[Treatment of the wound on donor site after great toe-nail flap grafting].

OBJECTIVE: To investigate the method of improving the vitality of skin graft on donor site of the great toe-nail skin flap. METHODS: From June 1982 to April 1998, 252 cases of the great toe-nail flaps with piece of phalangeal bone and 18 cases of the simple great toe-nail flap were repaired with thin skin graft and packed under proper pressure. The stitches were removed two weeks later in common situation. It should be postponed on split thickness or partial survival skin flap avoiding early mobilization. RESULTS: Sixty-six cases of skin graft were necrotic after operation. Among them, 38 cases needed second skin grafting and 28 cases were healed after changing dressing. The survival rate of skin grafting was obviously higher on phalangeal marrow surface than on periosteum of the naked phalange. Contracture of the skin graft after operation made the retained skin flap expanding from medial side to lateral side and covered the whole plantar surface of the great toe. CONCLUSION: The survival rate of the skin graft on donor foot is improved after adopting the improved measures on taking the flap from great toe and paying attention to skin graft planting and packing. Free flap grafting is advocated for repairing of the wound on donor area of the great toe nail flap.

[Ethanol-induced gastric mucosal injury and the protection of taurine against the injury in rats].

40 SD rats were randomly divided into control group, ethanol-induced injury group and taurine protective group for the measurement of the contents of endothelin (ET), nitric oxide synthetase (NOS), somatostatin (SS) and vasoactive intestinal peptide (VIP). The results are as follows. The gastric mucosa was damaged by 75% ethanol, being aggravated with prolonged application time, as shown by a significant increase of the ET content and decreases of the contents of NOS, VIP and SS in the gastric mucosa. In the taurine group, decrease of ET content and increase of the contents of NOS, SS and VIP in the gastric mucosa were observed. Apparently, the changes in ET, NOS, SS and VIP contents of gastric mucosa serve as reliable indices in the pathogenesis by ethanol and protective effect by taurine.

Evaluation of prostAsure index in the detection of prostate cancer: a preliminary report.

OBJECTIVES: Although prostate-specific antigen (PSA) has revolutionized the detection of prostate cancer, it has definite limitations with respect to its clinical sensitivity and specificity. Because a substantial number (20% to 40%) of men undergoing radical prostatectomy have a PSA level of 4.0 ng/mL or less, any new test offering diagnostic improvement must perform well in patients whose PSA level is less than or equal to 4.0 ng/mL, as well as in patients whose PSA is greater than 4.0 ng/mL. The performances of two tests, the ProstAsure index and the percent free PSA test, were evaluated in detecting cancer. METHODS: We retrospectively analyzed serum samples from 225 men who were grouped into three categories: 94 men who had a normal digital rectal examination and a serum PSA level of 4.0 ng/mL or less, 77 men who were clinically suspected of having benign prostatic hyperplasia (BPH) with a serum PSA level of 4.0 ng/mL or less, and 54 men with localized prostate cancer. The PSA assays were performed using the Hybritech and Tosoh assays and the ProstAsure index was determined by Global Health Net, Savannah, Ga. Receiver operator characteristic (ROC) curves were constructed to evaluate the performance of these two tests, and the areas under the curve were compared for significance. RESULTS: The sensitivity and specificity of detecting prostate cancer using ProstAsure were 93% and 81%, respectively. Using a cutoff value of 15%, the sensitivity and specificity of detecting cancer for percent free PSA were 80% and 74%, respectively (sensitivity increased to 93% and specificity to 59% for free PSA at 19%). In men with a total serum PSA level of 4.0 ng/mL or less, ProstAsure had a lower false-positive rate compared to free PSA level at 19% for men with or without clinical BPH as well as for men without clinical BPH using a 15% free PSA threshold. ProstAsure left fewer cancers undetected (7%) compared to free PSA at the 15% cutoff (20%). CONCLUSIONS: In this study of selected men, ROC curve analysis shows a statistically significant advantage in performance (P = 0.0023) for the ProstAsure index compared to free PSA in detecting prostate cancer.

