OF4949, new inhibitors of aminopeptidase B. IV. Effects of OF4949 and its derivatives on enzyme systems.

OF4949-I and II inhibited aminopeptidase B from Ehrlich ascites carcinoma in a competitive way and the Ki value for both against L-arginine-beta-naphthylamide was 8 X 10(-9) M. Inhibition by I and II of various exopeptidases and endopeptidases was examined. OF4949-I and II both strongly inhibited leucine aminopeptidase and enkephalin-degrading aminopeptidase; I also inhibited enkephalinase B. The inhibitory effects of various derivatives of I and II on aminopeptidase B activity, showed that the terminal amino and carboxamide groups are essential for activity.

[Hormonal environment following treatment with a small dose of estrogen--small dose of estrogen alone and in combination with an anti-androgen].

The testosterone, luteinizing hormone (LH), follicle stimulating hormone (FSH) and prolactin levels were measured by radioimmunoassay up to one year in 21 patients with prostatic carcinoma treated with diethylstilbestrol diphosphate (DESP), 50 mg/day, and chlormadinone acetate (CMA), 100 mg/day. The purpose of this treatment was to reduce the cardiovascular side effects which were believed to occur dose-dependently and to achieve the direct drug action to the prostate gland. The same measurements were made on 9 patients treated with DESP, 50 mg/day, alone. Marked reduction of plasma testosterone and FSH levels with weaker LH suppressions was observed during the first 1-3 months in both groups. The same levels were maintained up to the end of follow-up in DESP and CMA treatment group. In the DESP treatment group, the plasma level of testosterone showed a gradual increase to 1.0 +/- 0.5 ng/ml at the sixth month. Prolactin level increased gradually in both groups. Cardiovascular side-effects were found in 29% of the patients treated with DESP and CMA and in 22% of those treated with DESP alone. In two patients in the former group gynecomastia with lactation was observed.

Sézary-like syndrome in a 10-year-old girl with serologic evidence of human T-cell lymphotropic virus type I infection.

A 10-year-old girl with a Sézary-like syndrome is described. At age 6 months, her skin involvement began as psoriatic plaques. Generalized erythema, lymphadenopathy, and pruritus developed into erythroderma. Her general condition has been good in spite of severe pruritus and erythroderma. Atypical cells were found in the peripheral blood in numbers of 2500/mm3 and were present in the dermis of a skin biopsy specimen. When examined by electron microscopy, these cells showed lobulated or indented nuclei and were similar to Sézary cells. The patient showed a high titer (1:1280) of antihuman T-cell lymphotropic virus type I antibody. Her mother was also positive with a titer of 1:40 and seems to be a healthy carrier.

[Rhabdomyosarcoma of the kidney: report of a case].

A case of rhabdomyosarcoma of the right kidney is presented. A 78-year-old man was admitted with the complaint of abdominal pain and abdominal fullness on March 15, 1985. Radiological examination showed a giant tumor of the right kidney. Radical nephrectomy and right hemicolectomy were performed. Histological findings were embryonal rhabdomyosarcoma of the kidney. Residual tumor in the duodenal area recurred and he died of peri-duodenal abscess 2 months after admission. This case is the 17th case of rhabdomyosarcoma of the kidney in Japan. The literature is reviewed and discussed.

[Clinical experience of percutaneous nephrolithotomy].

Between July, 1984 and January, 1986, 20 cases of renal and ureteral stones were treated by percutaneous nephrolithotomy (PNL). Stone removal was successful in 17 cases (85%). In 14 cases, stones were removed completely and in 3 cases there were small residual fragments. There were 4 major and 9 minor complications. The major complications consisted of loss of tract in 2 cases, bleeding from the tract after removal of nephrostomy catheter in 1 case and pneumothorax in 1 case. The minor complications consisted of small perforation during PNL in 6 cases, gross hematuria impossible to continue PNL in 2 cases and ureteral injury due to ureteral occlusion catheter in 1 case. No patients required blood transfusion. In near future all urologists will be required to master the technique of PNL.

[Direct renal pelvic puncture in the bacteriologic study in patients with uretero-intestinal urinary diversion (a new technic for diagnosis of upper urinary tract infection)].

The renal pelvis of ten patients with ileal or colonic conduit was punctured with a 22G spinal needle under fluoroscopic guidance. Renal pelvic urine was obtained from nine patients. Two patients temporarily developed gross hematuria as a complication due to this procedure. The obtained renal pelvic urine was investigated bacteriologically. Direct renal pelvic puncture was concluded to be a useful and safe technique for locating urinary tract infection. Reflux of infected urine into the renal pelvis did not occur in any of the nine patients who had no obstruction of the ileal or colonic conduit.

