[Cytophotometric DNA analysis of gastric carcinoma--relationship between DNA ploidy and lymph node metastasis].

Two hundred forty-four cases of gastric carcinoma were subjected to cytophotometric DNA analysis. DNA distribution patterns were roughly classified into low and high ploidies. The ratio of high ploidy was 55.7% (44/79) in the positive group of lymph node metastasis, which was significantly higher than 31.5% (52/165) in the negative group (P less than 0.01). The incidence of lymph node metastasis increased rapidly in the high ploidy cases with the progression of carcinoma, while it increased slowly in the low ploidy cases. DNA ploidy was well correlated with lymph node metastasis, and the mode of metastasis seemed to be different between the low and high ploidy groups.

Glandular or mucus-secreting components in squamous cell carcinoma of the esophagus.

A review of 195 patients with carcinoma of the esophagus disclosed 41 cases (21.0%) with glandular and/or mucus-secreting components, in addition to the ordinary component of squamous cell carcinoma. These tumors could be grouped into three types according to representative histologic features of glandular and mucus-secreting portions: glandular type (23 cases), cribriform type (11 cases), and mucoepidermoid type (7 cases). The histologic features of the three types were reminiscent of those of adenocarcinoma, adenoid cystic carcinoma, and mucoepidermoid carcinoma of salivary glands, respectively. Moreover, areas showing glandular or mucus-secreting differentiation were in greater part located in the submucosa and the lamina propria mucosae, thereby suggesting that such differentiation had arisen in the esophageal glands or their ducts. In all 41 cases, the ordinary element of squamous cell carcinoma, invasive, or noninvasive, was admixed in various proportions with the glandular components, indicating that this type of esophageal tumor had originated not only from the covering squamous epithelium but also from esophageal mucous-gland or ductal epithelium. The findings also support the concept of the field origin of carcinogenesis in esophageal carcinoma.

Positive control of transcription initiation in Escherichia coli. A base substitution at the Pribnow box renders ompF expression independent of a positive regulator.

Expression of the ompF gene coding for a major outer membrane protein of Escherichia coli is positively regulated by the product of the ompR gene, OmpR. Using an ompF-tet chimera gene, ompF promoter mutants that render the ompF expression independent of the OmpR protein were isolated. In all of the four mutants that were isolated separately, the first base of the Pribnow box was changed from A to T. The mutant promoter did not require the upstream domain of the -35 region that is required for the OmpR-dependent functioning of the wild-type promoter. It is concluded that the domain upstream from the -35 region plays a role in the positive regulation by the OmpR protein. A statistical survey of the E. coli promoter sequence revealed that almost all of the genes that do not require an activator protein for their expression possess T at the first position of the Pribnow box, while the position is occupied by other bases in almost all of the positively regulated genes. Based on these facts, the mechanism of positive regulation of the gene expression by an activator protein is discussed.

Cellular vitamin A-binding proteins in liver tumors and adjacent tissues.

The levels of cellular retinol-binding protein (CRBP) and retinoic acid-binding protein (CRABP) were assessed in surgically resected ten human liver tumors and their adjacent tissues by sucrose density gradient centrifugation and Scatchard analysis. The tissues adjacent to hepatocellular carcinomas usually showed portal cirrhosis or hepatitis. There was no significant difference in the dissociation constant (Kd) values of binding proteins of CRBP or CRABP between tumors and adjacent tissues. Five of the ten liver tumors showed similar levels of CRBP as in their adjacent tissues whereas three of the ten liver tumors showed about 40% or less contents of CRBP in comparison with their adjacent tissues and two others had no CRBP activity. By contrast, CRABP activities were found in five liver tumors and in adjacent tissues of one case, whereas there were no detectable CRABP activities in the adjacent tissues except in one case.

Acute gastric anisakiasis. Analysis of 178 cases.

From 1969 to 1984, we treated 178 patients with acute gastric anisakiasis caused by gastric mucosal penetration of the Anisakis larvae. Because the larvae of Anisakis can always be observed by endoscopy and removed with biopsy forceps, early performance of endoscopy is highly recommended for patients in whom acute gastric anisakiasis is suspected and for those who have eaten raw fish within 12 hours before onset of the gastric symptoms. Endoscopic removal is essential to relieve the intolerable pain; after relief, the administration of antacids will repair the damaged gastric mucosa. Ulcer formation is rare.

Acute portal hypertension associated with liver resection. Analysis of early postoperative death.

