[Rectosigmoidectomy following neoadjuvant chemotherapy for advanced ovarian cancer].

Eleven women underwent rectosigmoidectomy as part of an en bloc removal of the adnexal masses, uterus, and pelvic peritoneum during cytoreductive surgery following neoadjuvant chemotherapy for advanced ovarian cancer. While 7 patients with stage IIIc underwent an exploratory laparotomy, 4 cases with stage IV did not because of their poor performance status (PS). Histologic subtypes were as follows: serous, 10 cases; endometrioid, 1 case. Patients with pleural effusion and ascites received immunotherapy. Thereafter, all patients were treated with 4-6 courses of neoadjuvant chemotherapy with cisplatin containing combinations, which delivered 9 PR, 1 MR and 1 NC. The PS score was improved by neoadjuvant chemotherapy and immunotherapy which may contribute to the low mortality and morbidity. Residual tumor of the largest diameter after debulking surgery were as follows: absent (5 cases), present (< 0.5cm; 2 cases, < 2cm; 3 cases, > or = 2cm; 1 case). The median survival for the entire group was 32.6 months. None of the 7 cases with residual disease < 0.5cm died of the disease within 2 years. Rectosigmoidectomy following neoadjuvant chemotherapy might be a useful procedure for patients who will be left with minimal residual disease after completion debulking surgery.

[Comparison between old and new FIGO staging of endometrial carcinoma--the significance of positive peritoneal cytology].

Three hundred fifty-two patients with endometrial adenocarcinoma who underwent primary surgical therapy between 1984 and 1991 were evaluated. (1) Eighty-three (23.6%) had positive peritoneal cytology (PPC). (2) The cumulative 5-year survival rates (5-y. survival rate) for the former clinical stages were 88.8%, 86.1%, 52.9% and 0% for stages I-IV, respectively, showing a significant difference between stage III and IV (p < 0.05). The new clinical stage 5-5. survival rates were 97.5%, 100%, 69.9% and 25.9% for stages I-IV, respectively. There was no significant difference between stages I and II, but there were significant differences between stages I.II and III (p < 0.001) and between stages III and IV (p < 0.05). (3) In cases of new stage III, 5-y. survival rates for stage IIIA and IIIB.C were 83.7% and 33.6%, respectively (p < 0.01). Those for stage IIIA with PPC were very good in comparison with stage IIIA cases with other factors (90.3% vs 49.2%; p < 0.05). When the patients in stage IIIA with only PPC were separated into 2 groups, i.e., a myometrial invasion less than 1/2 and no cervical involvement group and a myometrial invasion greater than 1/2 or/and cervical involvement group, their 5-y. survival rates were 97.2% and 75.1% respectively (p < 0.05). (4) Recurrence developed in 15.3% of patients in stage IIIA with only PPC, and in 64.7% of patients in stage IIIA with other factors (p < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

[Pelvic lymph node metastasis in endometrial cancer].

Various factors associated with pelvic lymph node metastasis were analysed in 393 cases with stage I endometrial cancer treated with extended total hysterectomy, bilateral salpingo-oophorectomy and pelvic lymphadenectomy. The results are as follows. 1. The positive rates for lymph node metastasis and the relapse rate in 393 cases were 8.4% and 12.5%, respectively. 2. Myometrial invasion, tumor size, vessel permeation, cervical involvement, peritoneal cytology and adnexal metastasis were significant factors for lymph node metastasis. 3. Internal iliac nodes were the most common metastatic sites for cases with a single node metastasis, whereas external iliac nodes were the most common in all cases with lymph node metastasis. 4. The relapse rate for cases with single node metastasis was 40%, while the relapse rate for cases with three or more metastatic lymph nodes was 70%. 5. The relapse rate for 33 cases with lymph node metastasis was 54.5% and all of the relapse cases showed distant recurrence, whereas the relapse rate for 360 cases without lymph node metastasis was only 8.6%. It is therefore considered that pelvic lymphadenectomy is important in detecting high-risk patients and to reduce the relapse rate. Cytotoxic chemotherapy seems to be more adequate as adjuvant therapy for cases with lymph node metastasis.

[Prognostic factors of endocervical type adenocarcinoma in uterine cervix].

