[Potential of noninvasive determination of the left atrioventricular aperture in mitral stenosis (according to data of ultrasound studies)].

The results of the measurement of the mitral opening in mitral stenosis patients by planimetry and by the computation method via the half-cycle of the atrioventricular pressure were found to be in satisfactory agreement except those in a group of patients with concomitant failure of the semilunar aortic valve. Moderate or appreciable mitral regurgitation and stenosis of the aortic ostium associated with mitral stenosis exerted no substantial effect on the accuracy of the design parameter of the mitral opening.

[Late diastolic mitral regurgitation].

Late diastolic mitral regurgitation (diastolic backflow of blood from the left ventricle to the left atrium at the end of left-ventricular diastole) is registered by Doppler echocardiography in patients with acquired heart defects and dilated cardiomyopathy. This phenomenon is based on the appearance at the end of left-ventricular diastole of the left ventricle/left atrium pressure gradient which is opposite to a normal relationship. Late diastolic regurgitation is more often encountered in patients with incompetence of the mitral and aortic valves. It is caused by disturbances of the work of the left heart and of kinetics of the mitral valve in relatively low frequency of cardiac contractions and is encountered in sinus rhythm and atrial fibrillation, and does not depend on the value of the transmural diastolic gradient. The clinical significance of mitral regurgitation remains unclear.

[Total and segmental left-ventricular myocardial function in patients with post-infarction heart aneurysm].

A study of total and segmental myocardial hemodynamics and contractility has demonstrated a close relationship between segmental disorders and the type and site of postinfarction left-ventricular aneurysms. A direct close correlation was established between total and segmental hemodynamic disorders, excepting the left-ventricular aneurysm area. A close correlation was also demonstrated between myocardial contractility and ejection fraction parameters in three left-ventricular segments. Standardized trisegmental hemodynamic and contractility analysis is shown to reflect left-ventricular myocardial condition in patients with postinfarction cardiosclerosis and aneurysms of the heart.

[Differential echocardiographic diagnosis of a giant left atrium and pericardial effusion].

With a sharp increase in the sizes of the left atrium its wall in the form of a fold can be seen behind the left ventricle. An echo-free space is recorded in such cases on echocardiograms. It can be interpreted as pericardial exudate. Proceeding from an analysis of the results of ultrasonic investigation of 5 patients the authors have proposed some criteria for differential diagnosis between the gigantic left atrium and pericardial effusion.

[Features of the course and evaluation of the efficacy of surgical treatment of adhesive pericarditis with constriction].

Two groups of patients with constrictive pericarditis were identified on the basis of clinical and instrumental examination including echocardiography and computerized tomography of the heart: patients with complete obliteration of the pericardial sac and constriction, and those with exudative/adhesive pericarditis and constriction. Part of the patients were subjected to subtotal pericardectomy. An improvement of left-ventricular diastolic function and hemodynamic parameters was demonstrated 1 to 1.5 months after the operation. Postoperative echocardiography showed persistent disorders of interventricular septum movement and multilayer abnormal echoes in the left-ventricular posterior wall area.

[Possibilities of echokymography in the evaluation of the left ventricular myocardial contractile function in patients with ischemic heart disease].

The authors compared the results of study of myocardial contractility using echokymography with those of one- and two-dimensional echocardiography in 42 CHD patients. A sufficiently high efficacy of the method for the assessment of regional myocardial contractility in patients with akinesia zones and without them was shown. A correlation analysis showed a close association of the wave depth of echokymography with the excursion of the interventricular septum and the posterior wall of the left ventricle. There was no significant association between the wave depth of echokymography and indices of the left ventricular total contractility.

[Prolapse of the aortic valve].

Prolapse of the aortic valve (PAV) was diagnosed in 20 patients using a method of two-dimensional echocardiography. PAV primary and secondary forms were distinguished. Congenital pathology of the other cardiac valves (prolapse of the atrioventricular valves and the bicuspid aortic valve) or the aorta was observed in primary PAV. Secondary PAV was observed as a concomitant pathology in dilatation of the aortic root resulting from atherosclerosis or in an infectious process on the aortic cusps in subacute septic endocarditis. Of non-invasive diagnostic methods the most effective one was two-dimensional echocardiography which could be regarded as a verifying method in PAV.

[Importance of ultrasonic studies in diagnosing and assessing heart function in aortic coarctation in adults].

Aortic coarctation was diagnosed with the aid of sectorial scanning from the suprasternal position in 6 of 7 patients. Ultrasonic investigation allows one to assess myocardial status and detect a prolapse of the atrioventricular valves, aortal valve protrusion, the presence of a bicuspid aortal valve and other abnormalities accompanying aortic coarctation in adult patients.

[Reproducibility of the flow-volume indices of healthy persons and patients with lung diseases].

Reproducibility of the flow-volume indicators at forced expiration and inspiration was calculated during examination of 210 healthy subjects and 420 patients with different pulmonary diseases. Reproducibility of the flow-volume indicators of the median and end part of the forced expiration which reflect largely the function proper of the patency of the distal parts of the bronchial tree was essentially higher in patients and tended toward increase with a rise in the degree of ventilation abnormalities. Reproducibility of the conventional spirographic parameters in healthy subjects and patients with ERF abnormalities did not differ whatever their character and degree.
