OBJECTIVE: To examine the relationship between serum insulin and lipoprotein (a) [Lp(a)] concentrations in both sexes in a large population. RESEARCH DESIGN AND METHODS: The authors conducted a cross-sectional investigation of fasting serum concentrations of insulin and Lp(a), other blood tests, blood pressures, anthropological measurements, physical activity index, smoking habit, alcohol consumption, and menopause. The subjects were 1,121 men and 1,480 women, ranging between 30 and 90 years of age, who were voluntary participants in the Jichi Medical Cohort Study and who resided in one of five rural communities in Japan. RESULTS: In men, insulin, age, BMI, LDL cholesterol, triglycerides, glucose, and fibrinogen were significantly correlated with Lp(a). In women, insulin, age, LDL cholesterol, fibrinogen, and activated coagulation factor VII were significantly correlated with Lp(a). However, all correlations were weak in either sex. Insulin was inversely correlated with Lp(a) in both sexes. However, the coefficients were weak (r = -0.16 in mean and r = -0.06 in women). In the partial correlation analyses, Lp(a) was not significantly associated with insulin in either sex. The result was not influenced by selecting the subjects on the basis of detectable values of insulin and Lp(a) and stratifying them by serum glucose level. CONCLUSIONS: There was no significant relationship found between serum insulin and Lp(a) concentrations in either sex of the cohort, which indicates that Lp(a) does not play a role in the increased risk of cardiovascular disease in hyperinsulinemic subjects.
Prospective study was conducted in order to investigate the relationship between cognitive and behavioral disturbances and falls in dementia. Falls and fall-related fractures were studied in 154 ambulatory patients who were admitted to a geriatric intermediate nursing facility with a diagnosis of Alzheimer's type dementia (DAT) or mixed Alzheimer and vascular dementia (MIX). The Mini-Mental State (MMS) was used for the evaluation of cognitive status, and the Dementia Behavior Disturbance Scale (DBD) was used as a behavioral parameter. In order to evaluate problematic behavior in walking, a derivative scale (DBD-W) was developed from the DBD by choosing three items relevant to wandering. During the 3 months of observation, 61 patients fell more than once; 15 of them experienced fractures. The mean MMS scores were higher in non fallers, fallers who did not sustain fractures and those complicated with fractures, in that order, and the difference between the non fallers and fallers with fractures was statistically significant. When the subjects were divided into three subgroups on the basis of the MMS scores, there was a trend toward a higher ratio of fallers or those complicated with fractures in the subgroup with the lower MMS score. The DBD and DBD-W scores were not associated with falls or fractures, although fallers had a slightly higher DBD or DBD-W score than non fallers. These findings suggest that the risk of falling or fracture becomes higher with the advance of cognitive impairment in institutionalized ambulatory patients with DAT or MIX. The findings also suggest that behavioral disturbances are not necessarily associated with falls or fractures.
Effects of freezing, thawing, and storage at room temperature of human liver samples on the contents and catalytic activities of individual forms of cytochrome P450 (P450 or CYP) were examined. The stability of liver genomic DNA was also investigated. There were no significant decreases in microsomal levels or catalytic activities of P450 enzymes by storage at -80 degrees C for 5 years. We then examined the effects of freezing/thawing of liver samples. Levels of P450 forms were determined immunochemically and by the activities of typical drug oxidation reactions in liver microsomes of human samples, which were divided into two groups. One group of samples was thawed and kept at 25 degrees C for 6 hr and then frozen again and kept for 1 week at -80 degrees C. In the other group, liver microsomes were prepared at the same time (as those from the other group), but not thawed and refrozen. Thawing the liver samples and storage for 6 hr at 25 degrees C decreased contents of total P450 by about 90% and activities of both NADH-ferricyanide and NADPH-cytochrome c reductases by about 80%. However, the decrease in b5 levels was only about 30%. Spectral studies of P450 suggested that thawing the liver samples and holding them at 25 degrees C produced inactive form P420. P450 proteins were detected by immunoblot analysis with or without thawing, but catalytic activities for individual P450s were decreased drastically by thawing and holding samples for 6 hr at 25 degrees C. Only 10% of tolbutamide methyl hydroxylation activity was present, and there was no detectable ethoxyresorufin O-deethylation activity in such microsomes after thawing and holding samples for 6 hr at 25 degrees C. Genomic DNA from human livers was also found to be degraded after the samples were thawing. These results suggested that thawing and holding the liver samples at 25 degrees C decreased the levels and activities of P450s in microsomes and that there are differences in stabilities in individual forms of P450 proteins. P450 proteins determined immunochemically do not always reflect P450-catalytic functions in human liver microsomes because of difficulties in obtaining fresh liver samples.

