Distraction osteogenesis. A comparison of corticotomy techniques.

The left hindlimbs of 15 adult mongrel dogs were lengthened using the Ilizarov external fixator. Five dogs were assigned to each of three separate groups: (I) corticotomy; (II) osteotomy with multiple drill holes and an osteotome; (III) osteotomy with an oscillating saw. Distraction began on postoperative day seven and continued at a rate of 0.25 mm every six hours for 26 days. The regenerate segments were evaluated with weekly radiographs and Dual Energy Photon X-ray (DEPX) beginning at day seven. Barium perfusion studies and histologic preparations were also performed after the dogs were killed. Periosteal callus formation was apparent in all groups at two weeks, and endosteal callus appeared in Groups I and II at three to four weeks. Perfusion studies at the end of the distraction period demonstrated vessels bridging across the regenerate gap in Groups I and II. Vessels bridging the gap were diminished in the oscillating-saw Group III specimens. During the distraction period, the appearance and length of the regenerate bone was similar for each group radiographically and by DEPX scanning. No histologic differences were seen in Groups I and II. Three of the four tibias cut with the oscillating saw did not consolidate at ten weeks. A simple transverse osteotomy yields regenerate bone that is indistinguishable from the technically more difficult corticotomy. Use of the oscillating saw may lead to delayed consolidation.

Evidence for the indirect utilization of glucose for the synthesis of hepatic glycogen in man.

1. This study was designed to test the hypothesis that three-carbon intermediates can be used in the 'indirect' pathway of glycogen synthesis in human liver (i.e. a route additional to the use of glucose by the 'direct' pathway). 2. After an overnight fast, 13 patients were given an infusion of 20% (w/v) glucose before elective abdominal operation. All received a 2.5 g bolus of 2220 kBq of selectively 3H- and 14C-labelled glucose before removal of a 1 g biopsy of liver. 3H and 14C were determined in purified glycogen as well as in glucose and lactate from samples of peripheral blood. 3. The ratio and specific activities of 3H and 14C in glycogen were found to be significantly lower than those in administered glucose. By calculation, 7-74% of glycogen repletion occurred by indirect pathways and not all of this was from the glucose supplied. 4. This study suggests that the operation of a direct pathway in man is not exclusive and that significant repletion of hepatic glycogen occurs by an indirect route.

Plasma vitamin A and carotene in maternal and cord blood.

Mean plasma vitamin A and carotene levels in 200 pregnant women at term were 30.3 mcg% and 43.6 mcg%. Sixty-four percent of women had vitamin A in lower limit of normal (less than 33 mcg%) and 26% had deficient and low levels of carotene. Mean values of plasma vitamin A and carotene in cord blood was 13.7 mcg% and 16.5 mcg%, respectively, and lower than that of maternal blood at term. Women who had adequate levels of vitamin A (greater than 33 mcg%) in blood had significantly higher intake of vitamin A. Similarly the newborn babies of those mothers who had adequate levels of vitamin A had significantly higher levels of vitamin A than those of mothers who had inadequate levels (less than 33 mcg%).

Dietary and serum calcium in pregnant women.

Dietary and serum calcium were estimated in 200 normal pregnant women between 37-41 weeks of gestation and in cord blood of their new born. Calcium intake per person per day was low, 11.8mmol/L, as compared with the daily recommended allowance of 30 mmol/L. Mean maternal serum calcium level was also low (1.9 mmol/L) and only 20% of mothers had serum calcium within normal range. Mean calcium value in cord blood was at lower limit of normal range, being 2.3 mmol/L; 38.5% of cord samples (77) were below the normal range.

Maternal factors affecting birthweight of uncomplicated pregnancy.

Two hundred full-term pregnant mothers with no complication of pregnancy and no associated disease selected from Jinnah Postgraduate Medical Centre were studied for their nutritional status and other characteristics. The effect of these factors on birthweight of their new-born was determined. Total calorie intake per capita per day was low being 1913. The mean birthweight was 2.9kg, 26% weighed 2.5kg or less. No significant sex difference was observed in the birthweight. Variables having a significant positive influence on birthweight were maternal age at first delivery, up to 25 years, beyond which correlation was not significant; maternal weight and maternal total calorie intake (irrespective of source) up to an intake of 2500 calorie per day, beyond which correlation was not significant. Combined effect of these three maternal factors on birthweight was highly significant. Coefficient of correlation, R being 0.81.

Excessive physical work during pregnancy and birth weight.

The effect of excessive physical work during pregnancy on birth weight was studied in 329 full term pregnant mothers with no complication of pregnancy and no associated disease delivering at Jinnah Postgraduate Medical Centre, Karachi. Birth weight was influenced by excessive physical work amongst mothers on diet deficient in calories according to calorie allowance recommended by WHO but not in mothers on recommended calories.

Serum total protein, albumin, globulin and their ratio in apparently healthy population of various ages and sex in Karachi.

Serum proteins, albumin and globulin in 456 (274 males and 182 females) subjects were within the normal range and showed uniform distribution. Mean protein levels were low in infants under 0-1 year of age. No significant change was noted with age and sex in other groups. These values were considered to be normal for our population as all protein values fell within 2.5 and 97.5 percentile (95% of the total population) irrespective of age and sex except babies under 0-1 year. The average intake of protein/kg body weight was almost upto the recommended allowances.

Dermatomal somatosensory evoked potentials: their use in lumbar radiculopathy.

The accurate diagnosis of lumbar radiculopathy secondary to intervertebral disc herniation or spinal stenosis remains a significant problem. Studies have stressed that misdiagnosis of root entrapment significantly contributes to the incidence of failed back syndrome. In an attempt to aid the proper selection of surgical candidates, dermatomal somatosensory evoked potentials ( DSSEP ) have been used in conjunction with standard diagnostic techniques to evaluate our patients. The advantage of this technique lies in the root specificity. The authors studied the DSSEP using two methods. In Method 1, using myelograms as the standard, the accuracy was 85.7%. In Method II, using surgical outcome as the standard, the accuracy was 87.5%. As a result, the authors have found the noninvasive, relatively inexpensive DSSEP to be a useful adjunct in the selection of patients undergoing lumbar spine surgery.