Changes in portal pressure before and after hepatic resection were monitored in 65 patients. Significant increases in portal pressure, from 226 +/- 13 mm saline to 277 +/- 16 mm saline were noted in 17 cirrhotics undergoing major hepatic resection. In 14 noncirrhotics undergoing major resection of the liver, pressure in the portal vein changed significantly from 198 +/- 10 mm saline to 226 +/- 9 mm saline. Conversely, there were no differences in the 26 cirrhotic and eight noncirrhotic individuals who underwent minor hepatic resection. Clinical analysis of these patients showed that acute portal hypertension induced by liver resection was not linked to increases in early postoperative death.

The value of ultrasound in the diagnosis and treatment of small hepatocellular carcinoma.

Thirty-two patients with a solitary hepatocellular carcinoma of less than 4 cm diameter were treated during the 13 years from 1971 to 1983, two-thirds of them since 1981. Serum alpha-fetoprotein levels proved to be an excellent diagnostic tool for these early stage carcinomas which are generally asymptomatic. Ultrasonography forms an invaluable test for the detection of occult liver cancer and has enhanced the ability to detect small sized lesions. The accuracy of this method increased from 25 per cent for the period 1971-1980 to 89 per cent for the period 1981-1983. The cumulative survival rates for the 32 patients treated by hepatic resection at 1, 2 and 5 years were 85, 70 and 22.2 per cent (excluded from these figures are four operative deaths giving a 12.5 per cent operative mortality).

Evaluation of surgical treatment of carcinoma of the oesophagus in the elderly: 20 years' experience.

The results of 231 oesophageal resections and reconstruction in 395 patients with thoracic oesophageal carcinoma presenting in the last 20 years are reported. In patients over 70, the incidence of multiple pathology and postoperative complication was high. The most frequent causes of death after surgery were pulmonary complications, and anastomotic leakage. As the mortality rate in the aged group exceeded that of the younger groups, careful pre-operative assessment, the indication for surgery and postoperative care should be carefully considered, even though only surgery offers a hope of cure for patients with oesophageal carcinoma. If the elderly can tolerate the surgical procedures, the 5 year survival rate equals that of the younger patients.

Early gastric carcinoma in Japanese patients under 30 years of age.

The clinicopathological characteristics of early gastric carcinoma (EGC) in 23 Japanese under 30 years of age were reviewed and the findings compared with data on 53 patients over age 70. In the young patients, there was a female preponderance, a macroscopic solitary lesion and the cancer was of the poorly differentiated type without intestinal metaplasia in the surrounding mucosa. In the old patients, there was a male preponderance; about half the number of patients had macroscopically elevated lesions and 10 of the 53 had a multifocal gastric carcinoma. Here, most lesions were well to moderately differentiated types with intestinal metaplasia in the surrounding area. Growth patterns of EGC in the young patients were analysed. Curative operations with dissection of regional lymph nodes were done in all the young patients. Metastases to the regional lymph nodes were present in only 2 of 23. Twenty of twenty-three followed for 5 years or longer are well.

Surgical treatment of concurrent cancer of the esophagus and stomach.

Out of a total of 250 resections for carcinoma of the esophagus done in our department between 1965 and 1983, there were 11 patients with a concurrent esophageal cancer, nine being gastric cancer and seven of them early gastric cancer. In these nine, four underwent resection of the esophagus and lesser curvature of the stomach with gastric tube reconstruction. Two of the four patients are alive 10 and 3 yr postoperatively. We hold the view that detailed preoperative gastric examination is most important to plan the surgical repair and that the small gastric tube may serve as a substitute by resecting the lesser curvature of the stomach in cases of early proximal gastric cancer.

DNA cytophotometry on atypical glands in stomach carcinogenesis of dogs induced by N-methyl-N'-nitro-N-nitrosoguanidine.

Microspectrophotometric measurement of the DNA content of cell nuclei was performed on the lesions (including atypical glands) in gastric carcinogenesis of 15 male beagle dogs, which had been induced by N-methyl-N'-nitro-N-nitrosoguanidine (MNNG). The distribution patterns of DNA content were classified into three types: normal, subnormal, and abnormal. The histograms of the distribution in normal and regenerative glands were a normal type and subnormal type, respectively, while adenocarcinoma showed an abnormal distribution type. In atypical glands, the distribution patterns in autopsy cases were subnormal and abnormal types. When sequential endoscopic observation of the angulus of the stomach in dog No. 3 was carried out, atypical glands were found in an ulcer in the early stage of MNNG administration and a precancerous lesion in the late stage after termination of MNNG. The atypical glands in the early stage were of the subnormal type, while the atypical glands in the late stage were of the abnormal type. According to the results, these two types-subnormal and abnormal - of distribution of DNA content on the atypical glands may be related to regeneration and subsequent development of cancer, respectively.

