Central pontine myelinolysis--a rare manifestation of CNS Sjogren's syndrome.

Central nervous involvement in Sjogren's syndrome (CNS-SS) is not uncommon and has a variety of manifestations. We describe a 47-year-old woman with Sjogren's syndrome who presented with distal renal tubular acidosis with severe hypokalemia and hypokalemic myopathy. She developed progressive obtundation after years of stable disease. ANA, anti-Ro antibodies were positive. Brain MRI showed a cleft in the mid pons which was hypointense on T1 and hyperintense on T2 which was considered to be classical of central pontine myelinolysis. Serial MRI showed initial enlargement of the lesion which persisted despite successful immunosuppressive therapy with pulse methylprednisolone, pulse cyclophosphamide, plasmapheresis and IVIG.

Malignant papillary neoplasms of the intrahepatic bile ducts: CT and histopathologic features.

OBJECTIVE. We describe the CT and pathologic features of malignant papillary neoplasms of the intrahepatic bile ducts in 15 patients. CONCLUSION. CT is a useful technique for revealing intraductal lesions, although the findings are nonspecific and variable. When intraductal masses or nodules are seen with localized dilatation of the intrahepatic bile ducts on CT scans, malignant papillary neoplasms of the intrahepatic bile ducts should be included in the differential diagnosis.

New ADA criteria in the Korean population: fasting blood glucose is not enough for diagnosis of mild diabetes especially in the elderly.

BACKGROUND: To compare the 1997 American Diabetes Association (ADA) criteria with the 1985 World Health Organization (WHO) criteria in categorization of the diabetes diagnostic status of Koreans and to define clinical characteristics of subjects diagnosed differently by the two criteria. METHODS: In 810 Korean subjects, we analyzed blood glucose and insulin response during 75 g oral glucose tolerance test (OGTT). According to current WHO criteria, the cutoff values of FPG which distinguish normal and IGT from diabetes were determined. Then the subjects were categorized according to both WHO and ADA criteria. The clinical characteristics of the subjects with different diagnostic categories by the two criteria were defined. RESULTS: The FPG cut point distinguishing diabetes from IGT was 117 mg/dl, and from normal was 110 mg/dl. The overall agreement between the ADA criteria and the WHO criteria was moderate, as reflected in the kappa of 0.45. 141 of subjects categorized diabetes by WHO criteria were not diagnosed with ADA criteria. These discordant subjects were older in age and showed blunted early insulin response than concordant normal subjects. CONCLUSION: These results suggest that mild diabetes by the WHO criteria, especially in the elderly, would not be diagnosed as diabetes by the ADA FPG criteria only. Thus, in a group at high risk for developing diabetes or in a relatively older age group, we should continue using the OGTT.

The effect of long-term glycaemic control on serum lipoprotein(a) levels in patients with Type 2 diabetes mellitus.

AIMS: To examine whether long-term glycaemic control affects lipoprotein(a) (Lp(a)) levels in patients with Type 2 diabetes mellitus. METHODS: Eighty-nine Type 2 diabetic patients (38 men, 51 women) were recruited from the diabetes clinic. Based on HbA1c concentrations at baseline, patients were divided into two groups: those with HbA1c < 8.0% (n =45) and those with HbA1c > or = 8.0% (n=44). Comparisons of Lp(a) levels were made between both groups. The effect of long-term glycaemic control on Lp(a) levels was investigated in a subgroup of 20 patients, selected from those with baseline HbA1c > or = 8%. All these patients were treated with a goal of HbA1c <7%. RESULTS: Lp(a) levels were not significantly different between those with HbA1c< 8.0% and those with HbA1c, > or = 8.0%. No correlation between Lp(a) and HbA1c or fasting blood glucose levels was noted in diabetic patients as a whole. After 2 years of intensive glycaemic control, all patients exhibited remarkable improvement of therapy: their average HbA1c levels were 6.5 +/- 0.7%, being < 7% in 70% of patients. However, no change in Lp(a) levels were observed after 2 years (19.5 +/- 14.8-21.4 +/- 13.4 mg/dl, P = 0.390). CONCLUSION: These results indicate that improvement of glycaemic control does not affect serum Lp(a) levels in patients with Type 2 diabetes mellitus.

Inflammatory pseudotumor of the liver in patients with recurrent pyogenic cholangitis: CT-histopathologic correlation.

