Simultaneous determination of seven unconjugated steroids in maternal venous and umbilical arterial and venous serum in elective and emergency cesarean section at term.

In order to assess specific gluco- and mineralocorticoid functions in both mother and fetoplacental unit in relation to the presence or absence of labor, serum levels of unconjugated aldosterone (A), corticosterone (B), deoxycorticosterone (DOC), progesterone (P), 17-hydroxyprogesterone (17-OHP), cortisol (F), and cortisone (E) were determined simultaneously. These levels were determined by specific radioimmunoassays in two groups of 24 paired maternal venous and umbilical arterial and venous samples obtained at term delivery by either elective (Group I, N = 8) or emergency (Group II, N = 8) cesarean section. In Group II, after spontaneous labor, mean maternal serum levels of all steroids investigated exceeded those found in Group I (not in labor). These increases were most pronounced (p less than 0.005) in F (74%) and DOC (106%) levels demonstrating stimulation of both the glucocorticoid (cortisol)--and the mineralocorticoid (aldosterone)--producing pathways of the maternal adrenals by spontaneous labor. Arteriovenous differences in umbilical steroid levels revealed in both groups the placental origin of P, 17-OHP, and E (p less than 0.05 to 0.005), with greater (more negative) mean AV differences after labor (Group II). The negative AV difference of DOC, B, A, and F found in Group I, however, decreased after labor and became even positive in the cases of B and F, reflecting the close relationship between spontaneous labor and the fetal adrenal's active production not only of the glucocorticoids B and F, but also, to a lesser extent, of the mineralocorticoids DOC and aldosterone.

Androluteoma syndrome of pregnancy.

The clinical course of a 26-year-old primigravid woman with a virilizing luteoma of pregnancy is described. It was demonstrated by endocrine studies that the tumor secreted an excessive amount of testosterone. After removal of the luteoma in week 32 of pregnancy, the virilizing symptoms of the mother completely regressed. A normal male infant was delivered in week 39. From 100 luteomas of pregnancy described in the literature, 26 cases have been cited to elicit endocrine virilizing activity. An evaluative synopsis indicates that including the case described in this paper only 13 can be regarded as a clinical, morphologic, and functional entity. This entity should be differentiated from the inhomogeneous group of luteomas as the "androluteoma syndrome of pregnancy." It is characterized by the following major criteria: virilization, beginning with the second trimester of pregnancy; histologic criteria of a luteoma of pregnancy; production of testosterone, leading to excessive plasma levels (exceeding the concentration of androstenedione), and masculinization of female fetuses.

[Pathomechanisms, signs, symptoms and management of amniotic fluid embolism (author's transl)].

The rare, acute and dramatic event of amniotic fluid embolism is increasing in relative importance as a cause of maternal mortality. Recent findings based on the estimation and characterization of circulating fibrin, fibrinogen content and fibrin-fibrinogen degradation products indicate that the coagulation disorder is caused by intravascular coagulation and extensive fibrinolysis and fibrinogenolysis. The program of treatment consists of: 1. Management of the acute respiratory insufficiency syndrome by early intubation and ventilation with positive end-expiratory pressure (PEEP). 2. Intensive therapy of shock and rapid restoration of blood volume with blood transfusion (fresh whole blood) and infusion of other fluids. 3. Treatment of the coagulation disorder with fibrinogen, antifibrinolytic agents (aprotinin--Trasylol), platelet transfusion and fresh frozen plasma.

[Monitoring of the results of the treatment of chorionic carcinoma (author's transl)].

Because of the high sensitivity and specificity of the method, radio-immunologic determination of the Beta Subunit HCG in the serum is superior to other methods of monitoring the therapeutic results of the treatment of chorionic carcinoma. A ratio of 225 to 1 of HCG concentrations in the serum versus the cerebral spinal fluid is evidence against the probability of a metastasis of chorionic carcinoma in the central nervous system. Following successful treatment of chorionic carcinoma it is advisable to determine the serum HCG with the Beta Subunit method weekly during the first two months and montly in the succeeding two years and thereafter at three months intervals.

[Thrombotic thrombocytopenic purpura (Moschcowitz-syndrome) in pregnancy (author's transl)].

The case of a primigravida with thrombotic thrombocytopenic purpura (Moschcowitz Syndrome) is reported. The patient delivered a healthy male infant at 35 weeks gestation during a remission. The patient died at 10 days postpartum from a relapse of the disease. The special points in the differential diagnosis and treatment of thrombotic thrombocytopenic purpura during pregnancy are discussed.

[Perinatal mortality in foreign workers (author's transl)].

From 1970 to 1972, there were 216 perinatal deaths among 5595 newborns at the I. Frauenklinik der Universität München. 54 of these deaths were children of foreign workers (so-called "Gastarbeiter"). The data have been processed on punch cards and analysed by a computer. The differences noted underwent significance testing by the CHI-Quadrat test. Only statistical significant results are published. The perinatal mortality in the above period shows no difference between foreign and German ward patients. There is, however, a significant lower perinatal mortality in private patients. We feel that this difference is due to a significant lower rate of prematures in the private patient group. The cocial status as well as higher interest and motivation in health resulting in better prenatal care are discussed as causal reasons for this fact.
