Antibody to 65-kD stress protein (HSP65) of Mycobacterium leprae in various inflammatory skin diseases. A preliminary report.

Cells from prokaryotes and eukaryotes exposed to environmental changes produce a series of highly homologous proteins called stress proteins, or heat shock proteins (HSPs). Recent investigators suggested that the reaction to the shared antigenic epitope between HSPs may link infections with induction of autoimmune processes. In the present study, antibody level to HSP with 65 kDa (HSP65) of Mycobacterium leprae was investigated with enzyme-linked immunosorbent assay (ELISA) in various skin diseases. Comparing to normal group (n = 9) including patients with nevus cell nevus showing 0.097 +/- 0.039 (mean +/- SD) in anti-HSP65 IgG level (OD492), 20 patients with palmoplantar pustulosis (PPP) and 22 with psoriasis demonstrated elevated (0.170 +/- 0.079, 0.111 +/- 0.053, respectively) level. Among them patients judged as focal infection-related PPP or psoriasis showed significantly higher level of anti-HSP65 (p < 0.01) than those without focal infection. Anaphylactoid purpura (0.125 +/- 0.085, n = 5), Behcet disease (0.178 +/- 0.045), atopic dermatitis (0.218 +/- 0.096, n = 13), urticaria (0.185 +/- 0.079, n = 30), and herpes zoster (0.193 +/- 0.092, n = 13) showed likewise elevated anti-HSP65 antibody. Similar tendency was found in anti-HSP65 IgM level but not in anti-HSP65 IgA. Western blotting confirmed specific immunoreaction bands to HSP65 in blood samples with high titer. Immunomodulation by stress proteins of bacterial or host cells is assumed in pathophysiology of inflammatory skin disorders, especially in relation to focal bacterial infection as observed in cases with PPP and psoriasis.

[Crystalloids in salivary gland pleomorphic adenoma].

We examined crystalloids in salivary gland pleomorphic adenomas. The crystalloids were detected in 4 of 34 pleomorphic adenomas (11.7%). In three cases they were found in the minor salivary gland and in one case in the major salivary gland. Light microscopy revealed that all the crystalloids were in parenchyma. They were composed of eosinphilic structure and radially arranged clusters of needle-shape fibers. The central parts sometimes contained clear space. They were usually closely surrounded by neoplastic myoepithelial cells. All 4 cases containing crystalloids were million reaction-negative and positive to van Gieson, Mallory and Reticulin Silver Impregnation stains. Therefore all the crystalloids in our 4 cases were considered as collagen-rich.

[Suppression of myoclonus associated with intermittent photic stimulation (IPS) in a case of Creutzfeldt-Jacob disease: with special reference to the specificity of IPS frequency].

The present study deals with electrophysiological findings 3 months after the patient fell into an apallic state during the course of Creutzfeldt-Jacob disease (CJD), when periodic limb myoclonus synchronizing with periodic synchronous discharge (PSD) began to diminish. The findings were as follows: On 2 Hz intermittent photic stimulation (IPS), myoclonus and the diffuse bisynchronous high amplitude sharp wave showed dissociation; the myoclonus was completely suppressed, while the high amplitude sharp wave was elicited. On 3 Hz IPS, myoclonus was incompletely suppressed, corresponding to every other high amplitude sharp waves which were induced synchronous to IPS. On 1 Hz IPS, the high amplitude sharp waves were scarcely induced while neither PSD nor the myoclonus were suppressed. The frequency of the PSD was about 1.2 Hz. Based on these findings, we propose that in addition to the cortical refractory period after PSD or the giant response to photic stimulation, there must be another refractory period in the distal impulse pathway longer than the cortical one. Although the exact electrophysiological mechanism involved in the present findings remains unknown, the possible mechanism responsible for the suppression of myoclonus by IPS and the specificity of IPS frequency in the suppression could be due to competitive suppression of the impulse between periodic myoclonus and the cortical reflex myoclonus through the interaction of the two independent refractory periods in the transmission pathway of the impulse.

[Pharmacokinetic and clinical studies of cefozopran in the field of pediatrics].