Adenocarcinoma of the uterine cervix accounts for an increasing percentage of cervical cancers with controversial clinical problems. Three hundred and thirty-seven cases of endocervical type, adenocarcinoma cases were treated at the Cancer Institute Hospital between 1950 and 1984. Of the many clinicopathologic variables evaluated for prognosis, the following were significant. (1) Clinical Stage: Five and 10 year survival in each stage was: 83.3% and 78.0% (stage I), 71.6% and 55.8% (stage II) and 27.1% and 19.7% (stage III), respectively (p < 0.01, < 0.05). (2) Myometrial invasion: One hundred and forty stage Ib.II patients were operated on and then separated into 3 groups according to the degree of myometrial invasion. Five and 10 year survival in the "< 1/3" group was 92.7% and 86.9%, the "1/3-2/3" group 86.6% and 82.3% and the "> 2/3" group 75.0% and 63.2%, respectively. There was a significant difference between the "< 1/3" and "1/3-2/3" groups and the "> 2/3" group (p < 0.05). (3) Pelvic lymph node metastasis: Five and 10 year survival in negative cases of pelvic lymph node metastasis was 90.7% and 85.8%, compared with 40.9% and 29.8% in positive cases (p < 0.05). (4) Cardinal invasion: Minimal 5 year survival for negative cases was 71.2%, compared with 11.1% for positive cases (p < 0.001). (5) Surgical therapy for early stage cancer: Minimal 5 year survival for operated cases was 81.3%, compared with 58.5% for irradiated cases (p < 0.01).

[Effects of 5'-DFUR against BALB/c retroperitoneal sarcoma with spontaneous liver metastases].

A permanent cell line, termed LMFS (liver metastasis from sarcoma) was established in vivo and in vitro from a spontaneously occurring murine retroperitoneal tumor of BALB/c mouse origin. After a subcutaneous injection of more than 1 x 10(5) cells in the dorsal side of mice, the LMFS cells proliferated at the inoculation site (100% take) and induced metastatic nodules spontaneously in the liver but not in the lung. The antitumor effects of 5'-DFUR against the LM FS tumor were examined. Mice with the LMFS tumor were given 5'-DFUR (0, 0.75, 1.5, 3.0 mM/kg) per os for 7 days from day 21 through day 27. Prolonging of the survival period was observed by the 5'-DFUR administration and the increase in the liver weight, which reflects tumor growth in the liver, was suppressed. Mice with the LMFS tumor were given 5'-DFUR (0, 0.75, 3.0 mM/kg) every day without Saturday and Sunday from day 21. Mean survival time of the control group was 57.8 +/- 4.9 days. And that of 5'-DFUR groups (0.75, 3.0 mM/kg) was 72.1 +/- 16.0 day (p < 0.05), and 80.5 +/- 9.5 days (p < 0.001), respectively. These results reveal that 5'-DFUR is effective for not only the primary site but also the liver metastasis.

[Recurrent endometrial cancer after complete surgery].

Between 1974 and 1991, 576 patients with endometrial cancer were treated with complete surgery. 1. The overall relapse rate was 14.4%. No recurrence was observed in surgical stage Ia, whereas 56.8% relapse was noted in stage IIIc. 2. The relapse rate in cases with adenocarcinoma was 13.0%. The relapse rate increased with poorer differentiation. Five of 6 cases with serous adenocarcinoma developed recurrence. 3. Myometrial invasion, size of tumor, vessel permeation, cervical involvement, adnexal metastasis and lymph node metastasis were significant factors for relapse. 4. Distant recurrence was noted in 63.9% of recurrent cases and 28.9% developed pelvic recurrence. The most common recurrent sites were lung, peritoneal cavity, and para-aortic lymph node in distant relapse and vaginal vault and parametrium in pelvic relapse. 5. The time to relapse ranged between 106 days and 5,518 days after complete surgery. Thirty one point three % of the recurrent cases manifested relapse within the first year, 60.2% within 2 years and 93.9% within 5 years. The mean time to relapse was 561 days. It was concluded that adjuvant therapy for cases with adnexal metastasis, lymph node metastasis, cervical involvement, adenocarcinoma (G3), and serous adenocarcinoma should be established urgently.

