[Significance of priming of hosts with virus in the tumor-specific immunotherapy model utilizing virus-reactive helper T cell activity].

A tumor-specific immunotherapy model was established utilizing vaccinia virus (VV) in the consideration of clinical application, in which we assessed the significance of priming of hosts with VV to induce in vivo anti-tumor immunity. C3H/HeN mice were pretreated with cyclophosphamide (Cy) to eliminate non-specific suppressor T cell activity and subsequently inoculated (primed) subcutaneously (s.c.) with 10(7)PFU of VV, leading to augmented induction of VV-reactive helper T (VV-Th) cell activities. Four weeks later, the mice were inoculated intradermally (i.d.) with syngeneic X5563 myeloma cells. Six days after the tumor cell inoculation, 5 X 10(7)PFU of VV was injected into the tumor mass 3 times at 2-day intervals. Seven of 10 mice which had received VV-priming and subsequent VV-injection into the tumor mass exhibited complete tumor regression. On the contrary, mice which had received mere intratumoral VV-injection in the absence of VV-priming failed to exhibit appreciable tumor regression. Mice (regressor mice) whose tumor had completely regressed by the above VV-immunotherapy were shown to have acquired systemic anti-tumor immunity which was confirmed by a challenge with syngeneic tumor cells after the tumor regression. In vitro analysis of these immune mice revealed that potent tumor-specific cytotoxic T lymphocyte (CTL) responses were preferentially induced, but with no detectable anti-tumor antibody responses. Such a potent tumor specific immunity was not observed in mice which had received intratumoral VV-injection in the absence of VV-priming.(ABSTRACT TRUNCATED AT 250 WORDS)

[Successful treatment of a patient with recurrent ovarian cancer by lentinan combined with intraarterial 5FU].

A patient with ovarian cancer recurrent in the pelvis showed partial response to consecutive intraarterial (IA) cisplatin (CDDP) combined with continuous IA 5FU treatment and received third look operation in which the recurrent tumor could not be completely removed. The biopsied specimen revealed that the histologic type of the recurrent tumor was papillary adenocarcinoma. Postoperative IA CDDP could not produce further effect against the remaining tumor. Thereafter, the patient has been treated ambulatorily with intravenous (IV) lentinan (2mg/wk) combined with IA 5FU (without CDDP) to date. Four months after the start of this therapy, the persistent tumor which became refractory to IA CDDP disappeared completely (CR). Such a remarkable effect was accompanied by the concurrent induction of significantly enhanced IL-2 production and increased ratio of Leu3a/Leu2a possibly by lentinan. At present, performance status of the patient is 0 (normal activity) and she has been free of disease for 6 months, which is confirmed by clinical assessment including physical examination, cytologic examination, CT, scintigraphy and B scope.

Biochemical aspect of HMG CoA reductase inhibitors.

Subsequent to the discovery of compactin (ML-236B) as a specific inhibitor of HMG CoA reductase, a series of compactin analogs have been either isolated or synthesized. Several of these compounds, which include compactin, mevinolin (monacolin K) and CS-514, have been extensively studied. The inhibition of HMG CoA reductase by these compounds is reversible and competitive (Ki = approximately 1 nM). The 3', 5'-dihydroxypentanoic acid portion of the acid form of compactin analogs, which resembles the HMG portion of HMG CoA, plays a crucial role in inhibitory activity. These inhibitors block sterol synthesis both in cultured mammalian cells and in animals. Strong inhibition of sterol synthesis results in a marked increase in HMG CoA reductase activity both in vitro and in vivo. These compounds strongly lower plasma LDL-cholesterol levels in animals and humans. The lowering of LDL-cholesterol levels occurs by an inhibition of LDL synthesis and/or by an elevation of the receptor-mediated LDL catabolism in the liver.

Clinical study of primary carcinoma of the fallopian tube: experience with 15 cases.

