Upregulation of a lymphoid serine protease in kidney allograft recipients.

The presence of a putative, cytotoxicity-linked lymphoid serine esterase (SE) has been studied in 79 kidney graft recipients. Peripheral blood lymphocytes (PBL) bearing an N-alpha-benzyloxy carbonyl-L-lysine thiobenzyl ester (BLT)-specific SE were evaluated by a novel cytochemical staining method. A characteristic of post-allograft patients was an increased presence of SE containing granules in PBL. In 46 patients with stable graft function SE + PBL were 33.41 +/- 10.34% (controls: 26.30 +/- 5.22%, P less than 0.0025), SE + CD4+ 4.32 +/- 3.85% (controls 2.13 +/- 1.52%, P less than 0.0025) and SE + CD8+ T cells 47.68 +/- 18.64% (controls: 28.50 +/- 6.50%, P less than 0.0005). In those graft recipients undergoing a rejection episode a marked upregulation of SE activity could be observed when compared to the stable graft group: SE + PBL were 59.91 +/- 10.89% (P less than 0.0005), SE + CD8+ 74.30 +/- 10.79% (P less than 0.0005) and SE + CD4+ T cells 28.56 +/- 13.50% (P less than 0.0005). In 10 cases this increase of SE activity was observed with a time lag of up to 37 days prior to the onset of clinical or biopsy proven rejections, promptly decreasing in response to methylprednisolone antirejection therapy. In patients with recurrent rejection episodes and subsequent graft loss, a repeating increase of SE activity indicated a failure of therapeutic agents.(ABSTRACT TRUNCATED AT 250 WORDS)

[Amanita phalloides poisoning in Austria (author's transl)].

An analysis of 28 cases of amanita phalloides poisoning serves as basis for a discussion of the clinical features and therapeutic problems involved. A critical review of recent experimental investigations in animals points to new possibilities in the treatment of amanita phalloides poisoning.

[Hemolysis after combined azathioprine and allopurinol therapy].

In a 37 year old renal transplant woman haemolytic anaemia was observed in the course of a combined azathioprine-allopurinol therapy. Azathioprine is converted to 6-mercaptopurine, which then catalyzed by xanthine oxidase is oxidized to 6-thiouric acid. When allopurinol and azathioprine are administered simultaneously, high 6-mercaptopurine concentrations in plasma may be expected because of inhibition of xanthine oxidase activity by allopurinol. We assume that elevated plasma 6-mercaptopurine concentration could possibly be responsible for haemolytic anaemia in this renal transplant patient.

[Forced diuresis].

Forced diuresis (FD) is a frequently used method for eliminating toxins. Its therapeutic effect has yet to be evaluated by a controlled clinical trial. In the absence of such a trial its usefulness can be judged only indirectly from urinary excretion rates. In former times the usual clinical laboratory procedures could not differentiate between the unchanged toxin and its metabolites. Using more selective methods, the discussions of the effect of FD in eliminating various drugs were renewed. The problem for the indication of FD is not only a missing knowledge about the kind and amount of the ingested drug, but also when knowing it, the missing evaluation of the effect of FD on its excretion. In such cases the pharmacokinetic behaviour of the drug can be helpful. If paying enough attention to the contraindications, to the principles of electrolyte and water balance, the complication rate is low. Many infusion regimes are proposed for this treatment, but only a standardized procedure can increase the safety and efficiency of this method. More complicated and more expensive methods should be used, when there is an intoxication with a substance of high mortality or when there is no effect of FD on eliminating the toxin.

[Retinal detachment caused by boxing].

Five cases (six eyes) of retinal detachment due to fisticuffs are recorded; at least four eyes went blind. Similar observations have been reported by other authors. In boxing, traumatic lesions of the head are inevitable since punches upon the head are crucial in this sport. Because of the risk of severe injuries including blindness and even death, at least brutal boxing must be rejected.

[Treatment of cystoid macular oedema according to Bangerter (author's transl)].