Cefozopran (CZOP) was administered intravenously to 22 infants (aged 3 months to 15 years) with infections excluding suppurative meningitis in doses of 10 to 40 mg/kg 3 to 4 times daily for periods of 3 to 16 days and its efficacy and safety in infantile infections as well as pharmacokinetic parameters were determined. The half-lives of CZOP after intravenous administration at doses of 10, 20 and 40 mg/kg were 2.84, 2.36 +/- 0.67, and 2.48 hours, respectively. The rates of recovery in the urine within 6 hours of administration were 83.6%, 62.9 +/- 23.1%, and 77.3%, respectively. The subjects for whom drug responses were evaluable consisted of 1 case of suppurative meningitis, 8 cases of respiratory tract infection, 2 cases of urinary tract infection, 2 cases of oral infection, 2 cases of pharyngotonsillitis, 1 case of skin soft tissue infection, and 1 case of external otitis, totaling 17 cases. The efficacy rate was 88.2%. The drug proved so effective on suppurative meningitis in particular that the patient was healed without leaving any sequela behind. Seven strains were identified as causative pathogens (5 strains of Haemophilus influenzae, 1 strain of Staphylococcus aureus, 1 strain of group B Streptococcus) isolated from 6 patients. All but 1 strain were eradicated (the rate of eradication 83.3%). In the patients with suppurative meningitis the bacterial count of spinal fluid was markedly decreased 6 hours after drug administration and the organism was eradicated in 45 hours. Eruption was noted in 1 patient as a side effect of CZOP, but disappeared soon after discontinuation.(ABSTRACT TRUNCATED AT 250 WORDS)

Gene expression of protein phosphatases in rat ascites hepatoma cell lines.

Gene expression of protein phosphatases 1 alpha, 1 gamma 1, 2A alpha, and 2C alpha in 14 rat ascites hepatoma cell lines was studied by Northern blot hybridization. The expression of PP1 alpha and PP2C alpha was increased and decreased, respectively, in all of the ascites hepatoma (AH) cells compared to rat liver, whereas the expression of PP1 gamma 1 and PP2A alpha was increased in about 50% of them. Relative gene expression was affected by several factors, such as harvest time, transplantation rate, percentage of free cells, and sex; the first factor was more important than the others. Relative gene expression of PP1 alpha had a negative correlation with harvest time, whereas gene expression of PP1 gamma 1 and PP2A alpha had a nonlinear (hyperbolic) correlation with harvest time. We suggest that there is a relationship between growth rate and expression of protein phosphatase genes. Our data also suggest that PP1 gamma 1 mRNA is positively controlled by PP2A alpha mRNA.

Recombinant human non-glycosylated granulocyte-macrophage colony-stimulating factor in allogeneic bone marrow transplantation: double-blind placebo-controlled phase III clinical trial.

In a randomized double-blind placebo-controlled phase III clinical trial, the effects of a non-glycosylated recombinant human granulocyte-macrophage colony-stimulating factor (CSF 39-300) were investigated in patients who had received allogeneic bone marrow transplantation. CSF 39-300 was administered at a daily dose of 10 micrograms/kg (maximum dose, 300 micrograms/body) via three-hour intravenous infusions from days 1 to 21 following reinfusion of the marrow. Twenty-eight patients received CSF 39-300 and 25, placebo. The median number of days to recovery for leucocytes (> or = 1000/mm3), neutrophils (> or = 500/mm3), lymphocytes (> or = 300/mm3) and reticulocytes (> or = 20/1000) were significantly shortened (16 vs 20, 17 vs 21 and 22 vs 28, respectively) with CSF 39-300. The duration of stay in laminar-air-flow room after transplantation was also significantly shortened in the CSF 39-300 group (21 vs 30 days). There was no significant difference in the incidence of acute or chronic graft-versus-host disease between the two groups. A follow-up during the year after transplantation revealed there to be no significant difference in either survival rate or leukemia relapse rate between the two groups. CSF 39-300 is therefore considered useful after allogeneic bone marrow transplantation, especially in the early period.

Recent advances in the diagnosis and surgical treatment of patients with carcinoma of the esophagus.

Five hundred and eighty-five patients with carcinoma of the thoracic esophagus underwent resection of the esophagus in the surgical department in which we work since 1965. The postoperative 30 day mortality rate was 11.9 percent during the early period, from 1965 to 1974, whereas it has decreased to 1.3 percent in the period since 1985. The most common cause of mortality was pulmonary complications (45.8 percent). The five year survival rate of patients who underwent a curative resection between 1975 and 1984 was only 29.1 percent, whereas it has remarkably increased to 49.9 percent since 1985, at which time cervical thoracic and abdominal lymph node dissection began to be regularly performed. There were three patients with intraepithelial carcinoma and 17 with mucosal carcinoma, respectively; none of these patients died within five years postoperatively. However, the five year survival rate of 73 patients with submucosal carcinoma of the esophagus was only 52.6 percent. Thus, it is clear that the prognosis substantially worsens when carcinomatous invasion reaches the submucosal layer. From the results of this study, it can be concluded that a combination of early detection and resection of the esophagus with three field lymph node dissection offers the best chance for long term survival in patients with carcinoma of the esophagus.

[Effect of intraocular irrigating solution on flicker electroretinogram during cataract surgery in humans].

