Antagonism of ceruletide, a cholecystokinin analog, to the neurochemical effects of the non-competitive N-methyl-D-aspartate (NMDA) receptor antagonists, phencyclidine and MK-801, on regional dopaminergic neurons in the rat brain.

In the present study, we investigated the effects of ceruletide (CL), a cholecystokinin analog, on the neurochemical response to non-competitive N-methyl-D-aspartate (NMDA) receptor antagonists, phencyclidine (PCP) and MK-801, of the dopaminergic neuron systems in the discrete regions of the rat brain. Systemically administered PCP (7.5 mg/kg, i.p.) or MK-801 (1.0 mg/kg, i.p.) produced significant increases in the tissue contents of dopamine metabolite, homovanillic acid (HVA), in the prefrontal cortex, the nucleus accumbens and the olfactory tubercle but not in the nucleus caudatus putamen after 60 min. The effects of NMDA receptor antagonists in the nucleus accumbens and the prefrontal cortex were partially antagonized by pretreatment with CL (80 and 400 micrograms/kg, i.p., at 60 min prior to the drugs). While CL alone decreased the dopaminergic metabolism only in the nigrostriatal pathways in naive rats, the present results indicated that CL also attenuates the activities of the meso-limbic and meso-cortical dopaminergic neuron systems when these are enhanced by either PCP or MK-801.

Multicentric intracranial fibrous xanthoma--case report.

Intracranial fibrous xanthoma is extremely rare; only 11 cases have been reported so far. The authors report a case of multicentric intracranial fibrous xanthoma. Precontrast computed tomographic (CT) scans revealed a left frontal subdural mass, which had previously been diagnosed as a chronic subdural hematoma at another hospital. However, operation disclosed no hematoma but a granulomatous tumor. The biopsied specimen was histologically diagnosed as fibrous xanthoma. Postoperative postcontrast CT scans showed intense homogeneous enhancement at the clival and the left frontal regions, both of which appeared as iso- to low-intensity areas on T1-weighted magnetic resonance (MR) images and as low-intensity areas on T2-weighted MR images. She gradually recovered by conservative treatment but suddenly died of cerebral infarction. Autopsy revealed fibrous xanthomas in the left frontal and the clival regions. This is the first report of the use of MR imaging for intracranial fibrous xanthoma, and its features differ from those of common intracranial parenchymal tumors, MR imaging can be helpful in the diagnosis of this tumor.

[Computed tomography for the diagnosis of thrombosed aortic dissection with extravasation].

Four cases of thrombosed aortic dissection with extravasation demonstrated by CT were presented. In CT diagnosis for this type of dissection, cautions should be employed not only in an inhomogenous density area in the mediastinum and pleural cavity but also in the presence of deviation of intimal calcification and relatively high density area of crescent shape in aortic wall on plain CT.

[An experimental and clinical study of thin-slice high-resolution CT for lung detail].

The clinical usefulness of thin-slice high-resolution CT in the diagnosis of the chest was assessed by (1) experiments using a phantom and an inflated fixed lung (Heitzman's method), (2) evaluation of the CT delineativity of the bronchi and major fissure and (3) clinical examination of patients with diffuse pulmonary diseases. 1) In a phantom experiment using catheters, 10 mm-thick slice scan showed the 6 catheters as a single faint line. By 1.5 mm-thick slice scan, the catheters were defined as 6 separate lines. The profile images suggested that 1.5 mm-thick slice scan enables delineation of more minute details of structures. 2) In an experiment using an inflated fixed lung, 1.5 mm-thick slice scan produced more informative images that resembled macroscopic or soft X-ray images. 3) By 1.5 mm-thick slice scan, the subsegmental bronchi of both the right and left lobes were identified in most cases, while identification was possible in only half the cases by 10 mm-thick slice scan. 4) By 10 mm-thick slice scan, the sub-subsegmental bronchi were not identified in most cases. However, identification was possible in approximately half the cases by 1.5 mm-thick slice scan. 5) By 1.5 mm-thick slice scan, the major fissure was delineated as a linear shadow in most cases. It was hardly recognizable by 10 mm-thick slice scan. 6) In diffuse pulmonary diseases, 1.5 mm-thick slice scan allowed more minute visualization of the patho-morphological changes compared to 10 mm-thick slice scan. 7) Thin-slice high-resolution CT is thus expected to contribute to pathological analysis and more accurate diagnosis of pulmonary diseases.

The role of endothelin in the pathogenesis of vasospasm following subarachnoid haemorrhage.