Between 1950 and 1986, fifteen cases of primary carcinoma of the fallopian tube were diagnosed and treated at the Cancer Institute Hospital, Tokyo. These cases constituted 0.14% of the total number of gynecologic malignancies at the hospital during that period. The average age of the 15 patients was 55.7 years. The most frequent symptom was atypical genital bleeding, seen in 12 cases (80%). Massive watery discharge was seen in 4 cases (27%). In preoperative cytologic examination, 6 cases (40%) were positive for cancer. All cases underwent operation as therapy. Postoperative irradiation, adjuvant chemotherapy, and/or second- or third-look operations were also performed. Histopathologically, all materials were found to be adenocarcinoma. Four cases were well differentiated, eight were moderately differentiated, and three were poorly differentiated. As for the prognosis, 7 patients were followed more than 5 years postoperatively. The 5-year survival rate was 57% (4/7). In stage I cancers, the 5-year survival rate was 80% (4/5). The prognosis of stage I cancer patients was estimated to be rather good.

Use of angio computed tomography to evaluate extent of endometrial carcinoma.

Patients with endometrial carcinoma underwent angio computed tomography (CT) prior to surgery to evaluate the extent of the carcinoma. The subjects of the study were 87 patients operated on at the Cancer Institute Hospital during the period 1983 to 1986. After preoperative bilateral internal iliac angiography, angio CT was performed using the already inserted catheters. Although the intact uterine wall was enhanced by the angio CT, the cancerous lesion was not, which permitted easy recognition of the cancerous lesion as a low-density area. To evaluate the degree of myometrial invasion, the minimal thickness of intact uterine wall was measured in the CT image, and was compared later with findings from the operative materials. The comparison showed that for all the patients, the minimal thickness of the intact uterine wall as shown in the CT image was comparable to the actual minimal thickness of the operative materials. Because depth of myometrial invasion is known as an important prognostic factor, measurement of the minimal thickness of the intact uterine wall as shown in the CT image is useful as an objective prognostic factor. Angio CT facilitates recognition of myometrial invasion preoperatively.

Management of patients with gynecologic cancer by serum sialic acid determination.

With an enzymatic technique, serum sialic acid (SA) levels were determined in patients with gynecologic tumors. Since the SA level for healthy females was 57.4 +/- 7.3 mg/dl, we set the upper normal limit of this parameter at 72 mg/dl. The SA level became larger in ascending order of uterine myoma, benign ovarian tumor, cervical cancer, corpus cancer, and ovarian cancer. The SA level proved to be significantly higher in cancer patients showing poor prognosis than in those having good prognosis irrespective of the category of therapy and it reflected well the clinical course of cancer patients. In patients who receive a combination therapy and need to be followed up for a long term, complete follow-up is sometimes impossible with tumor-derived markers alone. Even in such cases, the sialic acid level will work as a useful follow-up marker because it is nonspecific to histologic types of cancer.

[Boron neutron capture therapy using 10B entrapped anti-CEA immunoliposome].

A new murine monoclonal antibody (2C-8) was prepared by immunizing mice ip with CEA producing human pancreatic cancer cell line, AsPC-1.SDS-PAGE and Western blot analysis showed that 2C-8 monoclonal antibody recognized CEA and NCA. This anti-CEA monoclonal antibody was conjugated with large multilamellar liposomes incorporated 10B compound (Cs2 10B12H11SH). This immunoliposomes applicated to boron neutron capture therapy. AsPC-1 cells were incubated with the 10B-Lip-MoAb(CEA) for 8 hours. After the irradiation with thermal neutron (1 x 10(11)-1 x 10(13) n/cm2), boronated AsPC-1 cells were showed decreasing uptake of 3H-TdR compared with control group. The numbers of 10B atoms in liposomes bound to an antibody were in proportion to the dose of 10B compounds added and maximum number of 10B atoms was approximatory 1.2 x 10(4)/Ab. These data indicated that the immunoliposomes could deliver highly amount of 10B atoms to the tumor cells and exert cytotoxic effect by thermal neutron. BNCT with immunoliposome may be useful to the non resectable malignant tumors in clinical application.

[CDDP-ACR treatment in patients with recurrent ovarian cancer with prior chemotherapy containing CDDP--a preliminary study of a 14-day continuous infusion of CDDP with ACR].