PURPOSE: To correlate computed tomographic (CT) features of inflammatory pseudotumors of the liver with histopathologic results in patients with recurrent pyogenic cholangitis. MATERIALS AND METHODS: CT features of 13 cases of inflammatory hepatic pseudotumor in 10 patients with recurrent pyogenic cholangitis were reviewed. Diagnosis was made by means of surgical resection in all patients. CT scans were analyzed for the appearance of masses and ancillary findings in correlation with the histopathologic findings in each resected specimen. RESULTS: The masses were 2.0-7.0 cm (mean, 3.5 cm). At nonenhanced CT, the masses appeared as ill-defined, hypoattenuating lesions. At contrast material-enhanced CT, the masses exhibited central hypoattenuating areas with an iso- or hyperattenuating thickened periphery in four cases and a multiseptate appearance with hyperattenuating internal septa and periphery in nine cases. CT-histopathologic correlation showed that the central hypoattenuating area indicated the presence of chronic inflammatory infiltrates with foamy histiocytes, plasmacytes, and lymphocytes, while iso- or hyperattenuating areas in the periphery and internal septa of the mass represented fibroblastic proliferation. All patients had CT features of recurrent pyogenic cholangitis, such as hepatolithiasis, intrahepatic duct stricture and dilatation, common bile duct calculi, pneumobilia, or parenchymal atrophy. CONCLUSION: Although CT features are not specific, inflammatory pseudotumor should be included in the differential diagnosis in patients with recurrent pyogenic cholangitis and a hepatic mass detected at CT.

Function and survival of macroencapsulated syngeneic islets transplanted into streptozocin-diabetic mice.

BACKGROUND: Macroencapsulation is a strategy to protect transplanted islets from rejection and autoimmune attack. This study addresses questions about the survival and function of macroencapsulated syngeneic islets. METHODS: Planar immunobarrier membrane diffusion devices were used for syngeneic islet transplantation. After being mixed with a 1% alginate solution, a total of 250, 500, 750 or 1000 islets were loaded into the devices, which were implanted into the epididymal fat pad(s) of streptozocin diabetic mice. RESULTS: The success rate for restoration of normoglycemia at week 4 was highest for the recipients receiving two devices, each with 500 islets. Loading 750 or 1000 islets provided no improvement over loading 500 islets in a single device. Devices containing 250 islets were rarely successful. There was a striking tendency of transplants to either bring glucose levels into the near normal range or to fail with marked hyperglycemia. After an overnight fast at 1 and 4 weeks, but not at 12 weeks, hypoglycemia was found. The insulin content of devices from animals with normalized glucose values was higher than the insulin content in failed devices. Islet volume was maintained for 12 weeks, and fibrosis did not increase. CONCLUSIONS: A relatively small mass of macroencapsulated islet tissue can survive and function well enough to normalize glucose levels for at least 12 weeks. Maintenance of glucose levels in the near-normal range seems to have a beneficial influence on graft success. The finding of fasting hypoglycemia raises important clinical questions about islet dysfunction. Important limitations in the requirements for islet packing density in macroencapsulation have been defined. New approaches for improving islet packing density must be developed to make diffusion-dependent macroencapsulation more practical.

Discovery of LB30057, a benzamidrazone-based selective oral thrombin inhibitor.

Systematic variation of the so-called P-pocket moiety of benzamidrazone-based selective thrombin inhibitors led to the discovery of LB30057. It is potent (Ki = 0.38 nM for human thrombin), selective (Ki = 3290 nM for bovine trypsin), and orally bioavailable (58% oral bioavailability in dogs). LB30057 was efficacious in thrombosis animal models.

Progression to overt proteinuria in microalbuminuric Koreans with non-insulin-dependent diabetes mellitus.