[Two cases of renal angiomyolipoma].

Two cases of renal angiomyolipoma without tuberous sclerosis are reported. The first case was of a 35-year-old man with complaints of right upper abdominal and right flank pain. Preoperative diagnosis was right renal angiomyolipoma. Thoracoabdominal radical nephrectomy and lymphadenectomy were performed. The pathological diagnosis was renal angiomyolipoma with lymph node involvement. The second case was of a 46-year-old woman whose left renal mass had been accidentally found by ultrasound study. Preoperative diagnosis was left renal angiomyolipoma. This tumor was enucleated from the left kidney through flank incision.

A human T cell line established from a patient with Sézary syndrome. Application for assay of human interleukin 2 (IL-2).

A human T cell line, designated Sez 627, was established from a patient with Sézary syndrome. These clonal T cells have been cultured for more than 2 years in the presence of IL-2 without any antigen stimulation. The surface phenotype of Sez 627 was OKT3+, OKT4-, OKT8+, and Tac+, and infection with human T lymphotrophic virus type I (HTLV-I) was demonstrated by Southern blot hybridization analysis. In human IL-2 assay, Sez 627 cells were found to be superior to murine CTLL-2 cells with respect to their unresponsiveness to phorbol 12-myristate 13-acetate (PMA), and species specificity for human IL-2, as well as better cryopreservation, although they were less sensitive to IL-2 than CTLL-2 cells. Using Sez 627 cells, IL-2 production by lymphocytes from patients with systemic lupus erythematosus (SLE) was examined. Decreased IL-2 production was observed in the patients with active SLE but not in most patients with inactive SLE. These findings suggest that Sez 627 is a useful human T cell line for human IL-2 assay.

Cyclic AMP-dependent protein kinase in calf-snout epidermis.

The biochemical characteristics of cyclic AMP-dependent protein kinase in calf-snout epidermis were investigated. The activity of cyclic AMP-dependent protein kinase was higher in the lower layer than the upper layer of epidermis. The supernatant of homogenates of the lower layer of calf-snout epidermis was fractionated by DEAE-cellulose chromatography and contained two major peaks of protein kinase activity stimulated by cyclic AMP. This chromatographic pattern is similar to that referred to as Type I and Type II of cyclic AMP-dependent protein kinase in bovine muscle. Both peaks of cyclic AMP-dependent protein kinase in calf-snout epidermis could phosphorylate keratin polypeptides in vitro. The phosphorylation reaction was activated by cyclic AMP and inhibited by a heat-stable inhibitor of cyclic AMP-dependent protein kinase. When Type II enzyme of cyclic AMP-dependent protein kinase was incubated with [gamma-32P]ATP in the absence of substrates, such as histone or keratin polypeptides, the 54,000 dalton protein was phosphorylated and this autophosphorylation was inhibited by the addition of 10 microM cyclic AMP. These results suggest that cyclic AMP-dependent protein kinase in calf-snout epidermis has properties similar to those in bovine muscle and plays an important role in the phosphorylation of keratin polypeptides in calf-snout epidermis.

Ear swelling in response to UVB irradiation.

The skin response to UVB irradiation in mice was evaluated by means of ear swelling. ICR albino mice were irradiated with 500 mJ/cm2 UVB and the effect of various drugs on ear swelling was examined 24 h after irradiation. Intravenous injections of betamethasone (0.8 microgram/g body wt.) or indomethacin (24 micrograms/g) remarkably inhibited ear swelling, whereas intraperitoneal injections of diphenhydramine (20 micrograms/g), cimetidine (10 micrograms/g), or a combination of both these antihistamines did not. In contrast, the number of sunburn cells counted 24 h after UVB irradiation (200 mJ/cm2) in mouse ears was not affected by these drugs. The amount of prostaglandin D2 (PGD2) in mice ears at various intervals after irradiation with 500 mJ/cm2 UVB was determined by radioimmunoassay. Compared with the values before irradiation, the PGD2 levels were significantly higher 3 and 6 h after irradiation and gradually decreased and returned to the basal level by 12 h, although ear swelling continued after 12 h. These results suggest that prostaglandins are responsible at least in part for the development of ear swelling, but not for sunburn cell formation induced by UVB, and that ear swelling represents a simple and useful response model for the rapid in vivo screening of nonsteroidal or steroidal anti-inflammatory agents.

Percutaneous intrapelvic pressure registration in patients with ureterointestinal urinary diversion.

