beneficial for all PBC patients. Special care should be taken in the early phase of UDCA therapy in later-stage (III-IV) patients: frequent biochemical checks should be carried out, for instance every 2 weeks in the first 2 months after starting UDCA, especially the estimation of bilirubin.

[Effects of short-term psychotherapy on the success of an in vitro fertilization/embryo transfer program].

A pilot study investigates how the implementation of brief psychotherapy affects the results of an IVF/ET program. A main criterion in therapy is the reduction of the patients' psychic stress who are led to find new problem solutions building upon the abilities and faculties they already show. Emphasis is put on their gaining a cooperative relationship towards their body. Of the 15 patients in this study, 7 became pregnant by IVF after brief psychotherapy, which corresponds to a ratio of 47%. We assume that this is due to a change of attitude towards the desire for pregnancy as well as towards the possible ways of its realization.

Polycystic disease of the liver: a report of six patients.

Six patients with polycystic liver disease and a different clinical presentation are presented. Two patients were unaware of enormously enlarged livers, two patients with only minor complaints had huge livers reaching into the pelvis and finally, two patients had equally enlarged livers with almost unbearable complaints and insisted on operation. One patient was treated with deroofing and fenestration of multiple cysts (Lin Ty et al., Ann Surg 168: 921-923, 1968). The second patient underwent left hepatic lobectomy. Partial liver resection should be considered in patients with severe mechanical complaints if the majority of cysts is located in one liver lobe.

Tumors of the ampulla of Vater treated by local resection: a report of five cases.

The clinical features, diagnostic work-up and difficulties in establishing a diagnosis on endoscopic biopsy specimens were retrospectively analysed in five patients with ampullary tumors. In addition, the technique of local papillary resection is reported. Indications for local resection are: elderly patients with severe co-existing diseases, patients with benign ampullary tumors which are either adenomatous or show epithelial dysplasia or patients with inconclusive preoperative endoscopic biopsy or peroperative frozen sections.