GBV-C is recently discovered RNA virus which appeared to be member of Flaviviridae. We previously reported the possible involvement of GBV-C in the etiology of fulminant hepatitis(FH). It is still controversial whether GBV-C cause FH. So far, the only reliable tool for the diagnosis of GBV-C is the detection of the viral genome using PCR. Detection of GBV-C in serum of patient with FH dose not necessarily mean that GBV-C is causal virus. Serial quantification of serum GBV-C RNA in patient with FH may reveal a pathogenetic role of GBV-C. Further study is needed to elucidate the relationship between FH and a specific strain of GBV-C.
The quantitative assessment of infero-posterior defect on 123I-metaiodobenzylguanidine (MIBG) myocardial scintigraphy was studied in 150 patients with non-ischemic heart diseases at rest. The bull's eye map, obtained from the SPECT images at 4 hours after MIBG injection, was evaluated by generating the blacked out map which exhibits regions with reduced % uptake under mean-2 SD of 13 normal controls. The blacked out regions involved infero-posterior segments and were closely resembled to the sector form. The central angle of this sector, which was named as angle of defect (AOD), significantly correlated with both the heart-to-mediastinum activity ratio (H/M) and the myocardial clearance of MIBG. Because H/M and clearance are widely used as quantitative indices in MIBG myocardial scintigraphy, these results indicate that AOD can also be used as a quantitative index of abnormal cardiac sympathetic nervous function, which is likely to appear in infero-posterior regions in non-ischemic cardiac diseases.
The patient was a 65-year-old male with recurrent gastric cancer accompanied by metastasis to the liver and distant lymph nodes. He was treated by combination chemotherapy consisting of cisplatin 20 mg/body (day 8-14) and Carmofur 400 mg/body (on consecutive days), in three four-week courses of therapy. After 1 course of administration, the target lesion was reduced in size and the performance status was improved. Generally, a partial response was confirmed by computed tomography (CT). The patient has survived for 8 months in a state of PR. Adverse effects were mild and transient. Though treatment of recurrent gastric cancer with liver metastasis is often ineffective, this case suggests that a combination therapy of cisplatin and carmofur might be effective.

Isolated rat hearts were subjected to 30 min global ischemia followed 20 min reperfusion with various concentration of Mg2+ Krebs-Henseleit-bicarbonate buffer (KHB buffer) (Mg2+:1.2 mM, 2.5 mM, 5 mM, 10 mM) and 20 min reperfusion with original KHB buffer (Mg2+:1.2 mM) at 37 degrees C, respectively. Cardiac function and the incidence of ventricular arrhythmia were measured during pre-ischemia and reperfusion, and intracellular Ca2+ concentration was measured during pre-ischemia, ischemia and reperfusion period. The hearts reperfused with 1.2 mM Mg2+ presented a significantly higher frequency of ventricular fibrillation (VF) and ventricular tachycardia (VT) than the other groups. Percent recoveries of LVDP (Left ventricular developed pressure) at the Mg2+ concentration of 1.2, 2.5, 5, 10 mM after 40 min reperfusion were 64.3 +/- 5.7%, 108.4 +/- 6.1%, 100.4 +/- 6.1%, 101.5 +/- 5.5%, respectively. Intracellular Ca2+ concentration of the hearts with Mg2+ 1.2 mM revealed 248.9 +/- 15.6% at reperfusion, and that the other groups did not increased during reperfusion. These data suggest that extracellular Mg2+ inhibits reperfusion arrhythmia and Ca2+ overload during reperfusion, and improves cardiac function of reperfused heart.
A 6-day-old female presented with vomiting and an abdominal mass. At laparotomy, a pyloroduodenal duplication cyst was enucleated from the pyloric region. Of the diagnostic studies performed, IV cholangiography with spiral computed tomography and an upper gastrointestinal barium study were useful in the preoperative differential diagnosis.
