Development of synchronous bilateral lung cancers after esophagectomy for esophageal cancer: report of a case.

It is well known that squamous cell carcinoma of the esophagus can be associated with carcinoma of other organs. We report herein the rare case of a 60-year-old man who developed synchronous bilateral lung cancers after undergoing esophagectomy for esophageal cancer. Staged bilateral lobectomy was successfully performed to minimize respiratory complications 3 years after his esophagectomy. This case report serves to demonstrate that aggressive and careful surgical approach with adequate followup offers the chance of long-term survival for patients with multiple primary cancers.

[Efficacy of the fluid attenuated inversion recovery (FLAIR) sequence of MRI as a preoperative diagnosis of hippocampal sclerosis].

A newly advanced MRI pulse sequence, the FLAIR (fluid attenuated inversion recovery) imaging, in which a long TE spin echo sequence is used with suppression of the CSF with an inversion pulse, displays the CSF space as a no-signal intensity area. There have been only a few reports on the FLAIR pulse sequence of temporal lobe epilepsy (TLE) as yet. We examined 9 cases of intractable TLE by FLAIR images and analyzed the advantages and disadvantages of the FLAIR pulse sequence for decision making on temporal lobectomy. All patients underwent anterior temporal lobectomy with hippocampectomy, and the diagnoses were confirmed histologically after surgery. Abnormally high T2 signals (HT2S) were more conspicuous with the FLAIR sequence than with any of the conventional sequences. Tilted axial plane, orientated along to the long axis of the hippocampal body, clearly demonstrated hippocampal atrophy (HA). Selection of a FLAIR sequence into the routine MR examination of patients with TLE is recommended.

Surgical techniques and the results of a fronto-temporo-parietal combined indirect bypass procedure for children with moyamoya disease: a comparison with the results of encephalo-duro-arterio-synangiosis alone.

We recently treated children with Moyamoya disease using a fronto-temporo-parietal combined indirect bypass procedure. Three different indirect bypass procedures (frontal EMAS, EDAS, EMS) were simultaneously carried out at three different sites. We thus treated 16 sides in 12 pediatric patients with Moyamoya disease using this method. Both the collateral formation and the improvement in the clinical symptoms were evaluated postoperatively. These results were then compared with those of the patients treated by EDAS alone. The postoperative collateral formation was more extensively seen in the patients treated with the combined bypass procedure than in those treated by EDAS alone. The improvement in ischemic symptoms was also better in the patients treated by the combined indirect bypass procedure. We therefore conclude that the combined indirect bypass procedure is more effective than EDAS alone.

Cerebral Angiography

Increased activity of the ipsilateral motor cortex during a hand motor task in patients with brain tumor and paresis.

PURPOSE: To look for changes in the motor cortex in patients with brain tumors. METHODS: Both cerebral hemispheres in seven patients with brain tumors were examined with functional MR imaging during a motor task performed by the hand opposite the site of tumor. The ratio of the activated area in the motor cortex ipsilateral/contralateral to the tested hand was calculated for each subject. Twenty healthy subjects were also examined in the same manner for comparison. RESULTS: The ratio of the ipsilateral/contralateral activated area was abnormally high in three patients with tumor-related paresis of the tested hand. The ratio was significantly greater in patients with paresis than in healthy subjects. CONCLUSION: This study demonstrated increased activity in the ipsilateral (unaffected) motor area during a hand motor task in patients with brain tumor and paresis, which was thought to reflect compensatory reorganization induced by the functional damage.

Western versus Asian types of multiple sclerosis: immunogenetically and clinically distinct disorders.

The polymorphism of HLA-DRB1, -DRB3, and -DRB5 genes as well as magnetic resonance images of the brain and spinal cord were studied in 57 Japanese patients with multiple sclerosis (MS). Twenty-three patients clinically displayed selective involvement of the optic nerve and spinal cord and were classified as having Asian-type MS. The other 34 patients had disseminated central nervous system involvement and were classified as having Western-type MS. Patients with Asian-type MS had fewer brain lesions shown by magnetic resonance imaging, but more gadolinium-enhanced spinal cord lesions than did patients with Western-type MS (47% vs 17%). Furthermore, the DR2-associated DRB1*1501 allele and DRB5*0101 allele were associated with Western-type MS (41.2%), but not with either Asian-type MS (0%) or healthy control subjects (14.2%). Heterogeneity in the immunogenetic background and in the magnetic resonance imaging features between the two subtypes of MS thus suggests the presence of two etiologically distinct diseases in Asians.

Contrast-enhanced MRI in spinal arteriovenous malformations and fistulae before and after embolisation therapy.

MRI was performed in six cases of spinal arteriovenous malformation (AVM) and arteriovenous fistula (AVF) before and after embolisation. Intramedullary and perimedullary AVMs showed marked vascular enhancement after embolisation. This was thought to reflect feeding vessel occlusion and correlated well with a favourable clinical outcome. In dural AVFs, contrast-enhanced studies were essential for the diagnosis, unenhanced images being nonspecific. After embolisation, enhancement of the spinal cord was reduced, although one case with a poor outcome showed persistent enhancement.

Moyamoya disease associated with persistent primitive trigeminal artery variant in identical twins.

Identical twin cases of moyamoya disease associated with persistent primitive trigeminal artery variant are presented. Both of the children suffered from the cerebrovascular occlusive disease called "moyamoya disease," but there existed a remarkable time lag in the manifestation of their first clinical symptoms. Coexistence of persistent primitive trigeminal artery variant and high occurrence of moyamoya disease in identical twins suggest some congenital factors in the development of this disease. However, the time lag of the first clinical manifestation between the twins suggests certain acquired factors may also play a role in the manifestation of this disease.

