Positive histological tuberculous findings despite stable sterility of the urine on culture: results of 111 nephrectomies and partial nephrectomies.

The therapy of genitourinary tuberculosis has changed: 'néphrectomie nécessaire' has been replaced by 'néphrectomie opportune'. Today, it is generally assumed that tuberculosis can be cured completely by chemotherapy. The following question must, however, be asked: How far may the stable sterility of the urine be considered as a complete cure? In order to assess the efficiency of chemotherapy as a function of time and dosage, the activity of tuberculosis of the kidneys was investigated on the basis of the histological preparation, after removal of the tuberculous renal tissue. Despite stable sterilization of the urine, 15% of the cases showed active tuberculosis. Using the triple therapy combination of rifampicin, ethambutol and isoniazid, the disease was found no longer to be active after 2--3 months of treatment in 90% of the cases. A longer period of treatment brought no further improvement.

[Treatment of the neurogenic detrusor-sphincter dyssynergy with lioresal].

A double-blind placebo trial was taken with Lioresal, and antispasticity drug acting on the spinal cord level. The drug produced improvement of voiding function in 13 paraplegics with upper motor neuron lesions and detrusor external sphincter dyssynergia. As a main result, the opening time was reduced.

[Urodynamic measurements in enuretics].

Distal urethral stenosis in girls as well as meatal and bulbar narrowed urethral segments in boys are still subjects of discussion about their pathologic value in the literature. The urodynamic findings in enuretic children by urethral pressure profile and cystometry frequently point to a outflow resistance. Calibration with bougies à boule, urethroscopy and permictional cysturethrography disclose such urethral narrowing. The survey of our own 107 enuritic children shows that urethrotomy has given good results relative to urodynamic data. The effect of methantheline medications consists in suppressing the hyperactivity of the detrusor.

[Stress incontinence in the woman (author's transl)].

The morphology and function of the urethrovesical junction are described to give some understanding of the pathogenesis and therapy of female stress incontinence. Conservative procedures can only be helpful in stage-1 stress incontinence, whereas in stage 2 surgical treatment provides excellent results.

Dependence of resting urethral pressure on age and sex of healthy subjects and on technical details.

The authors used the Harrison and Constable variation of the original method of Brown and Wickham. The maximal pressure level in the urethral pressure profile depends on technical details. Younger healthy individuals show a higher resistance than older ones. Prostate enlargement leads to an elongation of the posterior urethra. The physiological elasticity loss disposes older persons to stress incontinence.

[Present status and treatment of urogenital tuberculosis].

Urogenital tuberculosis has not decreased in incidence like other forms of tuberculosis and this is not to be expected in the coming years. Following combined modern chemotherapy the chances of conversion and also clinical cure are increased. The patient's cooperation during a systematic triple drug therapy in adequate dosage is important. The possibility of ambulatory treatment will depend on the clinical findings and the patient's social situation. Physical and occupational therapy and early resocialization of the patient are important. Prolonged unemployment should be avoided. Urological care of the patient is particularly important in the early phases of the treatment.