Six patients with recurrent ovarian cancer who had prior chemotherapy were studied for the clinical efficacy of CDDP-ACR treatment. Five out of the 6 had received CDDP a total doses of 1,320, 780, 750, 475, and 340 mg. CDDP-ACR therapy consisted of continuous infusion of CDDP at a daily dose of 10 mg/m2 over 14 days (total CDDP doses; 140 mg/m2) and of intermittent infusion of ACR (aclarubicin) at a dose of 20 mg/body every other day (total ACR doses: 140 mg). There were one CR and five PR and a response rate up to 100% was noted. Toxicity was manifested in slight nausea or vomiting, but there was no nephrotoxicity. However bone marrow was severe. Thrombocytopenia less than 50,000/mcl in 4 pts (67%) and leukopenia less than 1,000 mcl in 3 pts (50%). The mean filterable platinum exposure measured by area under the concentration-time curve (AUC) was as high as 19.7 +/- 6/0 mg.hr/ml. In conclusion the bone marrow toxicity in this regimen was severe, but the therapeutic efficacy was promising. Further studies on the appropriate infusion time and the minimum effective dose of CDDP are needed.

[Comparison of ultrasound and computed tomography (CT) for the diagnosis of paraaortic lymphnode metastasis in patients with gynecologic malignancies].

Fifty-one patients with gynecologic malignancies were tested for paraaortic lymphnode metastasis by ultrasound and computed tomography (CT). Eleven patients were found to have paraaortic lymphnode metastasis by ultrasound, and CT also showed the same metastatic image in the same patients. The site and size of the positive image found by both means were identical. The other forty patients diagnosed by ultrasound as free of metastasis were also proved to have no metastasis by CT. Ultrasound is as useful as CT in the evaluation of paraaortic nodes. However, ultrasound is more convenient and easier to use than CT.

[Clinical study of 84 primary vulvar carcinomas].

Between 1949 and 1987, 84 cases of primary vulvar malignancy were treated at the Cancer Institute Hospital. These cases accounted for 0.8% of the total number of gynecologic malignancies at our hospital during the same period. The clinicopathological findings were described. The mean age of the 84 cases was 61.7 years. The most frequent symptom was a vulvar mass, seen in 35 cases (41.7%). In preoperative cytologic examination of vulvar smears, 44/78 cases (56.4%) had suspected malignancy. Histopathologically, 57 cases (67.9%) were found to be squamous cell carcinoma. According to FIG0 staging system, there were 4 (4.8%) stage 0 tumors, 11 (13.0%) stage I tumors, 25 (29.8%) stage II tumors, 41 (48.8%) stage III tumors, and 3 (3.6%) stage IV tumors. The patients were treated by operation and/or irradiation. The five-year survival rate was 51.4% (36/70) in all stages. The five-year survival rate was 66.7% (2/3) in stage 0 cases, 87.5% (7/8) in stage I, 78.9% (15/19) in stage II, 28.9% (11/38) in stage III, and 50.0% (1/2) in stage IV, respectively. In patients treated by radical vulvectomy, the five-year survival rate was 26.7% (4/15) when there was inguinal nodal metastasis, and 73.7% (14/19) when there was no inguinal nodal metastasis. The five-year survival rate was 59.5% (25/42) in cases treated by operation and was 39.3% (11/28) in cases treated by irradiation. In preoperative adjuvant chemotherapy, the histopathological effect was 44.4% (4/9). In conclusion, the first choice for treatment of vulvar malignancy was considered the operative method.

[Cervical cancer after subtotal hysterectomy (so called "stump cancer")--clinical studies on 226 cases].

The Department of Gynecology of the Cancer Institute Hospital diagnosed and treated 226 cases of stump cancer between 1950 and 1983. We obtained the following results. 1) The age of the subjects ranged from 32 to 78 years. The highest frequency age group was 50 to 59 (44.7%) with 101 cases, giving an average age of 53.8 years. 2) Subtotal hysterectomy was performed at ages 23 to 64, the average age being 39.5. 105 patients (46.5%) underwent subtotal hysterectomy between the ages of 40 and 49. 3) The major reasons for subtotal hysterectomy were myoma in 181 cases (80.1%), benign ovary tumor in 12 cases (5.3%), ectopic pregnancy in 10 cases (4.4%), cystic mole in 6 cases (2.7%) and cesarean section in 5 cases (2.2%). 4) Major subjective symptoms of stump cancer were metrorrhagia in 164 cases (72.6%) and fluor in 30 cases (13.3%). 24 cases (10.6%) were asymptomatic. 5) The hystological types were squamous cell carcinoma in 208 cases (92.0%), adenocarcinoma in 17 cases (7.5%), and adenosquamous cell carcinoma in 1 case (0.4%). These ratios were similar to those of cervical cancer cases treated over the same period. 6) The stages of cancer advancement were stage 0 in 24 cases (10.6%), stage I in 51 cases (22.6%), stage II in 96 cases (42.5%), stage III in 50 cases (22.1%), and stage IV in 5 cases (2.2%). The percentage of stage 0 cases was lower than that for general cervical cancer of the uterus.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinicopathological study on clear cell carcinoma of the ovary].