The biomicroscopic image of cystoid macular oedema was fully described by A. Bangerter in 1945. By means of fluorescein angiography additional cases may be revealed, but such cases are transitory and asymptomatic. Cystoid macular oedema seems to be caused by vascular disorders of inflammatory-toxic aetiology, the vitreous body being involved as a carrier of toxins. For treatment of cystoid macular oedema retrobulbar injections of vasodilators have been recommended by Bangerter. This therapy is definitely effective in cases of Irvine-syndrome, and in cases of cystoid macular oedema due to chronic iridocylitis, provided that application is starded early enough. Bangerter's treatment fails in other types of cystoid macular oedema. Corticosteroids are useful adjuvants, but they don't fully substitute Bangerter's treatment.

[Considerations on the rationalisation of an eye clinic (author's transl)].

The actual economical problems of hospital management demand critical considerations on the possibilities of rationalisation. Regarding the activities on an university eye-clinic in a big city the realisation of certain facilities could be well obtained: a decrease of the number of in-patients by treating of suitable cases as out-patients; shortening of the time of hospitalisation according to the progress of modern surgical and medical treatment; arrangment of departments for treating patients during the day in cases requiring intensive treatment and follow-up after full hospitalisation.

The pharmacokinetics of digoxin in patients with manifest hyperthyroidism and after normalization of thyroid function.

The pharmakokinetic data (elimination half-life, apparent distribution volume, total and renal clearance and cumulative urine excretion) were determined after intravenous administration of 1 mg digoxin in 9 female patients with an average age of 52 +/- 15 years with manifest hyperthyroidism. The study protocol was repeated after normalization of thyroid function by means of conventional thyrostatic therapy. Digoxin was determined by radioimmunoassay.

[Forty years' contemporary retinal detachment surgery (author's transl)].

A survey is given on the evolution of retinal detachment surgery during the past four decades. Contributions of the ophthalmological school of Vienna are particularly considered. The review includes simple diathermy-coagulation with subretinal drainage, the undermining operation of Lindner and his eye ball shortening procedure, the combined operation (sealing of retinal breaks by diathermy plus scleral resection), and vitreous tamponade using sodium hyaluronate. All these methodes have been widely replaced during the past years by cryopexy with plombage, and by encircling procedures. Though the rate of success could not be definitely increased, the introduction of the modern methods implies technical facilities for the eye surgeon, and advances for the treated patients. The evolution of retinal detachment surgery is not yet terminated, and further progress can be taken as granted. Retinal detachment surgery with very short hospitalization may become practicable in near future.

[Prospects and limitations of the treatment of senile maculopathies with phosphatides (author's transl)].

All kinds of treatment attempted so far in cases of senile maculopathies were unsatisfactory. Intramuscular injections of retinal phosphatides were introduced six years ago. In this follow-up study 216 eyes of patients with senile maculopathy treated by Etaretin have been checked. The former experience was confirmed that an improvement in visual acuity can be obtained only in early cases of the dry type. In such cases i.m. injections of Etaretin are actually the treatment of choice. In fairly advanced cases eventually some stabilization of the process may be obtained. In terminal stages no effect can be expected. Experimental observations revealed that the therapeutic effect of a phosphatide compound depends on the content of the polyunsaturated fatty acid C 22:6, but not on the animal organ which serves as the source of the extract. From an economic point of view these findings are of importance for further experimental work, as well as for broad clinical application.

Age Factors

[Acute poisoning with tricylic antidepressants and treatment with physostigmine salicylate (author's transl)].

Three cases of self poisoning with tricyclic antidepressants (TAD) are reported, one of them with a potentially lethal dose. All three cases were treated with physostigmine salicylate (PS) and in two cases there was complete reversal of coma within a few minutes. In striking contrast to the reported high incidence of cardiac arrhythmias no cardiac complications were observed in any of the cases. Therefore we think that the use of PS should be considered when treating cases of TAD poisoning.
