[Antigenic determinants on ovalbumin and ovomucoid: comparison of the specificity of IgG and IgE antibodies].

To delineate the antigenic determinants on ovalbumin (OA) and ovomucoid (OVM) recognized by specific IgG and IgE antibodies in sera from patients with allergy to hen's egg, we studied the binding activities of IgG and IgE antibodies to native OA or OVM and seven different OA or OVM preparations, i.e. heated OA or OVM (100 degrees C for 3 min and 80 degrees C for 30 min) or 0.3 M NaOH, dithiothreitol (DTT), sodium dodecyl sulfate (SDS), 6 M urea or HCl treated OA or OVM. Eight patients with IgE anti-OA antibodies and 12 patients with IgE anti-OVM antibodies were used in these studies. The binding activities of IgG and IgE antibodies to physically or chemically denatured OA were partially decreased compared with those to native OA, whereas IgG and IgE antibodies in sera from patients bound well to denatured OVM with similar binding activities to native OVM. These results strongly suggest that antibodies to OA recognize partially conformational antigenic determinants on OA, whereas antibodies to OVM mainly recognize sequential antigenic determinants on OVM, and that antigenic determinants of OVM recognized by antibodies in sera from patients are more stable than those of OA under these denaturation conditions. In addition, the binding activities of IgE antibodies to denatured OA or OVM were significantly different from those of IgG antibodies to these OA or OVM, suggesting that the specificities of IgE antibodies to OA or OVM may be different from those of IgG antibodies to OA or OVM.

Pulmonary alveolar proteinosis as a component of massive fibrosis in cases of chronic pneumoconiosis. An autopsy study of 79 cases.

Histopathological analysis of massive fibrosis (MF) in lungs obtained from 79 patients with chronic pneumoconiosis revealed evidence of pulmonary alveolar proteinosis (AP) in a surprisingly high proportion of the cases (73%), suggesting that AP may be a usual constituent of MF in chronic pneumoconiosis. AP is not a specific morphological counterpart responsible for acute silicosis. In order to explain the occurrence of AP in MF, several hypothetical mechanisms are proposed and discussed.

[A case report of Buschke-Lowenstein tumor (giant condyloma)].

A 28-year-old woman with diabetes mellitus and alcoholic hepatitis presented a rare case of Buschke Lowenstein tumor, or giant condyloma of vulva. HPV DNA of this tumor was detected by in situ hybridization using tritium labeled HPV 6 b, 16 and 18 DNA. This tumor appeared to harbour HPV 6 b DNA, but other HPV DNAs were negative. The distribution of HPV 6 b DNA was detected in the nucleus of the squamous epithelium showing koilocytosis. Total vulvectomy and resection of the clitoris were done. The postoperative course was uneventful and there has been no recurrence of tumor so far.

Adreno-hepatic fusion. An autopsy study.

Adreno-hepatic fusion (AHF) is defined as adhesion of the liver and right adrenal cortex with close intermingling of the respective parenchymal cells. Contrary to the general opinion, AHF is a rather common event of unknown biological significance: AHF was exhibited by 63 of 636 unselected autopsy cases (9.9%). Its incidence is much higher in older age groups, suggesting that AHF may be an ageing phenomenon. No causative relationship was found to exist between occurrence of AHF and pathological conditions of both organs.

Use of body surface electrocardiographic mapping to localize the asynergic site in previous myocardial infarction.

Body surface electrocardiographic (ECG) maps of myocardial infarction were analyzed using the departure mapping technique, which represents the abnormal potential distribution out of normal ranges. Body surface ECG mapping using 87 leads was performed on 65 patients with previous myocardial infarction and on 40 normal volunteers. Potential departure maps at 10, 20, 30, 40, and 50 msec after the onset of QRS were constructed; each map indicated, if present, the area of abnormal decreased potential that is more than 2 standard deviations from the normal range (-2 SD area). In patients with myocardial infarction, the appearance time and the location of the -2 SD area were specific for the sites of left ventricular asynergy; the sensitivity and specificity were 86% and 100% for the asynergy of segment 2 (20 msec, on the upper left anterior chest), 87% and 97% for segment 3 (30 msec, on the middle anterior chest), 86% and 80% for segment 4 (20 or 30 msec, on the lower right anterior chest), and 88% and 90% for segment 5 (30, 40, or 50 msec, on the middle back), respectively. The sensitivity of these criteria was better than that of 12-lead ECG, while the specificity was comparable. In the analysis of body surface ECG mapping data, departure maps aid in depicting abnormalities and in making an accurate assessment. Body surface ECG mapping can be used to improve the diagnostic ability of ECG to detect myocardial infarction.