Inhibition of oxidized low-density lipoprotein metabolism in macrophage J774 by helvolic acid.

The antibiotic helvolic acid inhibited cholesteryl ester accumulation in macrophage J774 treated with oxidized low-density lipoprotein (LDL) at a concentration of 50-350 microM. The agent reduced oxidized 125I-LDL degradation and [14C]oleate incorporation into cholesteryl ester. In a cell-free assay, ATP-dependent acidification of endosomes and lysosomes was significantly inhibited by 35 microM helvolic acid, suggesting that this activity accounts for the inhibition of oxidized LDL metabolism in the macrophages.

Stimulation of acyl-CoA:cholesterol acyltransferase activity by brefeldin A in macrophage J774 cells.

Brefeldin A (BFA), an inhibitor of secretory pathway, enhances incorporation of radiolabeled cholesterol and oleate into cholesteryl esters in cultured cells [12]. We studied the mechanism for this effect of BFA in the macrophage J774. When incubated with 2.7 microM BFA in the absence of lipoproteins, J774 cells synthesized and accumulated 1.5- to 4-fold more cholesteryl esters than did cells which received no BFA. BFA caused neither an elevation of cholesterol synthesis, inhibition of its secretion nor changes in cholesterol transport to plasma membrane, esterification of plasma membrane cholesterol and cholesteryl ester hydrolysis. Acyl-CoA:cholesterol acyltransferase (ACAT) activity in microsomes from BFA-treated cells was 1.5- to 1.8-fold higher than that from control cells. The effect of BFA was diminished by treatment with low temperature, which is known to abolish BFA effect on Golgi formation.

[Sequential MR images of the uterus after Gd-DTPA injection--studies of normal volunteers and uterine endometrial malignant tumors].

To investigate the sequential changes in signal intensity (SI) of normal and abnormal uteri, T1 WIs were taken repeatedly after the injection of Gd-DTPA. Six volunteers and 19 patients with known uterine body malignancy (18 carcinomas, one carcinosarcoma) were examined. The results in volunteers were as follows. In the secretory phase, SI of the endometrium was stronger in the late images than in the early ones, whereas in the proliferative phase, SI was stronger in the early images. SI of the myometrium decreased rapidly and there were no differences in SI between menstrual phases. In 17 of 18 endometrial carcinomas, the tumors showed hypointensity relative to the myometrium, and the contrast between the tumor and the myometrium was better in the early images. In the remaining two cases, the tumor showed hyperintensity and the contrast was better in the late images. After the injection of Gd-DTPA, the endometrium appeared differently according to the menstrual cycle in normal volunteers, and the appearance of uterine structures and endometrial malignant tumors changed sequentially. These findings must be kept in mind when evaluating uterine diseases by Gd-DTPA enhanced MRI.

Inhibition of protein prenylation by patulin.

The antibiotic patulin was found to inhibit protein prenylation in mouse FM3A cells. Thus, the agent reduced incorporation of [3H]mevalonate into proteins by 50% at a concentration of 7 microM. In a cell-free assay, patulin inhibited rat brain farnesyl:protein transferase, one of the enzymes responsible for protein prenylation. The inhibition was 50% at a concentration of 290 microM.

Cloning of two isozymes of Trichoderma koningii glyceraldehyde-3-phosphate dehydrogenase with different sensitivity to koningic acid.

Koningic acid inhibits glyceraldehyde-3-phosphate dehydrogenase (GAPDH) by binding to the SH group in the active center. The fungus Trichoderma koningii, the producer of koningic acid, contains two GAPDH isozymes (GAPDHs I and II). GAPDH I is inhibited 50% by 1.1.10(-3) M koningic acid, while GAPDH II is inhibited 50% at 6.8 x 10(-6) M. cDNAs of the two isozymes were cloned from T. koningii and their nucleotide sequences were determined. The sequence of coding region and codon usage in both clones were compared with each other and with those of the gene for Aspergillus nidulans GAPDH (enzyme activity is inhibited 50% by 2.7 x 10(-7) M koningic acid). Results indicated that GAPDH II is more closely related to A. nidulans GAPDH than GAPDH I. All essential amino acid residues, except 174 and 181, which are implicated in catalysis and binding of NAD and substrates, were conserved among A. nidulans GAPDH and GAPDHs I and II. Residues 174 and 181 are threonine in both A. nidulans GAPDH and GAPDH II, but alanine and serine, respectively, in GAPDH I. The side-chain of alanine-174 in GAPDH I can not replace threonine-174 functionally as threonine-174 side-chain forms a hydrogen bond with the catalytically essential histidine-176.

