Carcinosarcoma of the skin: immunohistochemical and electron microscopic observations.

A rapidly growing, hemorrhagic, exophytic tumor on the upper back of a 44-year-old male patient was investigated. Histological, immunohistochemical, and electron microscopic studies revealed both basal cell carcinoma-like and spindle cell sarcoma-like structures intermingled in the same tumor. Clinical consequences to this patient were mainly dependent on the sarcomatous element.

Selective Adhesion of Thiobacillus ferrooxidans to Pyrite.

Bacterial adhesion to mineral surfaces plays an important role not only in bacterial survival in natural ecosystems, but also in mining industry applications. Selective adhesion was investigated with Thiobacillus ferrooxidans by using four minerals, pyrite, quartz, chalcopyrite, and galena. Escherichia coli was used as a control bacterium. Contact angles were used as indicators of hydrophobicity, which was an important factor in the interaction between minerals and bacteria. The contact angle of E. coli in a 0.5% sodium chloride solution was 31 degrees , and the contact angle of T. ferrooxidans in a pH 2.0 sulfuric acid solution was 23 degrees . E. coli tended to adhere to more hydrophobic minerals by hydrophobic interaction, while T. ferrooxidans selectively adhered to iron-containing minerals, such as pyrite and chalcopyrite. Ferrous ion inhibited the selective adhesion of T. ferrooxidans to pyrite competitively, while ferric ion scarcely inhibited such adhesion. When selective adhesion was quenched by ferrous ion completely, adhesion of T. ferrooxidans was controlled by hydrophilic interactions. Adhesion of E. coli to pyrite exhibited a liner relationship on langmuir isotherm plots, but adhesion of T. ferrooxidans did not. T. ferrooxidans recognized the reduced iron in minerals and selectively adhered to pyrite and chalcopyrite by a strong interaction other than the physical interaction.

Protective effect of R(-)-1-(benzo[b]thiophen-5-yl)- 2-[2-(N,N-diethylamino)ethoxy]ethanol hydrochloride (T-588), a novel cerebral activator, against experimental cerebral anoxia.

Effects of R(-)-1-(benzo[b]thiophen-5-yl)-2-[2- (N,N-diethylamino)ethoxy]ethanol hydrochloride (T-588) on normobaric hypoxia, histotoxic anoxia by KCN and complete ischemia by decapitation were investigated in mice. T-588 (30-100 mg/kg, p.o.) showed a significant and dose-dependent prolongation of the survival time in all of the models studied. Bifemelane (100-300 mg/kg, p.o.) was also protective against all the models. Tacrine was protective against hypoxia but had no effect on anoxia and ischemia. Imipramine was protective against anoxia, but shortened the survival time of hypoxic mice. It had no effect on ischemia. The anti-hypoxic effect of T-588 was completely inhibited by pretreatment with scopolamine (1 mg/kg, i.p.), while the anti-anoxic effect was partially inhibited. Its effect on the ischemia was not affected by scopolamine. Hypoxia decreased the cerebral contents of ATP, phosphocreatine and glucose and increased the contents of lactate in mice. T-588 had no effect on these changes. Bifemelane prolonged pentobarbital-induced sleeping time in mice with the doses inducing anti-anoxic action, but T-588 did not. These results suggest that the activation of the CNS cholinergic system is involved as one of the mechanisms for the anti-anoxic action of T-588.

Identification of the soybean allergenic protein, Gly m Bd 30K, with the soybean seed 34-kDa oil-body-associated protein.

The soybean allergenic protein, Gly m Bd 30K [Ogawa et al., J. Nutr. Sci. Vitaminol., 37, 555-565 (1991)] which is most strongly and frequently recognized by the IgE antibodies in sera of soybean-sensitive patients with atopic dermatitis, has been characterized. The allergen was isolated from the crude 7S-globulin fraction as an oligomeric form with a molecular weight of more than 3000,000 by gel-filtration chromatography. On two-dimensional gel electrophoresis, the native oligomeric allergen had an isoelectric point of about pH 4.5 and was dissociated into a monomeric form with a molecular weight of about 32,000 by the treatment with sodium dodecyl sulfate and 2-mercaptoethanol. The monomeric allergen had an N-terminal amino acid sequence and amino acid composition identical with those of the soybean seed 34-kDa oil-body-associated protein or the soybean vacuolar protein P34 with close homology to papain-like thiol proteinases [Kalinski et al., J. Biol. Chem., 267, 12068 (1992)]. The identity was further confirmed by the immunological cross-reactivity to the antibodies produced against each of the purified allargen and the 34-kDa oil-body-associated protein. By this observation, Gly m Bd 30K was shown to have about 30% sequence homology with Der pI, a house dust mite allergen that is a thiol proteinase from Dermatophagoides pteronyssius.

