Immunoglobulin M and G antibody response in rabbits after experimental Haemophilus ducreyi infection.

The enzyme-linked immunosorbent assay and immunoblot technique were used to investigate the humoral immune response following intradermal infection of rabbits with Haemophilus ducreyi. Basically, the kinetics of IgM and IgG antibody synthesis was the same as in other bacterial infections. However, antibody titres in the enzyme-linked immunosorbent assay were remarkably low. In the immunoblot experiments, antibodies of both immunoglobulin classes were mainly directed against Haemophilus ducreyi antigens with relative molecular masses of 79,000, 62,000, 55,000, 49,000, and 26,000, suggesting that these antigens may play a dominant role in the humoral immune response after infection.

Unmasking of bacteriophage Mu lipopolysaccharide receptors in Salmonella enteritidis confers sensitivity to Mu and permits Mu mutagenesis.

The human pathogen Salmonella enteritidis 3b was found to be highly resistant to phage P22 and Mu derivatives. The Mu sensitivity (musA1) allele from Salmonella typhimurium could be transferred to S. enteritidis 3b at low frequency by cotransduction with hisG::Tn10. Sensitivity to Mu resulted in a large reduction in the number of lipopolysaccharide core-region oligosaccharides that were substituted with O-antigen polysaccharide. The residual high-molecular-weight lipopolysaccharide appeared to be a hybrid displaying O antigens which were immunologically related to those of S. typhimurium and not to those of S. enteritidis. Consequently, Mu d1(Ap lac) could then be transduced into Mus strains forming stable lysogens. On temperature induction, Mu transposition could easily be used to generate mutations in genes coding for cell surface antigens including fimbriae, lipopolysaccharide, and flagella.

[Particle agglutination test: a new technic for the detection of HIV-1 antibodies].

Two techniques for detection of anti-HIV-1 antibodies were comparatively investigated. It is shown that the particle agglutination (PA) is at least equivalent to the enzyme immunoassay (EIA) as far as sensitivity is concerned. A ten-fold increased sensitivity of PA was found when compared with Western Blot results. Furthermore, the detection of specific IgM antibodies in sera of HIV-1-infected persons is possible when the PA is used. The high stability of PA reagents when stored at 37 degrees C, the simple performance of the assay, and the possibility of automatization are important advantages of the PA when compared with the EIA.

[Perfectionism or practicability in osteosyntheses--limits of the carefulness responsibility].

The discussion of these complex themas can only partially try to show the way to successful osteosynthesis for not only the patient as also for traumatologists and orthopedic surgeons. "Perfection or practicability"-this question is to be answered so that exaggerated perfection, as seen overall in surgery, is not foundation of the successful result. On the contrary, osteosynthesis procedures conceived so that they are generally practicable and employable for those colleagues who are experienced and have confidence with the material. Perfection should not be isolated as bare organ surgery. At the same time it is clearer that it is not possible to stop the specialization in bone surgery. In its place arises the necessity of an interdisciplinary cooperation of all surgical specialties. These demands will justify our current concept of treatment under the condition that the surgeon realizes his limitations in difficult situations and behaves accordingly. Practicable and even to perfection is however the relationship of the bone surgeon to the patient who is injured in an accident. The professional assessment of the true or the apparent failures after osteosynthesis must observe the written laws. Mistakes, which reveal the ignorance and which grossly contradict the limiting prerequisites of the guidelines for the execution of osteosynthesis are to be appraised as such. The professional consultant, himself, must be so confident with the practice of osteosynthesis so that he can implement an objective and critical estimation of the situation.(ABSTRACT TRUNCATED AT 250 WORDS)

Transmission of IBR/IPV virus in bovine semen: A case report.

The transmission of IBR/IPV virus in imported semen is reported. Some of the inseminated animals showed seroconversion and, in accordance with Swiss law, had to be eliminated. To avoid such cases in international sperm exchange, methods of detecting IBR/IPV virus need to be improved. In the longer term, AI centres must be established in which all bovine stock is seronegative for IBR/IPV.

Plate osteosynthesis in posttraumatic deformities of the femoral shaft.

