Evidence of alpha 2-adrenoceptors in the nasal blood vessels of the dog.

alpha-Adrenoceptor subtypes were demonstrated in an in vitro preparation of the vascular smooth muscle of the canine nasal mucosa. alpha 1-Receptors were located on the surface of vascular smooth muscle (postsynaptic). alpha 2 Subtypes were situated both at postsynaptic and presynaptic sites on adrenergic nerve terminals. Presynaptic alpha 2-adrenoceptor stimulation yielded an inhibition of excessive norepinephrine release from nerve terminals. Stimulation of postsynaptic receptors, either alpha 1 or alpha 2, produced vasoconstriction. Although there was good evidence of the presence of postsynaptic alpha 2-receptors, they did not appear to be dominant among the postsynaptic receptors.

Calcium, calcium blockers, and nasal smooth muscle.

A canine in vitro nasal blood vessel preparation was tested to determine the effects of calcium (Ca) on smooth-muscle contraction. The contractions induced by either electrical stimulation or vasoconstricting agents were reduced or abolished by reducing the concentration of the Ca in the medium. The Ca response was dose-dependent and reversed at a high concentration. Contractions induced by vasoconstrictors were more resistant to the change of the Ca concentration than by electrical stimulation. Verapamil, a blocker of Ca entry, inhibited the contraction by simulating Ca depletion. These changes show that the extent of Ca influx regulates the smooth-muscle contraction of the nasal blood vessels consistent with the reports concerning other vascular smooth muscle.

[Complement components, whole complement activity, and circulating immune complexes in neoplastic diseases].

Serum levels of complement components(cc), whole complement activity (CH 50), and circulating immune complexes (IC) were measured in 41 patients with neoplastic diseases. The level of cc was higher than in healthy controls; the levels of C1q, C1INA, C4, C3c, C3ACT, C5, and C9 were statistically higher. In patients with lung cancer, the levels of cc were correlated with the clinical stage as well as the performance status. Both the IC serum level and the incidence of high serum IC levels in lung cancer were higher in stage III and IV than in stage I and II. Serum CH 50 was higher than in healthy controls, but not correlated with the clinical stage.

Neutrophil disorders in a child with necrotizing external otitis.

A two year old child presented with a history of cyclic fever since the sixth week of life, and otorrhea of six weeks' duration, unresponsive to treatment. Surgical removal of massive granulation tissue and sequestrated bone, combined with parenteral administration of pipellacillin and dibekacin successfully resolved the Pseudomonas infection. Neutropenia and the decreased chemotactic activity of neutrophils were observed in this patient. This neutrophil disorder seemed to be primary and a contributing factor.

Chronic myringitis and chronic suppurative otitis media.

Thirty-four chronic myringitis cases were examined by otoscopy, pure tone audiometry, tympanometry, skull X-rays, and bacteriologic study of otorrhea. Five atypical cases are presented in detail. Three of these five cases had a perforation of the tympanic membrane preceding the appearance of typical chronic myringitis. The remaining two cases showed a transient small tympanic membrane perforation during the long course of chronic myringitis. These cases suggest that chronic myringitis could occur in some chronic suppurative otitis media and that myringitis could cause tympanic membrane perforation.

Neutrophil chemotaxis in children with recurrent otitis media.

The etiology of the recurrence of acute otitis media is not thoroughly understood. Recently, abnormal functions of neutrophils have been regarded as the cause of recurrent infections, and a few papers dealt with defective chemotactic responses of neutrophils in patients with recurrent otitis media. With the in-vitro agarose plate technique, neutrophil chemotaxis and random migration were measured in 20 children with recurrent otitis media to yield a significantly decreased chemotactic index compared with age-matched controls. Patients with other recurrent infections, such as recurrent parotitis and recurrent tonsillitis, showed a normal chemotactic index. Other children who had not only recurrent otitis media, but also systemic manifestations, showed an extremely low chemotactic index and revealed either defective phagocytic activity or deficiency of IgA. Defective chemotactic response may be one of the essential factors causing repeated attacks of middle ear infection.

Personality factors in pathogenesis of polyps and nodules of vocal cords.

Patients who were diagnosed as polyps or nodules of vocal cords (PN group), and two control groups were tested with Japanese version of Maudsley Personality Inventory to evaluate the personality characteristics of patients with these behavior-induced vocal cord diseases. PN group showed significantly higher E (extraversion-introversion) scores than each control group. This extraversive tendency had no relation to their age, sex, polyp or nodule, and occupation. N (neuroticism) scores were not significantly different between PN group, and each control group. L (lying) scores of PN group were significantly lower than a control group. It was suggested that extraversive personality was one of the important factors relating to the pathogenesis of vocal cord polyp and nodule.