Enhanced lymphatic delivery of monoclonal antibody following OK432 pretreatment.

The effect of OK432 on the lymphatic delivery of monoclonal antibody (Mab) was investigated by injecting [125I]-Mab A7 into BALB/c mice pre-treated with OK432, and measuring the radioactivity in the regional lymph nodes. The [125I]-Mab A7, when administered subcutaneously to the foot pad, preferentially accumulated in the ipsilateral popliteal and para-aortic lymph nodes. Treatment with OK432 prior to antibody injection significantly enhanced the accumulation of Mab A7 in these regional lymph nodes. These findings suggest that pretreatment with OK432 may constitute a promising method of enhancing the lymphatic delivery of Mab, and improving the efficacy of therapy directed against lymphatic malignancies.

Two cases of thiamine deficiency-induced lactic acidosis during total parenteral nutrition.

Two cases of severe lactic acidosis induced by total parenteral nutrition (TPN) are reported. Both cases were admitted to our department for the surgical treatment of advanced gastric cancer, and subsequently underwent TPN because of poor nutritional status. Following the initiation of TPN, both patients went into an unstable circulatory state following shock. Both cases showed signs of peritonitis, suggestive of an intraabdominal abscess, and subsequently underwent an emergency laparotomy to explore the origin of the lactic acidosis. There was, however, no apparent infectious focus which would lead to severe metabolic acidosis in the abdominal cavity. One case died of irreversible shock. The other case similarly exhibited a deteriorated cardiovascular state but promptly responded to the administration of thiamine and was resuscitated. The blood thiamine level was low in both individuals, and the two patients were subsequently diagnosed as having thiamine deficiency-induced lactic acidosis.

Pheochromocytoma associated with nocturnal hypertension.

Pheochromocytoma is often associated with paroxysmal hypertension. We report a 49-year-old woman with pheochromocytoma whose blood pressure (BP) was elevated regularly only at night. Plasma norepinephrine (NE) and neuropeptide Y (NPY) concentrations increased in parallel with the elevation of BP. After resection of the adrenal tumor, these circadian changes disappeared. Plasma NE and NPY, especially the former, from the tumor were considered to be the cause of this unusual fluctuation in BP.

Irreversible suppression of CD8 expression in CD4-CD8+ thymocytes upon in vitro stimulation.

CD8 (Ly-2) expression was suppressed in purified murine CD4-CD8+ thymocytes at the mRNA level upon continuous stimulation with PMA and ionomycin in the presence of rIL-2. The level of CD8 expression on CD4-CD8+ thymocytes was reduced gradually during the culture and a majority of them turned into CD4-CD8- cells after 48 hr. This suppression was not transient, since CD8 expression was not recovered on these cells in additional 48 hr of culture without PMA and ionomycin. The suppression was dependent on the concentrations of PMA and ionomycin, and inhibited by adding an immunosuppressant, CsA to the culture. Treatment with either PMA or ionomycin alone did not induce suppression of CD8. Crosslinking of CD3-epsilon chains also induced suppression of CD8 for a part of CD4-CD8+ thymocytes. Interestingly, CD8 expression was hardly suppressed in CD4-CD8+ peripheral T lymphocytes, suggesting that the mechanisms of suppression of CD8 is developmentally regulated. We propose that the suppression of CD8 expression at CD4-CD8+ stage involves an additional mechanism of negative selection of thymic T cells.

Hyperthermo-chemo-radiotherapy for patients with early carcinoma of the esophagus.