The purpose of the present study was to evaluate the vasocontractile activity of endothelin, a newly isolated endothelium-derived constrictor peptide, in canine basilar arteries in vitro and in vivo. Endothelin at concentrations of 10(-12) M approximately 3 X 10(-8) M elicited dose-dependent contractions of canine basilar arteries in vitro. The maximum tension was larger than that induced by 40 mM KCl. The EC50 value was 1.9 +/- 0.6 X 10(-9) M (mean +/- SEM). The endothelin-induced contraction was reversed by 10(-8) M nicardipine or 10(-5) M approximately 10(-4) M papaverine. An intracisternal injection of 0.6 approximately 1.2 X 10(-12) mol/kg of endothelin caused biphasic contraction of the basilar artery lasting for more than 24 h. The initial phase of the contraction accompanied remarkable changes in vital signs such as an acute rise of blood pressure, bradycardia and respiratory arrest. An intracisternal injection of 2.0 X 10(-12) mol/kg of endothelin also induced acute contraction of the basilar artery. However, all of the dogs which received an intracisternal injection of 2.0 X 10(-12) mol/kg of endothelin died from sustained respiratory insufficiency. The present results demonstrate that endothelin induces strong and long-lasting contractions of cerebral arteries. Therefore, endothelin may play an important role in the pathogenesis of vasospasm.

Plasma adsorption using bilirubin-adsorbent materials as a treatment for patients with hepatic failure.

In order to achieve a higher degree of improvement in patients with postoperative hepatic failure, the effects of plasma adsorption (PA) using a serial connection of noncoated charcoal (N 350) and a new bilirubin adsorbent material, styrenedivinylbenzene (BR350), were investigated both experimentally and clinically. After in vitro perfusion of high bilirubin containing plasma through these columns for 3 hours, total bilirubin levels were drastically reduced to 21% of the preperfusion level in the serial connection of N 350 and BR 350, while it remained high at over 40% in the single use of each column. Total branched chain and aromatic amino acid levels were also drastically reduced in the serial connection of these columns to 50, 40, and 7%, respectively, while the total amino acid levels remained high at 87% in the single use of BR 350. The combination of these columns enhanced rather than interfered with one another. Patients who received this treatment achieved an initial reduction of plasma total bilirubin and aromatic amino acids of 57 +/- 6 and 84 +/- 7, respectively. Although the long-term prognosis for these patients was negative, improvement of clinical and laboratory findings were actually obtained by this treatment. This PA system could provide a possibility for an improved supportive therapy for hepatic failure, especially for patients with hepatic coma and hyperbilirubinemia.

Pharmacological effect of endothelin, an endothelium-derived vasoconstrictive peptide, on canine basilar arteries.

The purpose of this study was to evaluate the constrictive effect of endothelin, a peptide vasoconstrictor derived from endothelium, on canine basilar artery. Constriction was measured by an isometric tension recording method. Endothelin induced prolonged contraction of canine basilar artery in a dose-dependent fashion, the estimated concentration at 50% contraction being (2.1 +/- 0.5) X 10(-9) M (mean +/- SEM). Removal of endothelium significantly augmented the arterial response to endothelin. In Ca(++)-free medium no contractile response was elicited at 10(-8) M endothelin. Papaverine (10(-4) M) and nicardipine (10(-8) M), a calcium channel blocker, almost completely inhibited the contraction induced by 10(-8) M endothelin. Pretreatment with nicardipine (10(-8) - 10(-6) M) also significantly suppressed the response to endothelin. Acetylcholine (10(-7) - 10(-4) M), adenosine triphosphate (10(-7) - 10(-5) M), and thrombin (0.1 and 1.0 U/ml) dose-dependently inhibited contraction of canine basilar artery in response to 3 X 10(-9) M endothelin. These results show that endothelin has a potent constrictive effect on canine basilar artery and suggest that this substance may play an important role in the pathogenesis of vasospasm following subarachnoid hemorrhage.

Thermosensitive determination of patency in lumboperitoneal shunts. Technical note.

Surface cooling and thermistor recording over shunt tubing was used in 23 studies of cerebrospinal fluid shunt patency in 19 patients with lumboperitoneal shunts and normal-pressure hydrocephalus. Shunt patency was shown by downward reflection of the recording trace similar to that obtained for ventriculoperitoneal shunts. Obstruction was demonstrated by a flat-line recording or an upward deflection.

[Ultrasonic detectability of multiple small hepatic nodules].

We analyzed the ultrasonic detectability of multiple daughter nodules accompanied by 18 hepatocellular carcinomas and multiple small nodules in 11 metastatic liver tumors identified by infusion hepatic angiography. In the cases of hepatocellular carcinoma, ultrasonic detectability of daughter nodules was remarkably limited. If any daughter nodules are detected by US, there is the great possibility that many other daughter nodules are undetected. However, US is effective in detecting small nodules in the metastatic liver tumor.

[Mucin-producing signet ring cell carcinoma of stomach accompanied by microangiopathic hemolytic anemia and disseminated intravascular coagulation: a case report].