Hyperdynamic circulation in the left gastric venous area in patients with portal hypertension: angiological assessment.

The time of appearance of the left gastric vein on serial celiac arteriograms in patients with portal hypertension and esophageal varices was compared with that of the portal vein to assess regional hemodynamics in the left venous portion of the stomach, an area located in close proximity to the varices. In two thirds of all the patients with cirrhosis or non-cirrhotic idiopathic portal hypertension (IPH), the left gastric vein was visualized earlier or simultaneously than or with the portal vein, while in all but one patient with prehepatic portal obstruction, there was a delayed opacification of the left gastric vein. These results suggest the presence of a hyperdynamic circulatory state which promotes venous hypertension in the left gastric venous area of the stomach of a considerable number of patients with cirrhosis or IPH. In such a hemodynamic state, selective decompression of varices can be achieved by a left gastric venous caval shunt.

Synergistic effect of K-18 and concomitant hyperthermia for treatment of Lewis lung carcinoma in mice.

We investigated the synergistic anti-tumor effects of K-18 (conjugate of human gamma-globulin and melphalan) and concomitantly administered hyperthermia on Lewis lung carcinoma in mice. The antitumor effect of a subeffective dose of K-18 or an equivalent dose of melphalan alone was enhanced by local hyperthermia. K-18 administration demonstrated a greater potentiation for inhibition of tumor growth than that of melphalan alone. Local hyperthermia plus K-18 reduced the dose of melphalan required for tumor growth inhibition by melphalan alone. The combined application of K-18 and hyperthermia for treating cancer warrants further study.

Serum CEA levels facilitate detection of recurrences of cancer in patients after gastrectomy.

In an attempt to assess the usefulness of carcinoembryonic antigen (CEA) for predicting the progression of gastric cancer, CEA productivity was evaluated according to serum CEA levels, at the time of recurrence or relapse. In cases of a recurrence, abnormal CEA levels were observed in 14 of 17 (82.4 per cent) with differentiated carcinoma and in 9 of 21 (42.9 per cent) with undifferentiated carcinoma. Preoperative abnormal CEA levels were observed in only 6 of 41 patients (14.6 per cent). A median lead time of manifestation of recurrence was 5 months. In those with relapse, 9 of 11 (81.8 per cent) patients with differentiated carcinoma and 13 of 18 (72.2 per cent) with an undifferentiated carcinoma had abnormal CEA levels. Preoperative abnormal CEA levels were observed in 24 of these 89 patients (27.0 per cent). Postoperative monitoring of CEA seems to be useful for early recognition of gastric cancer progression, irrespective of the preoperative CEA levels.

Mucoepidermoid carcinoma of the esophagus--a case report.

We treated a 66-year old woman with mucoepidermoid carcinoma of the esophagus. The histologic features of this tumor seemed to originate from esophageal glands and their ducts. This deduction was based on the subepithelial growth pattern and the presence of in-situ carcinoma showing a glandular or squamous pattern at the location of the esophageal gland duct. Although the biological nature of this tumor was not elucidated, the prognosis is similar to that seen with the ordinary type of esophageal squamous carcinoma.

Esophageal varices produced in dogs.

In attempts to produce esophageal varices in dogs, we designed a procedure based on the concept of hyperdynamic flow. The first stage operation involved the achievement of an arteriovenous shunt between the left renal artery and the proximal splenic vein, in concert with the distal splenic venous-left renal venous shunt. About one month after the initial operation, sixteen of the twenty-eight dogs had tolerated the manipulations. Both shunts proved to be functioning well in six of sixteen survivors, in which an ameroid constrictor was placed around the splenic vein just proximal to the confluence of the left gastric vein. Again one month later, in five of these six, esophageal varices were evidenced, both endoscopically and histologically. Arterialization of left gastric vein concomitant with the distal splenorenal shunt, gave rise to a hyperdynamic state in the upper stomach and resulting in reproducible esophageal varices.

Antitumor effect of lipiodolized doxorubicin on VX2 carcinoma in rabbits.

Twelve-day-old VX2 carcinoma was inoculated in the hind leg of 24 rabbits and, after 12 days, Doxorubicin (Adriamycin) suspended in Lipiodol Ultra-Fluid (Lipiodol) was then given through the femoral artery. A selective deposit of the contrast material in the tumor for an extended time was evident on the x-rays and the antitumorous effect was remarkable. Lipiodolized antitumor agents warrant further investigation for possible clinical application.
