[Immunohistochemical study of new monoclonal antibodies--E29, 115D8 --to an epithelial membrane antigen (EMA) in ovarian tumors].

Mouse monoclonal antibodies E29 and 115D8 have been raised in the human milk fat globule membrane. The distribution of the antigen detected by these two monoclonal antibodies has been examined in formalin-fixed paraffin-embeded human ovarian tumors by an indirect immunoperoxidase technique. The highest positive rate of E29 and 115D8 was obtained in the malignant group suggesting that there is relationship between this positive rate and the histological malignancy. As for serous cystadenocarcinomas, a strong staining area of E29 was observed in the diffuse cytoplasm in poorly differentiated cases. These results suggest that the presence of E29 is a useful tumor marker for ovarian cancer.

[Metastatic brain tumor after chemotherapy of ovarian cancer].

A case of brain metastasis in ovarian cancer is reported. Recently cis platin-containing combination chemotherapeutic regimens have improved the prognosis of patients with advanced ovarian carcinoma. Metastasis to the brain in ovarian cancer is very rare. However, the incidence of cerebral metastasis after chemotherapy for ovarian carcinoma seems to be increasing, and this may be related to prolonged survival due to chemotherapy. Thus, neurological examination after chemotherapy with cis platin is necessary to detect metastases to the brain.

[Hachimijiogan having no estrogenic activity is effective for senile colpitis].

The effects of Hachimijiogan, a Chinese medicine, were studied in patients with senile colpitis. Eighty-eight postmenopausal patients, aged 42-76 years, were treated. Hachimijiogan was effective in 65 of 66 cases with abnormal discharge, 10 of 11 with pruritus vaginae and 60 of 62 with subepithelial hemorrhage. Overall more than ninety per cent of the patients responded well. The serum FSH, E1 and E2 levels were not statistically changed in this treatment. The vaginal Pap smear showed a significant decrease in parabasal type cells and an increase in intermediate type cells (p less than 0.01). Radioimmunoassay of E1, E2, E3, testosterone, FSH, LH and prolactin in Hachimijiogan were below the levels of sensitivity. These data suggest that Hachimijiogan is effective for senile colpitis and patients with senile colpitis associated with estrogen dependent tumors could be safely treated by Hachimijiogan.

[Clinical study of primary carcinoma of the fallopian tube: experience with 14 cases].

Between 1950 and 1986, 14 cases of primary carcinoma of the fallopian tube were treated and diagnosed at the Cancer Institute Hospital. These cases constituted 0.13% of the total number of gynecologic malignancies at the hospital during the period. The clinical/pathological findings and prognoses were described. Of the 14 cases, the average age was 56.0 years. The most frequent symptom was atypical genital bleeding, seen in 11 cases (79%). Massive watery discharge was seen in four cases (29%). In preoperative cytologic examination of vaginal smears, six cases (43%) were positive for cancer. All cases underwent operation as therapy. Postoperative irradiation, adjuvant chemotherapy, and/or second-or third- look operation was also used. Histopathologically, all materials were found to be adenocarcinoma. Four cases were well differentiated, seven were moderately differentiated, and three were poorly differentiated. Two patients with stage III and IV cancers died of the disease. Nine patients were still alive at the end of this study. The five-year survival rate was 57% (4/7). In stage I cancers, the five-year survival rate was 80% (4/5). The prognosis of stage I cancer patients was estimated as rather good.

[Histology and prognosis of endometrial carcinoma].

The relationship between the histology and prognosis has been analyzed, using 608 surgically removed uterus with endometrial carcinoma. Among the uterus studied, 506 cases (92%) were determined as being an adenocarcinoma, endometrial type, including adenoacanthomas. This type of carcinoma showed a favourable postoperative survival rate (83.3%), when compared to serous adenocarcinomas (65.8%), clear cell (mesonephroid) adenocarcinomas (32.4%), and adenosquamous carcinomas (63.7%). Among adenocarcinomas of the endometrial type, these with a well differentiated variant showed a better prognosis than others. The squamous component of the adenoacanthoma was found to play no significant role in the prognosis.

[Epidemiologic study of carcinoma in situ of the cervix].