Diffuse bone marrow metastasis of carcinoma sometimes accompanies microangiopathic hemolytic anemia (MHA) which is characteristic with red cell fragmentation. We report on a 56-year-old male case of signet ring cell type gastric carcinoma complicated with MHA and DIC. Bone marrow aspiration disclosed the metastasis of carcinoma cells. However, no other distant metastasis was evident by pathological studies of autopsy samples.

[Megakaryocyte proportion versus nucleated cells in the peripheral blood showing leukoerythroblastosis].

Megakaryocytes in the peripheral blood in which leukoerythroblastosis was recognized were studied by electron microscopy on the vertically cut section of the buffy coat of the blood, and percentages of them in 10,000 nucleated cells distributed from the top of the bottom of the buffy coat were counted. In 15 of 31 patients, percentages of peripheral blood megakaryocytes ranging from 0.01% to 0.64% were seen. There was difference of the result among diseases shown peripheral blood megakaryocytes. Namely, in patients with myelofibrosis and CML, in whom extramedullary hematopoiesis was predominant, many cases ascertained peripheral blood megakaryocytes were demonstrated. Because of this result, the extramedullary hematopoiesis appears to play an important role to the presentation of megakaryocytes in the peripheral blood. On the other hand, patients indicating both megakaryocytes and abnormal sideroblasts in the peripheral blood had sideroblastic anemia marrow. This result seems to show that a part of megakaryocytes are directly flowed out from the marrow into the blood simultaneously accompanied with abnormal sideroblasts.

[Spontaneous remission in a case of non-Hodgkin's lymphoma].

A 59-year-old-male was admitted in September 1984 with fever and right hypochondrial pain. On computed tomography, a bulky retroperitoneal tumor involving the intestine was detected. Exploratory laparatomy yielded a diagnosis of non-Hodgkin's lymphoma. Although intensive and aggressive treatment combined chemotherapy and radiotherapy was administered, no conclusion could be drawn about its efficiency, and no remission was indicated by followup studies. He refused further treatment because of bone marrow depression. However, 9 months after the last treatment disappearance of the tumor was recognized on computed tomography examination. This phenomenon was confirmed by the second laparatomy performed in order to treat skin-colon fistula caused from diverticulitis of the colon. It was recognized no evidence of malignant lymphoma in the resected colon specimen. He remains in excellent health 26 months after this events. The clinical course of this case was probably compatible with the concept of spontaneous remission of malignant lymphoma.

Fragmentation of the beta-subunit of human chorionic gonadotropin produced by choriocarcinoma.

When human chorionic gonadotropin (hCG) was subjected to sodium dodecyl sulfate-polyacrylamide gel electrophoresis under reducing conditions with dithiothreitol (DTT), a smaller weight material (CTP'), in addition to the beta-subunit, could be detected by Western blot analysis using antiserum for hCG beta-carboxy-terminal peptide (CTP). The CTP' band was much more apparent with urinary hCG from a patient with choriocarcinoma than with that from normal pregnant women. Second-dimensional electrophoresis of the choriocarcinoma hCG (c-hCG) after reduction with DTT indicated that the CTP', Mr 25,000, was released from the beta-subunit. The carbohydrate structure of the CTP' was analyzed by affinity with lectin-peroxidase on a nitrocellulose membrane. The CTP' did not interact with Concanavalin A, but exhibited strong interaction with both RCA120 and Arachis hypogaea after removal of sialic acid, indicating that it was released as a fragment containing an O-linked sugar chain as was found in the hCG beta carboxy-terminal portion. Western blot analysis using the antisera for hCG alpha, hCG beta, and hCG beta-CTP showed that the CTP' contains not only the carboxy-terminal portion but also a part of the internal (core) portion of the beta-subunit molecule. This dissociation of the c-hCG beta was further supported by the presence of a faster moving component (FMC) which may correspond to the NH2-terminal side counterpart. The desialylated FMC could be detected by Concanavalin A and RCA120 but not by Arachis hypogaea, indicating that it contains N-linked rather than O-linked sugar chains. The FMC does not contain any of the epitopes for the antisera examined in Western blot. These results indicate that the beta-subunit of the choriocarcinoma urine hCG has an unusual site which is dissociated into two components of Mr 25,000 (CTP') and Mr 18,000 (FMC) by DTT reduction.

A new technique for percutaneous study of lumboperitoneal shunt patency. Technical note.

A new technique for percutaneous imaging of a one-piece lumboperitoneal shunt is described. Patency of the shunt can be assessed by the intraperitoneal spread of contrast medium which is injected intrathecally via the lumbar route. No special equipment or instrumentation is required other than a typical lumbar puncture tray and a general examination tilt table for fluoroscopy.
