Colorectal cancer involving the urinary tract.

The survival rate among patients with colorectal cancer classified as Dukes' B or C who underwent extended resection due to infiltration of neighboring structures is significantly lower than among patients in the same Dukes' stage without infiltration of adjacent structures. Seventy-one patients with rectosigmoid cancer (37 primary and 34 recurrent tumors) involving the urogenital tract were retrospectively analyzed. Patients with primary tumors had a markedly higher mean survival rate after radical surgery (60.7 months) than after palliative procedures (28.2 months), while no such difference was seen among patients with recurrent tumors (17.6 or 14.1 months, respectively). From the urologic point of view, radical intervention is mandatory in cases of primary rectosigmoid carcinoma involving the urogenital tract. Aggressive surgery for recurrent tumors appears questionable, however; in these cases, priority should be given to improving the quality of life, recommending palliative surgery as the preferable approach.

Topical chemoprophylaxis of superficial bladder cancer with mitomycin C and adjuvant hyaluronidase.

The effect of intravesical instillation of 200,000 IU hyaluronidase in addition to mitomycin C as chemoprophylaxis of superficial bladder cancer was evaluated. Based on our result of a previous study that this approach results in a significantly reduced recurrence rate (7.1 versus 32.1%), 43 patients undergoing transurethral resection of Ta-T1 tumors were retrospectively analyzed after a mean observation period of 48.5 months. During the 2 years of prophylactic therapy, tumor recurrence was seen in 6 patients (13.9%). Of the 37 patients who remained recurrence-free under treatment, 5 (13.5%) had recurrent tumors later during a mean observation period of 24.5 (18-42) months after treatment. These values are significantly lower than those obtained previously from a group of 63 patients treated with mitomycin alone, with recurrence rates of 33.3% during chemoprophylaxis and 26.2% thereafter (mean observation period 50.4 months). It appears that additive hyaluronidase enhances the local effect of mitomycin C in the intravesical chemoprophylaxis of bladder cancer, presumably by improving diffusion into the bladder mucosa and catalyzing the breakdown of a hyaluronic acid contained in a protective halo around malignant urothelial cells.

High resolution ultrasonography in the diagnosis of simple intratesticular cysts.

Simple intratesticular cysts are a rare condition. High resolution sonography permits unequivocal sonographic differentiation between intratesticular cysts and neoplastic lesions with a cystic appearance, so that conservative treatment seems justified. Seventeen patients with sonographically diagnosed intraparenchymal testicular cysts were followed up for an average of 29 months. No increase in size or change in echo pattern was found and surgical therefore not indicated.

Therapeutic transrenal occlusion of the ureter: solution of plug migration problem.

Vesicovaginal fistulas in three patients and a vesicocutaneous fistula in one patient were treated by percutaneous transrenal occlusion of the ureter with a commercially available silicone occluder (Angiomed, Karlsruhe, Germany) which was secured by means of histoacryl placed on top. Urinary flow was diverted by a permanent nephrostomy tube. We observed no complications (e.g., urinary tract infection, occluder migration, or recurrence of urine discharge) at an average follow-up period of 9.1 months.

Long-term observation after intravesical metaphylaxis with mitomycin C in patients with superficial bladder tumors.

After transurethral resection of a superficial bladder tumor 63 patients were treated by chemoprophylaxis with mitomycin C over a period of two years: 33.3 percent experienced recurrent disease under metaphylaxis and 42 patients who remained recurrence-free during instillation therapy were observed further for an average of 26.4 months. During this period 26.2 percent of the patients showed recurrences ranging between two to forty-four months after completing instillation. The majority of the patients in whom recurrent tumors developed also had recurrent tumor processes before the instillation therapy was begun. Of the 42 patients 73.8 percent stayed recurrence-free during the observation period of twelve to fifty-four months after completing instillation therapy. In conclusion, during an average observation period of 50.4 months, 52.4 percent of the patients in this study showed recurrences.

Trace elements and urinary stone formation: new aspects of the pathological mechanism of urinary stone formation.

The urinary stone, serum and 24-hour urine concentrations of 14 trace elements were determined in urinary stone patients by inductively coupled plasma atomic-emission spectroscopy. The data obtained for 25 active stone patients and 32 whose last stone episode had occurred at least 12 months previously were compared with those of 25 healthy individuals. Urinary nickel, manganese and lithium excretion, and serum nickel, manganese and cadmium concentrations were statistically significantly lower for active stone patients compared to those with previous stone episodes and healthy individuals. No difference in the concentrations of trace elements could be found, however, for patients with previous stone episodes and healthy individuals. Nickel, manganese, lithium and cadmium could be of significance in the pathological mechanism of stone formation, not from mineralogical or crystallographic viewpoints but for the smooth flow of enzymatic reactions in biological systems.