Hyperthermia combined with irradiation and chemotherapy (hyperthermo-chemo-radiotherapy) was prescribed pre-operatively for 21 patients with submucosal carcinoma of the esophagus, and its effectiveness was evaluated by a comparison with patients who also had massive submucosal carcinoma, but received no pre-operative treatment (n = 12). There was no difference in the survival rates of these groups (hyperthermo-chemo-radiotherapy group: 3-year survival in 56.6%), but the patients who responded well to the HCR therapy (n = 6) demonstrated better survival rates (3-year survival in 83.3%) than did the patients without any pre-operative treatment (3-year survival in 33.9%). An HCR sensitivity test using the succinate dehydrogenase inhibition test was devised and showed good correlation with the clinical remedial values (the diagnostic specificity was 0.89). An accurate diagnosis of the esophageal lesion and appropriate selection of potentially treatable cases are considered essential for the optimal use of HCR therapy in patients with submucosal carcinoma.

[Effects of granulocyte-colony stimulating factor on hematogram examinations].

Effects of granulocyte-colony stimulating factor (G-CSF) on hemograms were studied in the leukopenic phase after chemotherapy on patients with solid malignant tumors and lymphocytic malignancies. The samples were from patients with solid malignant tumors (solid tumor group) and from those with lymphocytic malignancies (lymphocytic malignancy group). In the control group, the increase rate and the decrease rate of the white blood cell count were 1.48 and 0.67, respectively. With the administration of G-CSF, they were 4.99 and 0.29 in the malignant tumor group, and 11.83 and 0.19 changes beyond the range of physiologic fluctuations with the administration of G-CSF. In term of increasing rate, immature myeloid cells were increased more remarkably than mature neutrophil by administration of G-CSF, indicating that the target cells of G-CSF are immature cells, and the increase of immature cells is responsible for the increase of the whole white blood cell count. Though immature myeloid cells with atypical morphology were rarely detected in the solid tumor group, they were observed more frequently in the lymphocytic malignancy group. In this study, we stress that the information about the history of G-CSF administration is indispensable for quality control of hemogram examinations.

[Granulocyte-colony stimulating factor and clinical laboratory].

The effects of granulocyte-colony stimulating factor (G-CSF) on the white blood cell count and the morphology of peripheral white blood cells were studied during the period of leukocytopenia following chemotherapy in patients with non-hematological malignant tumors (solid tumor group) and those with lymphatic hematological diseases (lymphocytic malignancy group). Cell surface markers were also analyzed to evaluate the usefulness of G-CSF in peripheral stem cell transplantation. After G-CSF administration, the white blood cell count showed remarkable changes beyond the physiologic range; the increase and decrease were 5.0 times and 0.3 times previous values, respectively, in the solid tumor group, and 11.0 times and 0.2 times, respectively, in the lymphocytic malignancy group. Concerning the morphology of peripheral white blood cells, immature leukocytes increased markedly to 133.0 times the previous value. The frequency of immature cells with morphological abnormalities was 0.5 percent in the solid tumor group, but 1.6 percent in the lymphocytic malignancy group. These findings suggest that administering G-CSF affects precision control and microscopic findings in routine laboratory blood tests and that information about whether the patient was treated with G-CSF should be obtained from the clinical staff. A cell surface marker, c-kit, was analyzed to clarify whether stem cells useful for peripheral stem cell transplantation appear in peripheral blood after administration of G-CSF. The incidence of c-kit-positive cells was at 1-10% in 3 of 5 patients in the solid tumor group administered G-CSF and in all 6 patients in the lymphocytic malignancy group.(ABSTRACT TRUNCATED AT 250 WORDS)

[A new application of immunoconjugate to reduce the local recurrence of colorectal cancer].

To examine whether or not immunoconjugate-A7-NCS can contribute for the reduction of local recurrence of colorectal cancer, the present study was undertaken. The study included examination of local retension, lymphatic delivery and inhibitory effect on tumor development after local administration of A7-NCS. The result showed that locally injected A7-NCS showed a high local retension, a high regional lymph node accumulation and a high inhibitory effect on tumor development. This finding indicates that A7-NCS can be a new promising tool for reducing the local recurrence of colorectal cancer.
