[15N tracer technical studies of protein metabolism in arteriosclerosis patients].

The incorporation of the stable isotope 15N in plasma proteins and blood cells after oral application of 3 g 15NH4Cl (95 At% 15N) per 70 kg body weight was followed up in 11 patients with ischemic heart disease or peripheral arteriosclerosis and in 7 healthy control subjects. Preliminary results indicate that the turnover of plasma protein, especially fibrin, is elevated in patients with arteriosclerosis. Investigations of platelets intimate a decreased turnover of platelet protein in patients with arteriosclerosis as compared to control subjects. Possible reasons for these alterations are discussed.

[Recording protein metabolism and cell proliferation with the stable isotope 15N].

Investigations of nitrogen and protein metabolism with the stable isotope 15N were carried out on 11 patients with atherosclerosis and 7 healthy control subjects. After oral application of 3 g 15NH4Cl (95 At% 15N) per 70 kg body weight the incorporation of the isotope 15N in plasma proteins and blood cells and the 15N elimination in urine were followed up. Retardations of 15N elimination, an accelerated incorporation of 15N in fibrin and a retarded 15N incorporation in platelet protein were observed in patients with atherosclerosis. The described method enables complex assertions about protein metabolism of the whole body and so represents a possibility to evaluate objectively the influence of an intervention on metabolism.

The sensitizing potential of metalworking fluid biocides (phenolic and thiazole compounds) in the guinea-pig maximization test in relation to patch-test reactivity in eczema patients.

The sensitizing potential of seven industrial antimicrobial agents was evaluated using the guinea-pig maximization test. Preventol O extra (o-phenylphenol) did not produce a sensitization reaction. Preventol ON extra (sodium salt of o-phenylphenol), Preventol GD (dichlorophene) and Proxel XL and HL containing 1,2-benzisothiazolin-3-one were weak sensitizers, while Preventol CMK and Preventol L, both containing chlorocresol, were classified as extreme potential sensitizers. Both the weak and the extreme experimental sensitizers are occasional human sensitizers. The interpretation of the test results is discussed.

Guinea pig maximization tests with formaldehyde releasers. Results from two laboratories.

The guinea pig maximization test was used to evaluate the sensitizing potential of formaldehyde and 6 formaldehyde releasers (Forcide 78, Germall 115, Grotan BK, Grotan OX, KM 200 and Preventol D2). The tests were carried out in 2 laboratories (Copenhagen and Stockholm), and although we intended the procedures to be the same, discrepancies were observed, possibly due to the use of different animal strains, test concentrations and vehicles. The sensitizing potential was in general found to be stronger in Stockholm compared to Copenhagen: formaldehyde sensitized 50% of the guinea pigs in Copenhagen and 95% in Stockholm. For Grotan BK, the corresponding rates were 20% and 74%. Forcide 78, KM 200 and Preventol D2 sensitized 20% to 25% of the animals in Copenhagen, while Germall 115 and Grotan OX sensitized 60% to 70% of the animals in Stockholm.

How sensitizing is chlorocresol? Allergy tests in guinea pigs versus the clinical experience.

Chlorocresol is a biocide with widespread use in industry and pharmaceutical products. It is an occasional human contact sensitizer. The sensitizing potential of chlorocresol was judged strong using the guinea pig maximization test (GPMT) and doubtful in the less sensitive open epicutaneous test (OET). When different induction concentrations were used, the results indicated an optimal sensitizing concentration above which no further increase in the sensitization rate occurred. Rechallenge 2 weeks later showed a marked decrease in sensitivity. Consecutive human patch tests with chlorocresol 2% in pet. showed 11 reactions among 1462 patients tested, but none were explainable and reproducible during re-tests and provocative use tests, indicating that the GPMT overestimated the sensitization potential. The results from guinea pig allergy tests cannot stand alone but have to be validated by other sources of information.

Is Cytox 3522 (10% methylene-bis-thiocyanate) a human skin sensitizer?

Methylene-bis-thiocyanate is an antimicrobial agent in Cytox 3522 (American Cyanamid Corporation) and Nalco 206 (Nalco Chemical Company). Both are wide-spectrum industrial biocides. Cytox 3522 showed a strong sensitization potential in guineau pigs using the Guinea Pig Maximization Test and the Open Epicutaneous Test procedures. In spite of this, human sensitization has not been reported. This may be explained by several factors such as limited exposure, low-use concentration and a lack of diagnostic patch tests in selected patients.

Allergic contact dermatitis from 2-n-octyl-4-isothiazolin-3-one, a paint mildewcide.

Severe allergic contact dermatitis from a paint mildewcide, 2-n-octyl-4-isothiazolin-3-one, developed in a worker formulating latex paints within a paint manufacturing company, Guinea pig maximization testing demonstrated this to be a moderate sensitizer. Further cases of allergic contact dermatitis may be encountered as the use of this biocide increases in the market place.

Cost benefit of patch testing with textile finish resins.

Eleven years experience of textile finish resin patch testing of suspected textile dermatitis patients revealed 15 cases of allergic textile dermatitis among 428 patients tested. Ten of the 15 patients had a relevant positive patch test to one or more of a limited series of textile finishes; 1 was negative and 4 were not tested with textile finishes. All 15 patients were formaldehyde sensitive. No unexpected, relevant, positive textile finish resin patch test was found. In this study a negative patch test to formaldehyde virtually excluded allergic contact dermatitis from textile finishes.

Delusions of infestation treated by pimozide: a double-blind crossover clinical study.

Eleven patients with delusions of infestation were treated with pimozide using a double-blind crossover technique. Ten out of 11 patients improved. The investigation showed significant relief of itch and delusions during pimozide treatment. Brief Psychiatric Rating Scale (BPRS) points decreased significantly. Adverse effects were few by 2 patients showed dose-related depressive reactions during treatment. Pimozide treatment of patients with delusions of infestation should be entrusted to dermatologists, because these patients with delusions differ from most patients attending psychiatric clinics in that they have a monosymptomatic hypochondriacal psychosis.

Decreased lymphocyte aryl hydrocarbon hydroxylase and glutathione S-transferase activities in patients with hand dermatitis.

Aryl hydrocarbon hydroxylase (AHH) and glutathione S-transferase (GST) (EC 2.5.1.18) activity were measured in human lymphocytes of peripheral blood from 8 patients with irritant or allergic contact dermatitis of the hands, and compared with data from a control group. Both AHH and GST activity was found to be significantly depressed in the allergic and irritant contact dermatitis subjects, as compared with controls. It is suggested that AHH may be a quantitative marker of the inflammatory status.

Onychotillomania treated with pimozide (Orap).

A case of onychotillomania in a 70-year-old woman is presented. The disease had persisted for 51/2 years. All her fingernails were plucked away. The patient had a fixed hypochondrial delusion of nail disease. After treatment with pimozide (Orap) for 7 months the nails were normal.

[Chronic hypervitaminosis A with skin changes].

A case of hypervitaminosis A in a 43-year-old woman is presented. The patient had only dermatologic symptoms. The dermatitis had resemblance to contact dermatitis or pellagra. The patient had been eating 100,000 I.U. of vitamin A daily every 2 months for approximately 15 months. The dermatitis disappeared slowly after stopping the intake of vitamin A.
