A continuing use for Kielland's forceps?

A retrospective study over a 5-year period compared neonatal outcomes after birth by Kielland's forceps with those after caesarean section in the second stage of labour. The 253 babies born by these two modes of delivery showed no difference in Apgar score, the need for active resuscitation, incidence of jaundice or abnormal neurological behaviour. The neonatal outcome was no worse in the small number of patients where Kielland's forceps delivery was attempted but failed. This study offers support for the continuing role of Kielland's forceps in modern obstetrical practice.

Fetal weight estimation by real-time ultrasound measurement of biparietal and transverse trunk diameter.

Measurements of biparietal diameter and transverse trunk diameter with the use of linear array real-time B-mode scanning were performed on 100 fetuses within 72 hours of delivery to evaluate the reliability of Hansmann and associates, nomogram in predicting fetal weight. Under optimal conditions, 82% of predicted birth weights were within 10% of actual birth weight with a mean error of 150 gm (6%) and a correlation coefficient of 0.98. Predictions were most accurate in fetuses weighing less than 1,500 gm and in the 2,000 to 4,000 gm group and were least accurate in the infant weighing less than 1,500 gm and in the 2,000 to 4,000 gm group and were least accurate in the infant weighing more than 4,000 gm. Once the accuracy of the method was determined, It was applied to 100 consecutive cases to evaluate the feasibility of this technique in clinical practice. Measurements were unobtainable in 15%, but in the remainder, predictions were of sufficient accuracy throughout the range of fetal weights to complement clinical assessment in the management of high-risk pregnancies. Seventy-four percent of predicted weights were within 10% of actual birth weight; the mean error was 165 gm (7.2%) and the correlation coefficient was 0.97.

Toxic shock syndrome in the postoperative patient.

Toxic shock syndrome occurred in two patients within seven days of operation in women in whom Staphylococcus aureus was isolated from the wound incisions. Both patients had negative vaginal cultures. One patient was menstruating at the time of onset of the disease and the other was seven days' postpartum. Although toxic shock syndrome mainly occurs in menstruating women who use tampons and have positive vaginal cultures for Staphylococcus aureus, wound infection may also be a source for toxin production and adsorption.

The sequelae of serous retinal detachment in preeclampsia.

Retinal detachment is a rare complication of preeclampsia. In 24,920 consecutive deliveries from 1970 at the Vancouver General Hospital, preeclampsia was diagnosed in 1435 patients (5.76%), and in 2 cases serous retinal detachment occurred. Despite spontaneous retinal reattachment, focal pigmentary macular disturbances with visual impairment may persist. Serial examination of the optic fundi, visual acuity, and central visual field assessment by Amsler grid are simple, indispensable tests that will help identify the visually threatened patient with severe preeclampsia. Timely obstetric intervention in severe cases is suggested.