Long-term data concerning the progression of microalbuminuria are not available in Koreans with non-insulin-dependent diabetes mellitus (NIDDM). To elucidate potential risk factors of the development of overt proteinuria in microalbuminuric Koreans with NIDDM, we studied retrospectively 46 patients with NIDDM. Between 1989 and 1990, they were found to have persistent microalbuminuria, and then were followed up regularly. Urinary albumin excretion rates (UAEs) were measured on a 24-h urine sample. Microalbuminuria was defined as UAE between 20 and 200 microg/min, and overt proteinuria as UAE > 200 microg/min on two consecutive occasions. After a mean of 4.5 years (range 3-6), 23 patients progressed to overt proteinuria (progressors), and others remained microalbuminuric (nonprogressors). Duration of diabetes was significantly longer in progressors than in nonprogressors. Mean fasting plasma glucose and HbA1c levels during the follow-up were significantly higher in progressors compared with nonprogressors (11.5+/-3.6 vs. 8.7+/-2.5 mmol/l, P=0.006, and 8.9+/-1.5 vs. 7.5+/-1.4%, P=0.005, respectively). In addition, frequencies of overt proteinuria were significantly higher in patients with their mean HbA1c > 8% during follow-up than in patients with their mean HbA1c < or=80% during follow-up (65.2 vs. 30.4%, P=0.015). Mean systolic blood pressure and mean diastolic blood pressure during follow-up tended to be higher in progressors compared with nonprogressors. Multiple logistic regression analysis revealed that mean HbA1c levels and mean systolic blood pressure during the follow-up were the most significant predictors for the incidence of overt proteinuria at 4.5-year follow-up, when adjusted for various factors (P=0.023, P=0.038, respectively). We conclude that poor glycemic control, along with elevated systolic blood pressure, were powerful predictors for the development of overt proteinuria in microalbuminuric Koreans with NIDDM.

Cloning and expression of a Clostridium thermocellum xylanase gene in Escherichia coli.

A Clostridium thermocellum xylanase gene, designated xynX, was cloned in Escherichia coli and was categorized a novel gene as a result of the comparison of restriction patterns of the C. thermocellum xylanase genes so far reported. The xynX gene encodes a xylanase having the molecular weight of 105 kilodaltons. A number of smaller truncated proteins with activities towards 4-methylumbelliferyl-beta-D-cellobioside and xylan were also produced. The enzyme hydrolyzed xylan to xylo-oligosaccharide, indicating typical activity of endo-beta-1,4-xylanase. This endoxylanase hydrolyzed carboxymethylcellulose without notable reduction of the viscosity as an exo-beta-1,4-glucanase, even though the enzyme exhibited very low levels of activity against other soluble and insoluble cellulosic substrates.

Molecular cloning of a Bacillus sp. KK-1 xylanase gene and characterization of the gene product.

A gene encoding cellulase-free xylanase was cloned from the thermophilic bacterium Bacillus sp. KK-1 into Escherichia coli and the gene product was purified from the recombinant E. coli. This xylanase gene, designated xylY, was composed of 1,302 base pairs and encoded a polypeptide of 434 amino acids. No similarity was found between the nucleotide sequence of the xylY gene and those reported for other xylanase genes. The deduced amino acid sequence was homologous to those of cellulases belonging to the beta-glycanase family D. The purified enzyme exhibited maximum activity at 55 degrees C but also lost 70% of this activity even after incubation for 30 min at 55 degrees C. Bacillus sp. KK-1 may have acquired the xylY gene by an interspecies gene transfer during adaptation to mesophilic environment.

Biliary stricture caused by blunt abdominal trauma: clinical and radiologic features in five patients.

PURPOSE: To evaluate the clinical and radiologic features of biliary stricture after blunt abdominal trauma and to report the results of endoscopic stent placement. MATERIALS AND METHODS: Medical records and radiologic findings were reviewed in five patients with biliary stricture after blunt abdominal trauma. The level, length, and contour of the strictures were analyzed with endoscopic retrograde cholangiopancreatography (ERCP). Computed tomographic (CT) scans were also reviewed to determine the presence of biliary dilatation, configuration of the injured bile duct, and ancillary abdominal findings. Results from endoscopic stent placement were evaluated in all patients. RESULTS: Stricture occurred in the suprapancreatic portion of the common bile duct in four patients and in the intrapancreatic portion in one patient. At ERCP, the stricture contour was concentric and smooth in three patients, eccentric and smooth in one, and abruptly terminated in one. CT showed abrupt narrowing of the common bile duct with dilatation of the proximal portion in all patients. Endoscopic stent placement was successful in all patients. CONCLUSION: Patients with biliary stricture after blunt abdominal trauma exhibit a delayed onset of symptoms. A correct diagnosis may be difficult on the basis of findings from CT or ERCP alone without a clinical history or evidence of contusions at other sites.

Intestinal Behçet syndrome: CT features of patients with and patients without complications.