Malignant hemangiopericytoma of the pelvis. Report of a case with urological implications and immunohistochemical analysis.

A case of a malignant hemangiopericytoma located in the pelvic fossa of a 50-year old man is reported. The tumor displaced the urinary bladder and caused bilateral hydronephrosis. Light microscopic diagnosis was followed by immunohistochemistry. After surgery the patient was successfully treated by chemotherapy and irradiation. This paper reviews clinicopathological features in respect of prognosis and a possible treatment.

Diagnosis of renal artery occlusion by duplex sonography and successful lysis therapy.

A case of partial renal artery occlusion in a 44-year-old man is reported. Diagnosis was established by Duplex sonography and confirmed by digital subtraction angiography. Subsequently performed local lysis therapy with recombinant tissue plasminogen activator achieved partial revascularization. Doppler evaluation of renal blood flow is useful to detect renal infarction.

Value of routine citrate analysis and calcium/citrate ratio in calcium urolithiasis.

24-hour urinary citrate excretion was measured in 176 calcium oxalate stone formers and 100 normal controls. A statistically significant difference (p less than 0.03) could be found between the two groups. When stone formers were divided into a group of 69 patients with recurrent calcium urolithiasis (RCU) and a group of 106 patients with a single stone episode, the latter did not differ from the control group, while in RCU a significantly lower citrate excretion compared with controls (p less than 0.005) could be found. Thus, patients with RCU could benefit from alkali citrate prophylaxis. A female-male difference in citrate excretion could not be found in either the control group or stone formers. Recurrent stone formers presented a significantly higher calcium/citrate ratio compared with controls, which would indicate an increased risk for stone formation. The value of routine citrate analysis is limited, however, by the great, variability of citrate levels in stone formers and controls.

[Percutaneous sclerosing of kidney cysts with alcohol].

Symptomatic benign renal cysts in 23 patients were obliterated by ultrasound-guided puncture and instillation of sterile 95% ethanol. In no case did complications occur. In 5 cases the cysts recurred after an average of 3 months; the primary size (diameter) was more than 10 cm in all of these. This simple technique seems to be a suitable treatment option for smaller cysts up to 10 cm because of its low complication rate and because no hospital admission is required.

[Complications of one-stage urethroplasties].

Different techniques applied for one-stage urethroplasty were compared with reference to complications. We followed up 48 patients operated on for bulbous or penile strictures caused by inflammation or by urethral irritation following endoscopic manipulation or catheterization. The techniques compared were end-to-end anastomosis, pedicled scrotal skin patch, penile/preputial island flap, free penile skin, free full-thickness skin patch. With pedicled scrotal skin patches there was a 60% complication rate, while after penile island flap technique complications occurred only in 13%. Scrotal skin should no longer be used for urethroplasties if penile skin is available.

[The significance of citrate excretion and calcium/citrate quotients in urine in patients with calcium calculi].

In 111 patient suffering from calcium oxalate lithiasis more than 168 citrate and calcium analyses have been performed in 24 hour urine. In 69 patients with a single stone episode no significant difference of citrate concentration was found in comparison with 63 healthy volunteers. However, in 42 recurrent stone formers was the citrate concentration significantly diminished (p less than 0.005). The calcium/citrate ratio was in recurrent stone formers markedly higher (5.19 +/- 8.6) than in patients with a single stone episode (3.34 +/- 4.9). However, because great deviation of citrate values the calcium/citrate ratio may not be suitable for evaluation of stone formation risk.

Effect of tamoxifen and kallikrein on sperm parameters including hypoosmotic swelling test in subfertile males. A retrospective analysis.

151 subfertile men presenting OAT-syndrome were investigated in respect of an improvement of sperm parameters under therapy with tamoxifen, kallikrein and combined treatment of both drugs. After therapy with 30 mg tamoxifen per day (75 patients), a significant elevation of density and swelling test was seen. Medication with 600 IE kallikrein per day (36 patients) revealed only a slight improvement of motility. Also combined treatment of 30 mg tamoxifen with 600 IE kallikrein per day produced only an elevation of motility without any significant change of other sperm parameters.
