[Nutritional status of children treated with continuous ambulatory peritoneal dialysis and ambulatory peritoneal dialysis].

Malnutrition is a very important problem in children with ESRD on peritoneal dialysis. Examinations were conducted on 20 children (9 girls, 11 boys), 10 children were treated continuous ambulatory peritoneal dialysis (CAPD) and 10 automated peritoneal dialysis (APD). Anthropometric and serum biochemical parameters were examined. Dialysis adequacy by KT/V, SCCr, PCR were described. The most significant malnutrition was observed in youngest children with lowest BMI values and significant muscle and fat mass deficiency. Increased protein catabolic rate (PCR) in children under 6 years old was an important factor leading to malnutrition.

[Levels of selected soluble adhesion molecules in blood serum of children with chronic glomerulonephritis].

Increased concentrations of circulating soluble adhesion molecules have been reported in a variety of disorders. The aim of this study was to evaluate serum sICAM-1, sVCAM-1, sE-selectin, sP-selectin concentrations in 60 children with chronic glomerulonephritis GN (49 patients with GN and syndrome nephroticum, 11--with GN only), aged 3-17 years (mean 9 years) and in 15 healthy children (control group). The histopathological diagnoses of the 27 patients were: minimal change GN--10 cases, lupus nephritis--3, mesangiocapillary GN--4, mesangial GN--4, membranous GN--1, focal glomerulosclerosis--5. It was found a significant increase of tested soluble adhesion molecules in all children with GN compared to the control, independent on the histopathological type of GN. Serum sVCAM-1 and sP-selectin concentrations were higher in children with GN and syndrome nephroticum compared with patients with GN only. The results indicate activation of platelets, leucocytes, endothelium and its damage in GN. It seems, that more advanced vascular endothelium changes and platelets activation occur in children with GN and syndrome nephroticum.

[The role of collagen in the process of renal fibrosis].

The progression of chronic renal failure results from the process of fibrosis, which involves the proliferation of renal fibroblasts and the secretion of extracellular matrix proteins (ecm) by these cells. Collagen is the main component of ecm. Renal collagen types in normal kidney and in renal diseases were described. Mediators that stimulate fibroblast proliferation, collagen turnover and factors its regulating were discussed.

[Hyperuricosuria in children].

Hyperuricosuria (HU), defined as an increased urinary acid excretion, seems to be responsible for the of kidney stone formation. Hyperuricosuria was identified as a potential etiology of hematuria in children and adult patients too. The aim of the study was to analyze clinical course of hyperuricosuria in 77 children (43 girls and 33 boys) treated in 1995-1999. We analyzed familial history of urolithiasis, reasons of hospital admissions, laboratory findings and treatment. HU has been suspected to cause hematuria in patients. Children with higher urinary acid excretion are in increased risk of stone formation.

[Urolithiasis in children less than 4 years of age].

The aim of the study was to analyse the cases of nephrolithiasis in the youngest children from 2 months and 4 years. 30 children treated between 1955 and 1999 were included in to the study. Nephrolithiasis causes, clinical course of the disease and risk factors for the urinary stone formation were taken under consideration.

[Lupus nephritis in children].

UNLABELLED: Systemic lupus erythematosus is a very variable and pleomorphic disease. Lupus nephritis with clinical manifestation appears in 65-70% of patients, kidney morphological changes are observed in 80-100%. In this study we analysed clinical course of lupus nephritis in 15 children treated between 1989-1998 (10 girls, 5 boys, aged 9.5-16 years). The following data were considered: inherited susceptibility to the disease, the first symptoms, primary diagnosis, the symptoms at admission to clinical hospital, the time from first symptoms to diagnosis of lupus, laboratory results, clinical course, treatment, outcome of disease. All children underwent percutaneous renal biopsy. 2 patients had class II, 7 patients--class III, 5--class IV and 1 class V of lupus nephritis. In the treatment were used: oral prednisone, intravenous methylprednisolone, methylprednisolone pulse, cyclophosphamide (oral and i.v.), sandimmum. Total or partial remission with normal renal function was observed in 11 children, 1 had stable renal insufficiency, 1 with terminal renal insufficiency started regular HD treatment, 2 patients died. CONCLUSIONS: Lupus nephritis appears the main in young girls aged 12-15 years; the children with renal symptoms need of early renal biopsy and individual treatment; the prognosis of children with IV class lupus nephritis, severe anaemia, low complement, active urinary sediment and quick progress of renal insufficiency is poor.

[Acute renal failure in children with idiopathic nephrotic syndrome].

In the course of idiopathic nephrotic syndrome (INS), acute renal failure (ARF) may develop. The factors, which lead to ARF appearance are not entirely defined. In 1990-1997, 1006 children suffering from INS were admitted to our Department. ARF was noticed in 8 of them (0.8%), aged 6 to 17.5 years. The aim of study was to emphasize the clinical features of ARF and to evaluate factors that may contribute to renal failure in children with INS. The following data were considered: the course of INS (disease duration, the number of relapses, the remission periods), renal histology, degree of electrolyte disorders, lipid metabolism disorders, serum albumin level, intensification of proteinuria, presence of hypertension, applied treatment. According to the analysis, it seems that considerable hypoalbuminaemia, the presence of infection and severe course of nephrotic syndrome are the factors of ARF appearance risk in the course of INS.

[Enzymuria and urinary tract infection].

Urinary tract infection (UTI) can lead to kidney damage. The analysis of urinary enzyme activities is a sensitive, non-invasive method to recognise renal tubular lesions. The most important enzymes for kidney diagnosis, pathomechanism of urinary enzyme activity changes in the course of UTI and other factors influencing enzyme excretion are described. Interpretation of enzymuria, its clinical significance for kidney damage diagnosis and prognosis of disease are presented.

[Lipoprotein (a) in children with chronic renal failure treated conservatively].

The aim of this study was to evaluate serum lipid abnormalities, particularly lipoprotein (a), [Lp(a)] as an independent risk factor for cardiovascular disease in children with mild and moderate renal failure. Study were performed on 14 children of whom serum creatinine levels were above 265.3 mumol/l and 32 patients with serum creatinine levels below 265.3 mumol/l. Control group consisted of 27 healthy age-matched subjects. All children were tested for concentration of serum Lp(a), total cholesterol (TC), triglycerides (TG), low density lipoprotein cholesterol (C-LDL) and high density lipoprotein cholesterol (C-HDL). It was found a significantly increase in Lp(a), TC, C-LDL and significantly decrease in C-HDL in children with more advanced renal insufficiency compared to the control. In children with mild renal failure concentration of serum Lp(a) also increased but not significantly. Patients in this group had elevated serum TC and decreased C-HDL. These results suggest that even in the early stages of renal insufficiency in children abnormalities of lipoprotein are present. Such abnormalities, particularly Lp(a) might contribute to accelerated atherosclerosis in this patients.

[Perception of family by children and youth with chronic renal failure].

The perception of family by children and youth with chronic renal failure treated conservatively and with haemodialyses was studied. Intrafamilial relations were subjected to analysis. It was found that with the limitations due to the disease, family is the main environment where the psychophysical needs of the child are fulfilled and it is of decisive value for the quality of life of patients. In view of the importance ascribed to it, it may play a significant role in the accommodation of the patient to the disease, influencing the effectiveness of long-term treatment.