Studies on ras oncogene activation in endometrial carcinoma.

The frequency of K-ras point mutation(PM) at codon 12 was studied in 45 patients with endometrial carcinoma. In vitro amplification of target sequences of DNA extracted from endometrial cancer tissues by polymerase chain reaction and dot blotting with oligonucleotide hybridization were performed. Ten of 45 endometrial carcinomas disclosed K-ras PM at codon 12 (22.2%). Transition from GGT to GAT was most frequent in PM(41.7%). Simultaneously, double PM (GAT/GCT) were also detected in 2 cases. No relationship appeared to be present between PM and clinical prognosis such as clinical stage, histological type, histological grade of differentiation, depth of myometrial invasion, and ascitic cytology. The positive rates of lymph node metastasis tended to be higher in the group with positive PM than in the group without PM. K-ras and C-myc gene amplifications were found in 2 (5.1%) and 3 (7.7%) of 39 cases, respectively. No PM of H-ras at codons 12 and 61 was detected. Our results showed that the PM of K-ras gene at codon 12 was a fairly common event in genetic abnormality and suggested it would have some role in the progression of carcinogenesis in endometrial carcinoma.

Activation of peritoneal macrophages in patients with gynecological malignancies by sizofiran and recombinant interferon-gamma.

We investigated the influence of the combined use of sizofiran, a beta-1,3-glucan and a recombinant interferon-gamma (rIFN-gamma) upon biological activities of peritoneal macrophages (M phi). The number of peritoneal M phi and the production of cytokines (interleukin-1 beta, interferon-gamma and tumor necrosis factor) was increased by the combined treatment. Fully activated peritoneal M phi based on the increased number of elongated pseudopods were observed by electromicroscope. Sizofiran seems to assure a sufficient supply of M phi to kill tumor cells in the peritoneal cavity and co-administered rIFN-gamma seems to directly stimulate the accumulated M phi in addition to its direct cytotoxicity against tumor cells. This combination therapy may be a step to the prevention of the recurrence of gynecological malignancies including ovarian cancer, after a negative second-look laparotomy.

Establishment and characterization of BALB/c retroperitoneal sarcoma with spontaneous liver metastases.

The objective of this study was to examine the identity and characteristics of a spontaneously occurring murine retroperitoneal tumor of BALB/c mouse origin that selectively metastasized to the liver. From the primary tumor, a permanent cell line, termed LMFS (liver metastasis from sarcoma) was established in vivo and in vitro. After a subcutaneous injection of more than 1 x 10(5) cells in the side back of mice, the LMFS cells proliferated at the inoculation site (100% take) and induced metastatic nodules spontaneously in the liver, but not in the lung. By the limiting dilution technique, a cloned cell line, LMFS-1, was established in vitro. The LMFS-1 cell line had similar morphological characteristics to the LMFS cells both in vitro and in vivo. The doubling time of the LMFS-1 cell line was 10 h in passage 60. The number of chromosomes ranged from 71 to 108 and 93% of metaphases showed near-tetraploidy. In microscopic examination, no specific arrangement of the LMFS tumor cells was seen; the LMFS cell had medium- to large-sized atypical nuclei and clear and large cytoplasm. Electronmicroscopy showed that the cytoplasm of the LMFS cell had a moderate amount of rough-surfaced endoplasmic reticulum but no desmosomes or microvilli. Immunohistochemically, the LMFS cells were positive for vimentin, but showed no reaction for keratin or cytokeratin. Therefore, the LMFS tumor was considered to be an undifferentiated sarcoma. The LMFS cell line should be a useful tool not only for studies of metastasis, but also for experiments on the therapy of hepatic tumors.