Brain Ischemia

Type D dural carotid-cavernous fistula. Results of combined treatment with irradiation and particulate embolization.

PURPOSE: To assess the efficacy of irradiation combined with particulate embolization for treating type D dural carotid-cavernous fistulas, supplied by the meningeal branches of both external (ECA) and internal carotid arteries. MATERIAL AND METHODS: Nine patients were treated by particulate embolization via the ECA branches, followed by radiation therapy with doses of 30 Gy. One of these patients with recurrent symptoms received an additional 30 Gy one year after initial treatment, making a total dose of 60 Gy. RESULTS: In all cases, symptoms were improved immediately after the particulate embolizations. In 8 cases, complete resolution of symptoms were obtained in 4-19 months and an average of 8 months after treatment. In one case, chemosis was markedly improved and persisted minimally 41 months later. CONCLUSION: Particulate embolization is effective for immediate symptomatic improvement, and complete resolution of symptoms can usually be achieved by subsequent radiation therapy.

Hypopituitarism associated with transient diabetes insipidus followed by an episode of painless thyroiditis in a young man.

A 16-year-old male complained of a headache and a high fever followed by polyuria. The endocrinological studies showed he had hypopituitarism and central diabetes insipidus, and magnetic resonance imaging (MRI) revealed a pituitary mass. Diabetes insipidus gradually improved and hydrocortisone treatment was begun at three months after onset, but a month later painless thyroiditis developed. An MRI demonstrated a spontaneous shrinkage of the pituitary mass nine months after onset. Lymphocytic hypophysitis followed by painless thyroiditis was the most probable diagnosis, although it is very uncommon especially among men.

[Pulmonary arterio-venous fistula treated by embolization with steel coils].

A 63-year-old man was referred to our hospital for evaluation and treatment of severe dyspnea on exertion which had persisted for a few years. He presented with cyanosis and markedly clubbed fingers, and laboratory data disclosed hypoxemia, polycythemia, and liver dysfunction. A chest X-ray film showed increased vascular markings in both lower lung fields. Arterial blood gas analysis showed severe hypoxemia, with a PaO2 of 46 Torr and a PaCO2 of 31 Torr while the patient was breathing room air. The PaO2 increased only slightly with inhalation of 100% oxygen, which suggested the presence of a large R-L shunt. The hepatopulmonary syndrome was diagnosed. Angiography of the pulmonary artery revealed a large pulmonary arterio-venous fistula with markedly dilated arteries in both lower lobes. Transarterial embolization was done three times with a total of 62 metal coils. There were no complications. Embolization reduced the shunt from 56% to 31%, increased the PaO2, and relieved the dyspnea. Pulmonary artery embolization can be useful in treating pulmonary arterio-venous fistulas associated with the hepatopulmonary syndrome.

[A case of orbital myositis with blepharoptosis demonstrating the swelling of superior levator palpebrae muscle on magnetic resonance images].

A 51-year-old woman developed diplopia and left blepharoptosis with an insidious onset. On orbital MRI, the extraocular and a left superior levator palpebrae muscles were swollen and enhanced by gadolinium, suggesting orbital myositis. The left blepharoptosis was improved two weeks and the swelling of superior levator palpebrae muscle one month after prednisolone administration. This is the first report that the lesion of superior levator palpebrae muscle was demonstrated by MRI in orbital myositis.

[A case of central homonymous hemianopsia due to cerebral infarction of the occipital tip].

A 54-year-old man was admitted to the hospital because of the restriction of the right visual field. Goldmann's visual field test revealed the right central hemianopsia. MRI showed the infarction of the left occipital lobe tip. Cerebral angiography showed the occlusion of the left calcarine artery but no abnormality in the branches of the middle cerebral artery (MCA). The occipital lobe tip receives the projection from the macular area and is supplied by both calcarine artery and a branch of MCA. Therefore, cases of central homonymous hemianopsia due to vascular disorders have been relatively rare and the macular vision is usually spared. In contrast to the above knowledge, only one artery occlusion resulted in the central hemianopsia in our case. Poor anastomosis between PCA and MCA in the occipital tip of our patient may explain occurrence of the infarction of that area.

Spontaneous resolution of hypertrophic cranial pachymeningitis.

A 36-year-old woman presented suffering from severe headache and diploia. Magnetic resonance imaging showed a diffuse thickening of the cranial dura mater which was enhanced by gadolinium-DTPA. Two months later, an improvement of the clinical symptoms and a spontaneous resolution of the dural thickening were noted. Hypertrophic cranial pachymeningitis and other diseases causing diffuse dural thickening are discussed.

Double cerebral venous angiomas: MRI.

Using a 1.5 Tesla MR imager, we examined 58 patients with venous angiomas (VA); images were also obtained following contrast medium in 33. Of the 58 patients 29 underwent selective cerebral angiography. We found in all 63 VA, including two with arterial components; 5 of the 58 patients thus each had two VA. The VA were supratentorial and bilateral in 4 cases; the remaining patient had supra- and infratentorial VA. Of these 5 patients, 4 received intravenous contrast medium and in 2 of them, the second VA was so small that it was detectable only on the contrast-enhanced images. The incidence of double VA in patients receiving contrast medium was 12% (4/33).