PURPOSE: To analyze the computed tomographic (CT) features of intestinal Behçet syndrome and to determine the usefulness of CT in detecting complications. MATERIALS AND METHODS: The CT scans of 28 patients with intestinal Behçet syndrome were retrospectively reviewed. Five patients had bowel perforation, and two had peritonitis. Patterns of bowel involvement, patterns of contrast enhancement, and ancillary findings were compared in patients with complications (n = 7) and patients without complications (n = 21). RESULTS: Ten patients had polypoid lesions, nine had a thickened bowel wall, and nine had both findings. Lesion enhancement was mild in eight patients (29%) and marked in 20 (71%). Polypoid lesions were more commonly seen in patients without complications (P = .020); a thickened bowel wall was more commonly seen in patients with complications (P = .030). Seventeen of 18 patients (94%) with minimal perienteric infiltration did not have complications (P = .0003), whereas all five patients with severe perienteric infiltration did have complications. CONCLUSION: In patients with known intestinal Behçet syndrome, CT can be useful in determining the extent of the lesions and in identifying cases in which complications are likely to occur.

Usefulness of CT in patients with intestinal obstruction who have undergone abdominal surgery for malignancy.

OBJECTIVE: The purpose of this study was to evaluate the usefulness of CT in determining the causes of intestinal obstruction in patients who have undergone abdominal surgery for malignancy. MATERIALS AND METHODS: We analyzed the CT scans of 55 patients with benign (n = 26) or malignant (n = 29) intestinal obstruction that developed after abdominal surgery for malignancy. After calculating the diagnostic accuracy of interpretations by three radiologists, we compared CT findings for benign and malignant intestinal obstructions with respect to peritoneal involvement patterns and other ancillary findings. Multivariate logistic regression analysis was used to determine the diagnostic performance of CT in revealing causes of obstruction. RESULTS: Diagnostic accuracies of the three radiologists were 67%, 75%, and 78%. CT findings indicating malignant obstruction were a mass at the site of obstruction or prior surgery, lymphadenopathy, or an abrupt transition zone and irregular bowel wall thickening at obstructed sites (p < .05). Conversely, the chance for benign obstruction increased when CT revealed mesenteric vascular changes, a large amount of ascites, or a smooth transition zone and smooth bowel wall thickening at the obstructed site (p < .05). With multivariate logistic regression analysis using two variables (a mass at the site of obstruction or prior surgery and lymphadenopathy), we calculated the overall accuracy of CT as 84% (46/55 patients). CONCLUSION: CT is useful in differentiating benign from malignant intestinal obstructions in patients who have undergone abdominal surgery for malignancy. However, CT has limitations in patients not having a demonstrable peritoneal mass.

Hypoxia induces vascular endothelial growth factor gene and protein expression in cultured rat islet cells.

The formation of new microvasculature by capillary sprouting at the site of islet transplantation is crucial for the long-term survival and function of the graft. Vascular endothelial growth factor (VEGF), an endothelial cell-specific mitogen with potent angiogenic and vascular permeability-inducing properties, may be a key factor in modulating the revascularization of islets after transplantation. In this study, we examined the gene expression of VEGF mRNA in three tumor cell lines and in isolated whole and dispersed rat islets in vitro by Northern blot hybridization in normoxic (5% CO2, 95% humidified air) and hypoxic (1% O2, 5% CO2, 94% N2) culture conditions. Increased expression of VEGF mRNA was observed in beta-TC3, RAW 264.7, and IC-21 tumor cell lines when subjected to hypoxia. With isolated whole islets in normoxic culture, a threefold increase in VEGF mRNA (P < 0.001) was seen at 48 h as compared with freshly isolated islets. This response was similar to the 3.8-fold increase observed with islets subjected to hypoxia. Dispersed rat islet cell clusters cultured on Matrigel for 24 h under hypoxic conditions showed a 3.4-fold increase (P < 0.01) in VEGF mRNA compared with those cultured in normoxia. This correlated with increased VEGF secretion as determined by enzyme-linked immunosorbent assay. Immunohistochemical studies revealed the presence of increased expression of VEGF protein near the center of islets after 24 h of normoxic culture. Islet cell clusters on Matrigel showed intense cellular localization of VEGF in both beta-cells and non-beta-cells. These findings suggest that rat islet cells, when subjected to hypoxia during the first few days after transplantation, may act as a major source of VEGF, thereby initiating revascularization and maintaining the vascular permeability of the grafted islets.

A case of sulindac-induced enteropathy resulting in jejunal perforation.

Non-steroidal anti-inflammatory drugs (NSAIDs) are widely prescribed for rheumatic conditions. Their effects on the upper gastrointestinal tract are well recognised. Clinically important damage to the small intestines is less common and often unrecognised. We report a case of sulindac-induced jejunal perforation in a rheumatoid arthritis (RA) patient with previous gastrectomy for gastric carcinoma. The prevalence, diagnosis, pathogenesis and treatment options of NSAID-induced enteropathy will be discussed.