Three-dimensional analysis of facial morphology before and after orthognathic surgery.

A method for three-dimensional analysis of the facial hard- and soft-tissue morphologies is described. The soft-tissue analysis consisted of calculating three-dimensional values of reference points on the face by perspective transformation of their values in two pairs of photographs, taken simultaneously, from the right and left sides of the face. The shape of the mandible was analyzed three-dimensionally by the simultaneously taken frontal and lateral cephalograms. The hard- and soft-tissue changes were analyzed with the method in 28 patients in whom mandibular prognathism had been corrected by orthognathic surgery. The magnitude of the surgically-produced soft tissue volumetric changes in the anterior mandibular region was proportional to the posterior movement of the mandible. Asymmetry of the face also improved in response to correction of lateral deviation of the mandible and a close correlation between the directional indices of asymmetry of the hard and soft tissues was observed. Thus, the method was found to be quite useful for the analysis of facial morphology in jaw deformity.

Significance of discordant ST alternans in ventricular fibrillation.

With the use of epicardial mapping, we investigated the electrical alternans of the ST segment during acute myocardial ischemia and studied the difference in ST alternans between dogs with resultant ventricular fibrillation and those without it. During the 7-minute occlusion of the left anterior descending coronary artery below its first diagonal branch, 60 epicardial unipolar electrograms were recorded simultaneously at 1-minute intervals by a computerized mapping system. ST alternans was found in the eight dogs we observed. The amplitude of ST alternans (difference in the ST segment elevation of two consecutive electrograms) was greater in dogs with ventricular fibrillation (n = 4) than in those without it (n = 4) (3.92 +/- 1.24 versus 0.58 +/- 0.49 mV, p less than 0.05). Three of the four dogs with ventricular fibrillation demonstrated discordant ST alternans (i.e., adjacent leads were out of phase). Results from the present study indicate that an increased amplitude and discordance of ST alternans during acute myocardial ischemia are related to ventricular fibrillation and act as indicators of time and spatial unevenness of ventricular repolarization.

Drug-susceptibility and isolation of a plasmid in Haemophilus (Actinobacillus) pleuropneumoniae.

We isolated 56 Haemophilus (Actinobacillus) pleuropneumoniae strains from the pneumonic porcine lung tissues and tested them for antimicrobial susceptibility. Two drug-resistant strains were obtained. One, named KH-265, was resistant to streptomycin (SM) and sulfonamide (SA), and the other, named KH-195, was resistant to tetracycline (TC). The minimum inhibitory concentrations (MICs) of drugs for resistant strains were 100 micrograms/mliters for SM, 3200 micrograms/mliters for SA, and 12.5 micrograms/mliters for TC. KH-265 possessed a 8.3Kb nonconjugative plasmid, pMS260, encoding SM and SA resistance, which was transformable to E. coli strains. pMS260 belonged to none of 14 incompatibility groups including Inc. P and Inc. Q, so far tested. It was mobilizable to various causative strains for respiratory infections, Pseudomonas aeruginosa, Bordetella bronchiseptica, Pasteurella multocida and Haemophilus pleuropneumoniae, by RP4 (Inc. P) plasmid.

Potassium iodide inhibits neutrophil chemotaxis.

We studied the effect of potassium iodide on the chemotaxis of neutrophil in 15 healthy subjects with a modified Boyden chamber method. Orally administered potassium iodide (15 mg/kg/day for 3 days) significantly inhibited the neutrophil chemotaxis in peripheral blood. It is postulated that the therapeutic effect of potassium iodide on erythema nodosum, nodular vasculitis, and Sweet's syndrome might be mediated through the inhibition of neutrophil chemotaxis by this agent.

[A study of linearity and reciprocity during shock applied with a hammer to human dry skull].