Epidemiological survey of oral submucous fibrosis in Xiangtan City, Hunan Province, China.

Oral submucous fibrosis is a high risk precancerous condition and is suggested to be caused by areca nut chewing. Areca nut chewing is popular in Hunan Province of China, and is more concentrated in Xiangtan City. Two and nine cases of oral submucous fibrosis (OSF) were first noticed in 1984 and 1985 respectively, and an epidemiologic survey was subsequently performed in 1986. The epidermiologic method of cluster sampling was used. The Yuhu District, one of the five urban districts of the Xiangtan City with a population of 100,000 was selected as a whole body in the survey, 57 independent units of various professions were randomly selected as group samples and more than 70% of subjects in each unit were examined. Definite fibrous band on palpation was used as a main diagnostic criterion for OSF. A total of 11046 individuals were examined; among them were 3907 areca nut chewers (35.37%) and 7139 non-chewers (64.63%). 335 cases of OSF were found, comprising a prevalence rate of 3.03%. The disease involved mainly the middle third of the oral cavity. All of the OSF cases were areca nut chewers. No case had been found in non-chewers. Four cases of oral carcinoma were found on the basis of OSF, the malignant transformation rate was 1.19%. The high prevalence of OSF may be due to areca nut chewing plus extensive and heavy use of hot pepper in Xiangtan people. The result supports the role of the areca nut as an etiologic factor in the development of OSF. The low malignant transformation rate of 1.19% compared with the 7.6% in an Indian report may be because Xiangtan people chew areca nut without tobacco.

Hand grip strength: an indicator of nutritional state and the mix of postoperative complications in patients with oral and maxillofacial cancers.

The aim of the study was to assess the reliability of hand grip strength as an indicator of nutritional state, and to see if it was of any value in the prediction of postoperative complications. One-hundred and twenty-seven patients who presented with oral and maxillofacial cancer were studied for measurement of hand grip strength, mid-arm muscle circumference, and creatinine-height index. Hand grip strength correlated well with mid-arm muscle circumference and creatinine-height index. Patients whose hand grip strength was < 85% of the control value developed significantly more postoperative complications than those in whom it was 85% or more (15/31 (48%) compared with 12/65 (18%), P = 0.004). In conclusion, hand grip strength is not only a useful, non-invasive indicator of skeletal muscle mass, but may also be of use in predicting postoperative complications.

Anthropometry↗

Nutritional status of patients with oral and maxillofacial malignancies.

A nutritional assessment battery consisting of the patient's history, anthropometric measurements, and laboratory tests was used to characterize the nutritional status of 127 patients with oral and maxillofacial malignancies. Forty-three percent of these patients had good nutrition, 21% fair, and 36% poor. The pattern of nutritional impairment was a protein-calorie deficiency. A close correlation with malnutrition was found for diminished oral intake and tumor stage. Gender, tobacco or alcohol consumption, job, and living place were not related to nutritional status.

Applicability of the general nutritional status score to patients with oral and maxillofacial malignancies.

The general nutritional status (GNS) score was used to assess the nutritional status of 127 consecutive patients with oral and maxillofacial malignancies. Forty-six of our patients (36.2%) were undernourished (group 2), while 81 (63.8%) were in good nutritional condition (group 1). Comparison of nutritional laboratory tests between these two groups showed that the differences in serum albumin, transferrin, and creatinine-height index (CHI) had statistical significance, yielding an excellent correlation between the GNS score and the nutritional laboratory test. Undernourished patients had a significantly higher postoperative complication incidence (48.3%) than well-nourished patients (19.4%) (chi 2 = 6.637; P < 0.01), indicating that the GNS score can be used as a prognostic index.

[Experimental study on oral candidiasis].