The success of precisely indicated surgical treatment of posttraumatic deformities of the femur shaft depends on painstaking preoperative planning with exact assessment of the locus of correction, careful determination of the correction angle, and choice of an adequate operation method. In correction operations plate osteosynthesis is necessary, in contrast to the various possibilities for care of primary fractures of the femur. Therefore we concentrate in this paper mainly on the use of plate osteosynthesis, without forgetting that there are also indications for intramedullary nailing, especially of not yet consolidated fractures or aseptic wound healing impairments. Long X-rays of the leg are an essential part of the preoperative preparation. The success of all operative treatments does not depend only on the degree of disablement, the age of the patient, and realistic target. Particularly in view of the difficulty of plate osteosynthesis and the variety of methods of primary osteosynthesis, the surgeon has to consider the rules of biomechanics as they apply to the incorrectly set femur and to simulate the surgical treatment with preoperative planning in order to attain physiological axial conditions with the best operative method. This applies particularly to the knee joint. Axial deformities are very often combined with shortening, and surgical correction is therefore sometimes difficult.

[Ruptures of the tendons of the extensor system of the knee joint].

Ruptures of the quadriceps and patellar tendon are comparatively rare lesions. From the number of cases from 1973 till 1983 17 ruptures of the femoral extensor tendon and 8 ruptures of the patellar tendon were controlled diagnostically, therapeutically and in view of their long-term results. Causally in the group of quadriceps tendons 13 spontaneous ruptures were facing 4 traumatically caused ruptures. In case of traumatic ruptures 3 of them were adolescents after extreme traumatic violence by rapid traumata. Degeneratively conditioned subcutaneous quadriceps tendon ruptures we only found in case of men who were on an average older than 50 years. In spite of physiologically extreme resistance to ruptures 6 of 8 ruptures of the ligamentum patellae were ascribed to the fact that by the progressing degeneration the remaining strain capacity of the tendon was reduced below the measure of normal load. In each individual case the aspects of expert opinion were traced - above all in case of tendon ruptures with osseous patella lesions. For the first time a temporally staggered bilateral spontaneous rupture of the patella was described. The operative approach has been discussed just as much as the necessity to examine the internal knee-joint structures arthroscopically in case of the plurality of accompanying retinaculum lesions.

[Development of habitual shoulder dislocation].

The starting point of the retrospective analysis of 177 primary dislocations was the question of frequency of habitual dislocations, the correlation between habitual dislocation and time of early immobilisation, as well as the search for additional statements which would result in a differentiated therapy. 18.1% habitual dislocations were seen to develop, mostly after immobilisation for less than three weeks. It is an interesting fact that the technique of immobilisation made no difference. We found that after the first even there was generally no loss in function, whereas 14.5% of the men and 34.3% of the women complained of severe subjective symptoms. The 12 patients who underwent an operation because of habitual dislocation suffered neither from dysfunction nor from painful movement, and no dislocation was found. The rate of habitual dislocation in our patients showed a lower rate of dislocation of the shoulder joint. We found 18.1% dislocations, whereas the literature showed an average quota of 30%. To improve the results we recommend a special X-ray technique following the first reposition. In the age group between 20 and 30 years, which is most susceptible, we also recommend arthrography and arthroscopy of the shoulder joint. To improve typical lesions, such as osseous rupture of the capsule or what is known as Hill-Sachs lesions, primary operation is discussed as an alternative treatment.

[An instrumented external fixation device as an instrument for measuring external fracture management].

The article deals with a method of measuring which, without changing the buildung components of the AISF-fixateur externe, determines the force at the fracture gap in the case of compression osteosynthesis. For this purpose there has been developed a ring force measuring element and expansion measuring strips. It is the aim of this measuring method to describe the course of force during the operative process of straining and to control it regarding accuracy of measuring, to superrise the forces occurring during the healing process, to be able to gauge in a definite manner in case of disturbances of the external compression system and finally to control the force attitude at the fracture gap also under dynamic load. The ring force measuring boxes can be set up at the free pipe end without changing the necessary systems of assembling. Advantages are the unlimited use, costs incurred only once, easy handling, high immunity from disturbance, the possibility of gas disinfection and no obstruction of external construction forms. With this system so far we have controlled 22 patients by means of an external compression osteosynthesis. With compression degrees at the fracture gap ranging between 400 and 800 N, one can mechanically talk of stable external osteosynthesis. With the help of bioptic material taken from the region of the direct osseous contact of a human fracture gap, the morphological signs of the primary healing of the bone could also be proved.