The authors used a human dry skull on which the cranial bone mandible had been joined with an artificial articulator disk to form a single unit. Impact acceleration corresponding to weak and strong tapping was considered a dynamic load in examining the vibration transfer characteristics of the facial cranial bone when impact was applied from the mentum section in a situation designed to be closer to reality. Flexion injection type (resonance frequency f0 = 100 to 150 Hz, produced by GC Corp.) was applied to the human dry skull as an artificial periodontal membrane at thickness of 0.3 mm. In addition, Exaflex heavy body type (f0 = 400 Hz, produced by GC Corp.) was applied as an artificial disk. This was then placed on a damper produced by spreading a rubber dam sheet with a thickness of 35 microns on a tire tube with a diameter of 35 cm and an air pressure of 35 kg/cm2. Investigations were then made concerning linearity and reciprocity to determine whether an experimental system could be achieved or not. This was then followed by modal analysis. As a result, the following matters were ascertained: (1) The resonating area differed according to the extent of the force. (2) An increase in the viscoelastic elements of the silicon was accompanied by attenuation of force. (3) Directionality of force attenuation was caused by the complexity of bone structure. (4) A tapping force of 0.3G or 1G was sufficiently attenuated by the facial cranial bone. (5) The transfer function at the bone seams and thinner areas of the bones was insufficient for modal analysis of the facial region and total cranial bone of the human dry skull.

[A case of Hurst's encephalitis complicated by hepatoma].

The patient, a 29-year-old female, was hospitalized because of clouding of consciousness, fever and right hemiplegia 4 days after the onset. On first examination she was found having fever in the 37 degree range, positive CRP, hepatomegaly, anemia and hepatic function impairment. Neurological examination revealed somnolence, conjugate deviation to the left and stiff neck. The muscular power measured about 3+ for the upper extremities and 0 for the lower extremities. Babinski sign was present on the right side. The spinal fluid showed an increase in cell counts, especially the neutrophil count. CT scans showed diffuse white-matter hypodensity in the left hemisphere. Soon after admission the patient fell into coma and died 6 days after admission. Autopsies led to a diagnosis of Hurst's encephalitis complicated by hepatoma with liver cirrhosis. It was reported that the immune complex was found in 25.9% of liver cirrhosis patients. From this fact it is suggested that Hurst's encephalitis might be elicited by some immunological mechanism.

Methamphetamine induced locomotor rhythm entrains to restricted daily feeding in SCN lesioned rats.

Rats were lesioned in the SCN and treated with methamphetamine dissolved in drinking water. A robust rhythm appeared in spontaneous locomotor activity which was not affected by blinding. Periodic food restriction (RF) of a 24 hr period was imposed on SCN lesioned rats with free-access to food for 4 or 6 hr per day, while water was given ad lib. The locomotor rhythm induced by methamphetamine treatment was phase-set by RF immediately in most cases but with transients in some. The phase-angle difference (psi) between food presentation and the activity onset became more negative by increasing the dose of methamphetamine. Because there was a positive correlation between methamphetamine dose and the period of locomotor rhythm, the change in psi was most likely due to lengthening of the period. After the termination of the RF schedule, the locomotor rhythm started to free-run from the prior phase set by RF. These results indicate that the methamphetamine dependent locomotor rhythm entrains to RF.

Blastic transformation of essential thrombocythemia. A case report.

A case of blastic transformation of essential thrombocythemia (ET) is reported. A 69-year-old male was first admitted to hospital because of fever in February, 1982. He was diagnosed as having ET because of marked thrombocytosis (205.5 x 10(10)/1), absence of erythrocytosis, absence of splenomegaly, normal karyotype and no increment of blasts in the bone marrow, and normal levels of neutrophil alkaline phosphatase, vitamin B12 and folate. He was treated with busulfan, and subsequently his platelet count was well controlled for about five years. At the second admission, blasts were present in the peripheral blood, and later accounted for 49% of the total leukocyte count. Histological examination of a bone biopsy specimen showed homogeneous proliferation of blastic cells and slight reticulin fibrosis. At autopsy, the degree of bone marrow fibrosis had increased. This was considered to be a very rare case of ET with blastic transformation in the terminal phase.