We studied the effects of intraocular irrigating solutions on the 30Hz flicker electroretinogram (ERG) during extracapsular cataract surgery in 45 eyes of 35 patients. The effects of two commercially available irrigating solutions. S-MA2 (Opeguard MA) and DE-057 (BSS Plus) were compared. After irrigation and aspiration (I/A) of the residual cortex with S-MA2, ERG amplitude increased to 111.2 +/- 5.8% of the pre-I/A amplitude. It significantly increased to 116.9 +/- 7.0% at the end of operation (p < 0.05) compared with before I/A. After I/A with DE-057, ERG amplitude increased to 109.5 +/- 5.3% of the pre-I/A amplitude. It significantly increased to 115.7 +/- 6.5% at the end of operation (p < 0.05) compared with before I/A. After I/A with S-MA2, ERG peak time was significantly prolonged to 103.9 +/- 0.8% of the pre-I/A peak time (p < 0.01). It was significantly shortened to 101.5 +/- 0.9% at the end of the operation (p < 0.001) compared with after I/A. After with DE-057, ERG peak time was significantly prolonged to 104.2 +/- 1.2% of the pre-I/A peak time (p < 0.01). It was significantly shortened to 101.3 +/- 1.22% at the end of the operation (p < 0.05). There were no statistical differences between changes of amplitude and peak time with S-MA2 and those with DE-057. We speculated that a drop in the retinal temperature during I/A in the anterior chamber and an increment of the photopic ERG amplitude during light adaptation with the operating microscope caused these ERG changes.

Vasodilator effect of adrenomedullin and calcitonin gene-related peptide receptors in rat mesenteric vascular beds.

The effect of adrenomedullin, a novel peptide of 52 amino acids, on vascular tone was investigated in the perfused rat mesenteric vascular bed. In the vasculature contracted with methoxamine, perfusion of adrenomedullin (10(-11)-10(-7) M) caused a concentration-dependent decrease in perfusion pressure due to vasodilation. Additionally, a bolus infusion of adrenomedullin (300 and 500 pmol) produced a long-lasting vasodilator response, which was not affected in the presence of atropine (10(-7) M) and propranolol (10(-7) M). However, this response was inhibited in the presence of CGRP [8-37] (10(-6) M), an antagonist for CGRP receptor. These results suggest that adrenomedullin induces nonadrenergic and noncholinergic vasodilation in which CGRP receptors may be involved.

Molecular cloning and biological activities of rat adrenomedullin, a hypotensive peptide.

Adrenomedullin is a new hypotensive peptide recently identified in human pheochromocytoma arising from adrenal medulla. We report here the cDNA encoding a rat adrenomedullin precursor. The precursor is 185 amino acids in length, including a 21 residue putative signal peptide at the N-terminus. Rat adrenomedullin consists of 50 amino acids similar to, but distinct from human adrenomedullin; 2 residues were deleted and 6 residues were substituted compared to those in the human peptide. In the proadrenomedullin N-terminal 20 peptide, whose amino acid sequence is present in adrenomedullin precursor, 3 amino acids were substituted. RNA blot analysis showed that rat adrenomedullin mRNA was expressed in adrenal glands, lung, kidney, heart, spleen, duodenum and submandibular glands. Synthetic rat adrenomedullin elicits a potent and long-lasting hypotensive activity in anesthetized rats.

Hemodynamic effects of a novel hypotensive peptide, human adrenomedullin, in rats.

The hemodynamic effects of human adrenomedullin were investigated in anesthetized Wistar rats. Intravenous administration of adrenomedullin (1.0 nmol/kg) caused a rapid and marked reduction in mean blood pressure associated with a decrease in total peripheral resistance. This reduction in mean blood pressure was closely correlated with the decrease in total peripheral resistance. These findings indicate that human adrenomedullin is a potent vasodilator and may have some role in the regulation of blood pressure.

Cloning and characterization of cDNA encoding a precursor for human adrenomedullin.

Adrenomedullin is a novel hypotensive peptide recently isolated from human pheochromocytoma. Since a high concentration of immunoreactive adrenomedullin was found in pheochromocytoma tissue, the cDNA library of pheochromocytoma was constructed, and the cDNA clone encoding an adrenomedullin precursor was isolated and sequenced. The precursor for human adrenomedullin (human preproadrenomedullin) is 185 amino acids in length, including an adrenomedullin sequence. Proadrenomedullin (proAM) contains a unique twenty amino acid sequence followed by Gly-Lys-Arg in the N-terminal region. It is possible that a novel 20 residues peptide, termed "proadrenomedullin N-terminal 20 peptide" (proAM-N20) whose carboxy terminus may be Arg-NH2, is processed from proadrenomedullin. By RNA blot analysis, human adrenomedullin mRNA was found to be highly expressed in several tissues including adrenal medulla, ventricle, lung and kidney as well as pheochromocytoma.