During the period from 1950 to 1983, 1633 patients with cervical carcinoma in situ (CIS) were treated at the Cancer Institute Hospital, Tokyo. These cases were retrospectively investigated to determine the relation between the epidemiologic features and social phenomena. The number of patients with CIS tends to increase year by year. The incidence of CIS tends to be larger in the younger age group. Delay in the age of primiparity, a lower incidence of multiparas and a higher incidence of artificial abortion were seen in the CIS patients. Recurrence was seen in 0.4% of the CIS patients, and the recurrence site was most frequently the vaginal stump.

[Epidemiological survey on the relationship between obesity and endometrial cancer].

Endometrial carcinoma is found more frequently in obese subjects. In this study, we evaluated the comparative risks of endometrial cancer relative to obesity indices calculated using the Brocas coefficient. From 1979 to 1983, we treated 185 patients with endometrial cancer at the Cancer Institute Hospital, Tokyo. Compared to nonobese subjects, those who exhibited an obesity index of 1.2 or more were found to have a relative risk value of 2.0 or more times that of normal subjects across all age groups. Based on these findings, it is suggested that a practical and effective preventive measure against endometrial cancer is to avoid becoming obese.

Five years experience of cardiopulmonary resuscitation in a children's hospital.

The emergency call for resuscitation in a children's hospital is reported. Forty-seven resuscitation attempts via the emergency call were made in 43 patients over the past 5 years; 24 out of 43 patients had heart diseases. Cardiac patients were more likely to die within a few days after their resuscitation. The patients, who had previously been in cardiac arrest, lost their lives more often. These results implied that it was very crucial to prevent cardiac arrest especially for cardiac patients in order to save lives. Among the causes of emergency calls, complications with the endotracheal tubes were most common (13) and the airway obstructions followed (11). Summation of two causes, the airway problems, accounted for more than half of the series. Sixteen out of all emergency calls occurred on weekends, and all endotracheal tube troubles broke out during a night shift or on a holiday, that is, the lives of those patients were threatened especially when fewer staff were on duty. Those accidents were embarrassing for us because we believe that all patients must have been under our surveillance all day. Some hospitals have a successful resuscitation rate, organizing a cardiopulmonary resuscitation (CPR) team. However, we obtained better results (about 30%) without a team. It should be borne in mind that it is more desirable to prevent cardiac or respiratory arrests initially rather than save the patients in emergencies.

Regulation of cholesterol synthesis in cultured mouse mammary carcinoma FM3A cells.

Mouse mammary carcinoma FM3A cells, which are able to grow in a serum-free medium, have novel characteristics that could be valuable in biochemical and somatic cell genetic studies. In FM3A cells grown in the presence of serum, both sterol synthesis and the activity of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase, the major rate-limiting enzyme in the cholesterol biosynthetic pathway, were strongly suppressed by human low density lipoprotein (LDL). The addition of LDL (50 micrograms protein/ml) resulted in a 50% decrease in the reductase activity within 3 h and a 95% reduction after 24 h. Similarly, over 90% suppression of the reductase activity was obtained by the addition of LDL or mevalonolactone when the cells were grown on a serum-free medium. ML-236B (compactin), a specific inhibitor of HMG-CoA reductase, inhibited sterol synthesis from [14C]acetate by 80% at 1 microM. Reductase activity in FM3A cells was increased by 2.5- to 5-fold when the cells were treated with ML-236B (at 0.26-2.6 microM for 24 h). Thus, in FM3A cells, HMG-CoA reductase activity responded well to LDL, as is observed in human skin fibroblasts. Along with other novel features of this cell line, the present observations indicate that FM3A cells should be useful in biochemical and somatic cell genetic analysis of cholesterol metabolism, especially as regards the regulation of HMG-CoA reductase activity.

Dihydromonacolin L and monacolin X, new metabolites which inhibit cholesterol biosynthesis.

Dihydromonacolin L and monacolin X those are structurally related to monacolin K, a potent inhibitor of cholesterol biosynthesis, were isolated from cultures of a mutant of Monascus ruber. The structures of these two metabolites were determined by a combination of physical techniques. Data for dihydromonacolin L and monacolin X as inhibitors of hydroxymethylglutaryl-CoA reductase and sterol biosynthesis in vitro are also given.