Intrapelvic pressure registration using percutaneous needle renal pelvic puncture in 4 patients with intestinal loop diversion was done to determine whether or not there is loop-ureteral reflux under physiologic condition in loop diversion without any urinary tract obstruction. There was no pressure elevation related to reflux on pressure recording. We concluded that loop-ureteral reflux in intestinal loop diversion does not occur without obstruction.

OF4949, new inhibitors of aminopeptidase B. I. Taxonomy, fermentation, isolation and characterization.

New aminopeptidase B inhibitors that we named OF4949-I, II, III and IV were isolated from the culture broth of a fungus, Penicillium rugulosum OF4949. The molecular formula of I was C23H26N4O8 and that of II, C22H24O8, judging from elemental analysis and secondary ion mass spectrometry. The concentrations of I, II, III and IV required for 50% inhibition of aminopeptidase, using Ehrlich ascites carcinoma cells as the source of the enzyme, were 0.0054, 0.0048, 3.4 and 1.7 micrograms/ml, respectively. Components I and II augmented delayed-type hypersensitivity in mice to sheep red blood cells.

OF4949, new inhibitors of aminopeptidase B. II. Elucidation of structure.

The structures of OF4949-I, II, III and IV were identified by analysis of the products of their chemical degradation and by 1H NMR, 13C NMR, and mass spectrometry. These compounds were new cyclic peptides containing diphenyl ether as a chromophore. OF4949-I had two amino acids, beta-hydroxy-L-asparagine and 4-methylisodityrosine. The structural differences between I and II and between III and IV lay solely in the diphenyl ether moiety; the phenolic hydroxyl group in II and IV was methylated in I and III. OF4949-III and IV contained L-asparagine instead of the beta-hydroxy-L-asparagine moiety of I and II.

Colloid body formation in bullous pemphigoid.

Thirty-eight biopsy specimens from 18 cases of bullous pemphigoid (BP) were observed using direct immunofluorescence (IF) techniques with fluorescein isothiocyanate (FITC)-labelled antisera against human serum factors. In addition to deposits of immunoglobulins and serum components at the basement-membrane zone (BMZ), 15 specimens from eight cases displayed homogeneous and globular fluorescent bodies in the uppermost dermis and/or the blisters when FITC-labelled antisera to human IgM and other serum factors were used. Using immunoperoxidase staining, haematoxylin/eosin (HE) and periodic acid-Schiff (PAS) staining, these immunoglobulin and/or complement-positive cell-sized bodies were shown to be slightly eosinophilic and PAS positive. Electron microscopy revealed entangled networks of microfilaments approximately 7-8 nm in diameter. These homogeneous, fibrillar bodies were histologically, immunohistologically and ultrastructurally indistinguishable from the colloid bodies found in lesional skins of lichen planus, lupus erythematosus, dermatomyositis and several other dermatoses. In BP, degenerated keratinocytes adjacent to the blister roof, may, after undergoing a filamentous change, drop off into the dermis and subsequently form homogeneous, fibrillar bodies in the uppermost dermis when reepithelization is completed.

Effect of ultraviolet irradiation on mast cell-deficient W/Wv mice.

The effect of UV irradiation on the skin was investigated in (WB-W/+) X (C57BL/6J-Wv/+)F1-W/Wv mice, which are genetically deficient in tissue mast cells. Their congenic littermates (+/+) and normal albino mice (ICR or BALB/c) were used as controls. Mice were irradiated with 500 mJ/cm2 of UVB and the increment of ear thickness was measured before and 6, 12, and 24 h after irradiation. Ear swelling in W/Wv mice at 12 and 24 h after irradiation was significantly smaller than that in +/+ and ICR mice. In contrast, the number of sunburn cells formed 24 h after UVB irradiation (200 or 500 mJ/cm2) was similar in W/Wv, +/+ and ICR mice. On the other hand, when mice were treated with 8-methoxy-psoralen (0.5%) plus UVA irradiation (4 J/cm2) (topical PUVA), ears of W/Wv and BALB/c mice, which were both white in color, were thickened similarly 72 h after treatment, but less swelling was observed in +/+ mice, which were black in skin color. The amount of prostaglandin D2 (PGD2) in ears, determined by radioimmunoassay specific for PGD2, was elevated 3-fold in +/+ and ICR mice at 3 h after irradiation with 500 mJ/cm2 of UVB in comparison with basal level without irradiation. However, such elevation was not observed in W/Wv mice. These results suggest that mast cells play an important role in UVB-induced inflammation, and PGs from mast cells are responsible at least in part for the development of this reaction. However, neither mast cells nor PGs contribute to the sunburn cell formation and ear swelling response by PUVA treatment.