The successful rate in producing oral candidiasis of the animal is not high. By utilizing: (1) fresh cell-line isolated from the oral cavity of the patient suffering from oral candidiasis and passing twice in rabbits and re-isolated to increase its toxicity; and (2) expanding the area of examination, the authors successfully (100%) produced oral candidiasis in Wistar rats. The specimens are studied histologically on its nature of whether it is a precancerous lesion or not.

[Effect of candidal infection on the hyperplastic oral epithelium].

DMBA was used to produce oral epithelial changes, from benign hyperplasia to epithelial dysplasia of different severity in golden hamsters. Thereafter, they were inoculated with candida albicans. The result shows that candidal infection can induce epithelial dysplasia in benign hyperplasia; and in epithelial dysplasia, candidal infection will promote malignant transformation. It implies that candidal infection of the oral leukoplasia should be detected and treated.

A comparison between the clinical behavior of neurilemmomas in the neck and oral and maxillofacial region.

The records of 49 patients with neurilemmoma in the neck and oral and maxillofacial region (OMR) were reviewed. The results showed some location-related differences in clinical behavior, including six cases of recurrence and two cases of malignant change found in the OMR. It was suggested that anatomic factors play an important role in the poor prognosis of tumors in the OMR.

Adenomatoid odontogenic tumor: biologic profile based on 499 cases.

Topographically, the AOT occurs in peripheral and central variants, the latter further in follicular (with embedded tooth) and extrafollicular (no embedded tooth) types. The AOT is slow growing with few or no symptoms. Tumor growth may cause displacement of teeth rather than root resorption. The follicular AOT mimics a follicular cyst, the extrafollicular a residual or "globulo-maxillary" cyst and the peripheral a gingival fibroma. All variants of AOT show identical histologic features. The central variants account for 97.2%, 73.0% of which are follicular. The follicular variant (M:F ratio 1 to 1.9) is three times as frequent as the extrafollicular. The follicular variant is diagnosed earlier in life (mean age 17 yr) than the extrafollicular (mean age 24 yr). 53.1% of all variants occur within the teens (13-19 yr). Follicular AOT is associated with one embedded tooth in 93.2%. Maxillary permanent canines account for 41.7% and all four canines for 60.1% of AOT-associated embedded teeth. Ranking four among the odontogenic tumors the AOT is not a particularly rare tumor. Conservative surgical excision is the treatment of choice. Documented recurrences have not been reported.

[Effects of tetrandrine on hypertensive responses induced by electric stimulation and alpha-adrenoceptor agonists in pithed rats].

Tetrandrine (Tet) 5 mg.kg-1 i.v. inhibited the pressor action of norepinephrine (NE) release induced by electric stimulation (10 V, 0.5-16 Hz) of spinal cord T11-L2. However, Tet 5 mg.kg-1 ia did not obviously attenuate the hypertensive responses to NE 0.51-16.91 micrograms.kg-1 i.v., indicating that Tet did not affect the alpha 1-adrenoceptors-mediated vasoconstriction. Tet 5 mg.kg-1 ia decreased the pressor responses to NE 0.05 and 0.17 micrograms.kg-1 i.v. and markedly reduced the dose-dependent hypertensive responses to B-HT920 i.v., a selective alpha 2-adrenoceptor agonist, proving that Tet reduced alpha 2-adrenoceptors-mediated vasoconstriction.

Cleavage furrow formation of Rana amurensis eggs observed with fluorescence pattern photobleaching.

Taking advantage of the extremely slow lateral diffusion of proteins on Rana amurensis egg surface (Xu et al., 1984), it was possible to make a sharp concentric ring pattern on the egg surface by photobleaching of the fluorescein-labelled egg surface. The shape change of the pattern reveals the movement of the egg surface. The surface, even in front of the furrow tip, is drawn towards the furrow centre. The furrow tip differentiates into border and central lines as shown by the distribution of surface protrusions. Between the border and central lines the nascent membrane inserts while the width of furrow tip increases to nearly 100 microns.