The clinical and pathological findings in 23 cases of clear cell carcinoma of the ovary treated at Cancer Institute Hospital from January 1951 to September 1987 are discussed. Clear cell carcinoma accounted for 7.1% of all malignant tumors of the ovary. In the last 21 months of the study, the incidence of clear cell carcinoma increased to 23.7 at the institute. This tumor was frequently associated with endometriosis, and the ratio of nulliparous women was as high as 47.8%. Operative findings revealed adhesion of the tumor to surrounding tissues in more than half of the cases. Moreover, rupture of the tumor capsule during operation occurred in 42.1% of the cases. Incidences of clear cell carcinoma by clinical stage were 82.6% for stage I, 8.7% for stage II, 8.7% for stage III, and 0% for stage IV. The incidences of serous carcinoma were 20.8%, 24.2%, 45.8%, and 9.2%, respectively. Histologically, clear cell carcinoma could be divided into solid and tubular types, with the solid type being slightly more frequent. The three-year survival rate for stage I cases was 95.2% for serous carcinoma and 54.5% for clear cell carcinoma, showing a poor prognosis for clear cell carcinoma. When classified by the histologic type of clear cell carcinoma, the three-year survival rate was 50.0% for solid type and 60.0% for tubular type, with no significant difference. For clear cell carcinoma, the prognosis was poor despite even though most cases were stage I.

[A study of the leucine enkephalin like substance and enkephalin degrading enzyme activity in human cerebrospinal fluid].

Leucine enkephalin like substance (Leu-Enk) and enkephalin degrading enzyme activity (EDEA) in cerebrospinal fluid (CSF) of three patients suffering from pain were measured. As compared with control values before treatments, the values of Leu-Enk in CSF decreased, and those of EDEA increased in all the patients. After the treatment, two patients were fortunately cured of pain, and their two values returned to normal. But the other patient could not recover from severe pain, and their values remained abnormal. It is concluded that Leu-Enk and enkephalin degrading enzyme in CSF may play an important role in regulation of pain sensitivity.

[The combined effect of cisplatin and aclarubicin on human lymphocytes in vitro].

The combined effect of cisplatin (CDDP) and aclarubicin (ACR) which was reported to inhibit the repair of the sublethal and the potentially lethal X-ray damages, was studied with human lymphocytes. The cell kinetics during administration of CDDP or ACR were analysed by flow cytometry. CDDP at the concentration of 0.5 microgram/ml had no effect on the cell progression. ACR blocked the cell progression at G1 period at the concentrations of 100 and 200 ng/ml, but the number of dead cells was not increased. When these two agents were combined, the number of dead cells increased more than 10 times compared to that of ACR alone. In conclusion, it was found that the combination of CDDP with ACR had a synergistic effect.

Peritoneal fluid cytology and prognosis in patients with endometrial carcinoma.

Cytologic examination was performed on peritoneal fluid collected from a total of 235 consecutive patients with endometrial carcinoma who underwent laparotomy as the initial treatment at the Cancer Institute Hospital from 1971-1985. The rate of cases positive for cancer cells in the peritoneal cytologic examination was 18.7% for all stages (44 of 235 cases). In stage I endometrial carcinoma, the 5-year and 10-year cumulative survival rates with positive peritoneal cytologic findings were 91.6 and 90.0%, respectively; those in cases with negative cytologic findings were 90.2 and 90.2%, respectively, with no significant difference. The recurrence rate in the same stage was 12.0% for cytologically positive cases versus 9.5% for negative cases. In stages II and III, no significant difference was noted in the survival rate between nine cases with positive peritoneal cytologic findings without macroscopic peritoneal metastasis and 47 comparable cases with negative cytologic findings. Therefore, in endometrial carcinoma, the presence of malignant cells in peritoneal fluid is not a useful prognostic factor.
