[Two-line triceps surae augmentation-plasty. An alternative treatment concept in surgery of Achilles tendon rupture].

From January 1990 to December 1992 a total of 62 patients with subcutaneous Achilles tendon ruptures underwent surgical treatment. The majority of ruptures were caused by sports injuries. In a retrospective study, 25 cases who had a two-line triceps surae augmentation graft in addition to standard repair were evaluated. The functional results after 6-24 months were studied with reference to the patients' activity and graded according to the classification of Trillat. This alternative treatment concept gave excellent functional results. Surgical treatment of Achilles tendon ruptures, if necessary for reconstruction with a primary graft for augmentation, remains the appropriate therapy.

[Classification of unstable pelvic ring injuries--treatment methods].

Any classification of pelvic ring injuries has to take respect of the mechanism of injury, the anatomy and the dimension of instability. These factors involve a base for a standard conception of treatment and evaluation of the results of treatment. Between 1/90 and 12/91 50 patients with injuries of the pelvic ring were treated. The results of treatment and follow up study of 14 patients with instable injuries type B and C (Tile 1980) show the necessity of a conception basing on the registration concerning the form of injury and stability. Indications and operative procedure of the anterior and posterior stabilisation will be presented. Only stability of the pelvis will guarantee the early mobilisation and rehabilitation of any patient.

[Importance of the Segond avulsion fracture as a sign of complex ligamentous knee injury].

The Segond fracture is a small bony avulsion of the lateral tibial condyle. A study of 11 patients, treated between 1986 and 1990, points out the importance of this injury. In our series all patients showed an additional major ligamentous damage. 10 patients had an injury of the anterior cruciate ligament. This harmless "lateral capsular sign" on the standard x-ray of the knee-joint should give reason for further diagnostic measures, mainly the arthroscopic examination.

Morphometric analysis of the angioarchitecture of the synovial membrane in rheumatoid arthritis and osteoarthritis.

Using three different immunohistochemical methods we measured the number of vessels, vessel area and diameter and their form factor in the synovial membrane of 102 patients suffering from different joint disease. The variables were evaluated by means of immunomorphometric analysis. We found UEA (Ulex europeus) immunostaining to be the optimal method for quantification of data characterizing the vasculature of the synovial membrane. Irrespective of causes of the given joint disease, we found increases in the number of vessels, vessel perimeter, vessel area and the product of the number of vessels and vessel area over and against the control group (patients without arthritis). Consequences are discussed regarding local bioavailability of medicaments in the synovial membrane.

[Prognosis of thoracoabdominal trauma with reference to concomitant injuries].

A total of 301 patients with thoracoabdominal lesions had concomitant craniocerebral lesions (69.1%) and injuries of the locomotor system (66.1%). In 65.4% operative intervention was necessary, while 34.6% could be primarily treated conservatively. Operative emergency therapy was required in the thorax in 34.7%, and in the abdomen in 50%. Under these conditions, the mortality for the thorax was 70.6% and for the abdomen 40.5%. As a result of mostly combined causes (66%), 101 patients died (33.6%). Multiple organ failure was in the high range (80%), while the most frequent single cause of death was craniocerebral trauma, ranking before thoracic and abdominal traumata.

[Results of treatment following surgical management of complete acromioclavicular joint dislocation (Tossy III injury). Management using ligament sutures and direct transarticular and indirect extra-articular stabilization].

In the last 5 years 36 patients with complete akromioklavikular separation were treated in 21 cases by ligament suture and Balser-plate, in 15 cases by ligament suture and "Zuggurtung". Good and very good results were found in 85%. Patients in which the protection of the ligament suture was given by application of the Hook-plate had better results than those with "Zuggurtung". The use of the Balser-plate remarks a safe method in protecting the coracoid-clavicular ligament suture. The rate of complications is rare, compared with the patients in the other group. Functional results seem to be better in comparison to other methods of operative treatment.

[Treatment results following compound osteosyntheses. Pathological fractures and osteolyses in the extremities].

65 combined osteosyntheses were applied to 49 patients for existent or imminent pathological fractures. The most frequent reason was carcinoma of the breast in 2/3 of the cases. Surgical intervention was done palliatively non radical in 52 cases as a posttraumatic and in 13 cases as a prophylactic procedure. Surgery was well tolerated by all patients with a primary hospital mortality of 10,2%. On an average the survival time for 10 patients still living was 22 1/2 months, for 39 patients who had died 8 1/2 months. In all cases a substantial increase in quality of life was gained, both by enabling the patient to use the extremity concerned almost without pain and by greatly relieving bedside care. These results indicate that combined osteosyntheses should be applied on a broader basis.

[Combined thoraco-abdominal injuries. Diagnosis, therapy, treatment results].

81 patients with combined thoraco-abdominal lesions had at the same time craniocerebral lesions (54 cases) and (in 70 cases) injuries of the locomotor system. These 64 men and 17 women had an average age of 44.8 years. In 72.9% surgical intervention was necessary in the regions of the thorax and abdomen at the same time, whereas in 18.5% of the cases surgery was required either at the thorax or at the abdomen only. Only 8.6% of the patients could be treated conservatively. 42 patients died (51.9%), two-thirds of these from direct organ lesion and one-third from an indirect organ lesion. The most frequent cause of death was craniocerebral trauma, ranking before thoracic and abdominal trauma.

[Therapy and prognosis of 300 primary extrahepatic bile duct cancers].

A total of 300 patients (66% women, 34% men) had carcinoma of the bile ducts; 64% were located in the gallbladder and 36% in the bile ducts. In 77.3% the carcinomas had metastasized or were conglomerate tumors, while in 22.7% a curative resection was performed. In 35.3% a palliative operation or resection was indicated. An exploratory laparotomy was performed in 42%, but 75% of the patients died within 6 months. After 1 year 10.2% had survived. After curative resection 46 patients survived on the average over 17.6 months, while 12 patients survived an average of 5.1 years.

[The problem of the polytraumatized patient (author's transl].

From 1977-79 a number of 140 patients suffering from polytrauma were in need of treatment at the Berufsgenossenschaftliche Unfallklinik Tübingen, as the degree of severity by the injuries caused acute danger of life. In all patients fractures of the extremities were found in combination to injuries of the body cavities. In 87 p.c. associated traumata of the skull, in 47 p.c. of the thorax, and in 30 p.c. of the abdomen indicated a broad variation in polytraumatized patients, which became manifest in 66 patients as a double, in 58 patients as a triple and in 16 patients as a quadruple combination of severe injuries. 25 casualties had to die involving 6 p.c. og the first, 24 p.c. of the second and 44 p.c. of the third group. Any therapy has to considerate the individual pattern of systemic lesion under concern to the vital systems of brain, breathing and circulation. Operative procedure following the classification into term of vital urgency has to distinguish among operations by urgent priority, among those of secondary urgency and treatment of choice. Early osteosynthesis can help effectively avoid complicating factors.
