[Case of simultaneous triple cancers of the duodenal papilla, gallbladder and stomach].

The patient was a 76-year-old man with simultaneous triple cancer arising in the duodenal papilla, gallbladder and stomach, whose complaint was epigastralgia. Endoscopic and radiologic studies revealed early gastric cancer (IIc) at the posterior wall of the antrum; ERCP disclosed obstruction with "apple core" appearance in the distal bile duct. Pancreaticoduodenectomy, cholecystectomy and distal gastrectomy with lymph node dissection were performed. Histopathological examination of the resected specimens revealed papillo-tubular adenocarcinoma in the duodenal papilla, well differetiated tubular adenocarcinoma in the gallbladder, and moderately differentiated tubular adenocarcinoma in the gastric mucosa.

[A Lance-Adams syndrome of a diver (author's transl)].

A 39 years old man with severe action myoclonus and epileptic seizure was reported. He had been in good health and worked as an occupational diver until August 16, 1966, when he had to dive into the sea bottom about 35 m deep without diving equipment. After finishing his work, he fell into unconsciousness and drowned. Sometime later, he was rescued and treated as decompression sickness. When he became alert, severe action myoclonus and epileptic seizure appeared. 8 years later, he was admitted to our hospital for rehabilitation. The danger of diving without equipment was indicated, because in sea bottom alveolar oxygen pressure (Pao2) is about 450 mmHg and alveolar air pressure 4.5 atmospheric pressure. In this condition patient could hold his breath longer time than in sea surface. When he finished his work and floated with muscle relaxing, however, his blood supply to the brain decreased. It was thought, therefore, that he fell into unconsciousness and drowned. In somatosensory evoked potentials (SEP) by median nerve stimulation, giat patterns were recognized but there were differences between SEP of myoclonus epilepsy and that of this patient. P50 and N70-80 were giant in him, suggesting that he had a brain stem disturbance. Since the inhibition system of fast conducted fiber did not work well in this state, the component of slow conducted fiber was magnified. Action myoclonus may be due to the disturbance of inhibition system in the brain stem.

Isolation of Chlamydia trachomatis from the middle ear aspirates of otitis media.

Chlamydia trachomatis was cultured from the middle ear aspirates of patients with acute otitis media and otitis media with effusion (OME) to assess the relationship between the microorganism and middle ear disease. The agent was recovered in 2 of 6 patients with acute otitis media, in 3 of 13 with acute OME and in 9 of 31 with chronic OME. Serum antibody to the agent was detected in 8 of 9 patients with C. trachomatis infection in the middle ear. Chlamydial infection was found, serologically or by isolation of the agent from the genital tract, in the parents of 5 of 6 children with the infection. Therefore perinatal transmission appeared to be the most probable mode of infection in the infants. These studies indicate that C. trachomatis causes middle ear diseases more often than has been suspected.
