The effects of hyperbaric oxygen on visual functions in ischaemic optic neuropathy.

The effects of hyperbaric oxygen therapy were studied in nine patients with non-arteritic anterior ischaemic optic neuropathy, who had been unsatisfactorily treated with corticosteroids. In four patients with signs of optic disc atrophy there was no improvement of visual functions after hyperbaric oxygen treatment, whereas in the other five patients, who had no signs of optic disc atrophy, visual acuity and visual field improved. The achieved improvements were maintained at a control examination six months later. Results suggest that hyperbaric oxygen might be a new remedy for the selected cases of non-arteritic ischaemic optic neuropathy and call for a controlled clinical study aiming to accurately assess its efficacy.

Cardiovascular reflex testing and pupil cycle time in open-angle glaucoma.

The aim of the study is to assess the value of function tests of the autonomic nervous system in open-angle glaucoma by testing the cardiovascular reflexes and pupil cycle time. The experimental group consisted of 19 glaucoma subjects with controlled intraocular pressure. The control group consisted of 20 healthy volunteers. The Valsalva test has proven to be considerably accurate in the assertion of autonomic dysfunction (p < 0.05). Pupil cycle time was significantly prolonged (p < 0.001). These results confirm the role of autonomic dysfunction in open-angle glaucoma.

A prospective study on the etiology of uveitis.

We prospectively studied and classified 152 cases of endogenous uveitis treated at the Ophthalmological Clinic in Split, Yugoslavia, according to clinical presentation, several immunologic parameters including HLA subtyping, and the presence of an associated systemic disease. Using our diagnostic criteria we successfully classified 91.4% of patients. Anterior uveitis was the most common form of clinical presentation (94 patients, 61.8%), whereas an intermediate uveitis was present in only 5.3% of patients (8 patients). By HLA typing we found that 115 patients (75.7%), were positive for HLA-B27 (57 patients, 50%), B5 (30 patients, 26%) and B35 (28 patients, 24%) risk antigens. Of these patients 96 (83%) had an associated systemic illness. In the 8.6% of cases (13 cases) in which the etiology of an endogenous uveitis was not determined, none were associated with an HLA-B risk antigen. Our study emphasizes the association of HLA-B risk antigens and concurrent systemic disease in patients with endogenous uveitis.

[Etiopathogenic characteristics of posterior uveitis in children].

Nine children with acute posterior uveitis were treated for period of 3 years (3.2% of all children treated during that period). The authors speak in favor of the investigation of the HLA-system and the analysis of humoral and cellular immunologic parameters. In this study they present the relationship between these characteristics and the clinical diagnosis. Three of 9 children have with eye diseases associated HLA-B5 and HLA-B12 antigens and grave consequences for the sight of both eyes already as children. The results of immunologic tests in the examined group compared to the control group differ significantly in the total number of lymphocytes, total number of B lymphocytes and serum values of IgG, as well as C3 values of complement component (P < 0.05). Seen in percentage, however, the ratio of T to B lymphocytes, as well as the total number of leukocytes don't show significant differences. We consider that a proper and well-aimed history and complete etiologic detection according to the protocol have contributed to a clearer understanding and better insight into the prognosis of our patients.
