Poisoning with antidepressive drugs: a five-year retrospective study.

From 1976 to 1980, there were 3,193 admissions due to acute drug overdosage at the Resuscitation Center of the Klinikum Charlottenburg of Berlin, Free University. We determined the frequency and characteristics of self-poisoning with antidepressants and some low potency neuroleptic drugs (perazine and thioridazine). These drugs were involved in 92 cases (i.e., 3%) during a 5-year period. Amitriptyline - in combination with a benzodiazepine - was the most common antidepressant taken by the patients. 10 of the patients required assisted ventilation. Complete ECG recordings were carried out in 24 patients; 21 of them had abnormalities comprising prolonged QTC and PR intervals (19, 15 and 8 patients, respectively). Sinus tachycardia was present in half of those patients. In no cases were convulsions or cardiac arrhythmias requiring special treatment described in the medical records. The percentage of patients showing ECG changes and respiratory depression was higher when other drugs such as ethanol were ingested along with antidepressants than when only antidepressants were taken. The incidence of antidepressant self-poisoning in this area was relatively low compared to the results of other studies. Possible explanations for its low frequency could be a low rate of prescription of antidepressants, a low dosage prescribed or the success of antidepressants in the treatment of depression and thereby in the prevention of attempted suicide.

Acute poisoning with pine oil - metabolism of monoterpenes.

A patient attempting suicide ingested 400-500 ml pine oil and was admitted to the clinic. Since more than the lethal dose had been ingested hemoperfusions with activated charcoal and amberlite and a hemodialysis were performed. The composition of the ingested pine oil was determined by gaschromatography/mass spectrometry. Four monoterpenes were identified: 57% alpha-pinene, 8% beta-pinene, 26% carene, 6% limonene and 3% other hydrocarbons. The blood and urine monoterpene concentrations were continuously monitored. The data suggest that monoterpenes are poorly resorbed in the gastrointestinal tract. The resorbed portion of the hydrocarbons cumulates in the lipophilic body compartments and is slowly metabolized and then excreted by the kidneys. The main metabolic pathways are hydratation, hydroxylation, rearrangement, and acetylation. Five metabolites were identified.

Sectioning and imaging hard mineral fibres in biological tissues.

A technique is described which overcomes the problems associated with sectioning biological tissue containing hard mineral fibres. 0.2--0.5 micrometer thick sections were cut with a diamond knife, placed in a folding grid, conventionally stained with uranyl acetate and lead citrate and viewed at an accelerating voltage of 200 kV in the scanning transmission mode.

Mixed crystal deposition in an osteoarthritic joint.

The case of a 72-yr-old woman with osteoarthritis of the shoulder, is described. A mixture of crystals of hydroxyapatite and calcium pyrophosphate dihydrate was identified in the joint tissues using analytical electron microscopy and infra-red spectroscopy.
