[A case report of primary double cancer of the stomach and choledochus].

We encountered a relatively rare case of primary double cancer arising simultaneously from both the stomach and choledochus in a 72-year-old woman with obstructive jaundice. The patient was admitted to Saiseikai Shigaken Hospital, and was diagnosed as having both early gastric cancer at the lesser curvature of the antrum and cancer of the choledochus. She underwent curative pancreato-duodenectomy with extended gastrectomy. One of the resected tumors was histopathologically diagnosed as a well differentiated, tubular adenocarcinoma of the stomach with invasion reaching as deep as the submucosa. The other tumor was a moderately differentiated, tubular adenocarcinoma of the choledochus with lymph node metastases (hepatic, retroligamentic, paracholedochal, and posterior and superior pancreaticoduodenal lymph nodes). Our review of the literature suggests that the incidence of primary double malignancies is on the increase.

[Clinico-pathological features of intracranial germ cell tumors with dissemination or metastasis in childhood].

Recently reports of the cases with intracranial germ cell tumor associated with dissemination to the central nervous system are increased since the introduction of CT scan. The authors experienced 8 cases with the dissemination among 19 cases of intracranial germ cell tumor in childhood, and discussed their clinico-pathological features. Before the CT scan era, the authors experienced only one case with dissemination, who was a 11 years-old boy (Case 1) and revealed two cell pattern pinealoma disseminating on the subependymal layer of all ventricles and in the basal cisterns at autopsy. Since the introduction of CT scan, 4 cases were found, who showed periventricular dissemination already at the first admission. Among them three cases revealed tumors in the pineal or suprasellar region and also dissemination of tumors on the wall of the lateral ventricles. The other one (Case 3) did not reveal any tumor at pineal or suprasellar region, but disseminated tumors on the wall of the third and lateral ventricles. Histological appearances of all 4 cases were two cell pattern pinealoma by examination of surgical or CSF materials. Only one case (Case 5) was found spinal dissemination later. All 4 cases were good prognosis. The authors considered that there might be diffuse primary ependymal germ cell tumor besides diffuse metastatic one. In the CT scan era, there were three cases who showed dissemination or metastasis along their clinical courses, but had not showed at the first administration. One (Case 6) of 3 cases showed a tumor at the right thalamus at the first administration. Five years later, the tumor metastasized to the cervical cord and he died.(ABSTRACT TRUNCATED AT 250 WORDS)

[An epidemiologic study on low-birth-weight babies].

A case-control study was made in Gunma Prefecture of 1,390 mothers of babies born weighing 2,500 grams or less and an equal number of mothers of 3,000-up to-4,000 gram babies matched by place and month of birth. A correlation was found between low-birth-weight babies and maternal age, stature, menstrual history and past history. The mother's occupation, educational career, smoking habits, amount of sleep each day, date of issue of the Mother's Handbook and the number of the periodical health examinations received can be listed as socio-medical factors. Bleeding and lower abdominal pain during pregnancy, anemia and toxemia of pregnancy are found as prenatal factors. Low-birth-weight babies are found to be correlated with multiple pregnancy, breech presentation, placenta previa and premature separation of the placenta, also.

Microvascular abnormalities in ethylnitrosourea (ENU)-induced rat brain tumors: structural basis for altered blood-brain barrier function.

The fine structure, histometric characteristics, and permeability of microvessels were studied by electron microscopy in normal and in ethylnitrosourea (ENU)-induced glioma tissue from rats, using horseradish peroxidase (HRP) as a tracer. The tumor vessels were classified into (1) capillary buds (Type I); (2) round small to large capillaries (Type II); (3) sinusoidal or venule-like microvessels (Type III), and (4) abnormal arteriole-like microvessels (Type IV). All endothelial cells, basement membranes and periendothelial cells in the tumor tissue demonstrated changes in structure. The most striking alterations occurred in the endothelial cells; there were abnormal endothelial tight junctions, altered pinocytotic activity, and thickening. In the tracer study, the reaction product of HRP was present around some sinusoidal or venule-like microvessels (Type III) and extended to the widened extracellular spaces around the microvessels. The endothelial cells of Type III microvessels showed decreased nuclear and mitochondrial fractions, and increased euchromatin content and a rough endoplasmic reticulum fraction. The pinocytotic vesicles with the HRP reaction product in the endothelial cells were not increased in number. Fenestrations and gaps of the endothelial cells were observed. These alterations of the endothelial cells of sinusoidal or venule-like microvessels (Type III) are considered to be the main cause of breakdown of the blood-brain barrier in this tumor.

Kinetics of d-tubocurarine disposition and pharmacologic response in rats.

After bolus intravenous dosing of d-tubocurarine (d-TC) to rats, the twitch heights of the tibialis anterior muscle indirectly stimulated were followed, and its decrease was defined as pharmacologic response of d-TC. The relation between dose and response intensity was found to be well described with Hill's equation. According to a theory proposed by Smolen, Hill's equation was also applicable to the biophase d-TC concentration-response relation; the time courses of the relative biophase d-TC concentration indicated linear kinetics with dose levels less than or equal to 0.15 mg/kg and the occurrence of dose-dependent disposition with 0.30 mg/kg. After bolus i.v. dosing of 3H-d-TC, plasma d-TC concentration obeyed a dose-independent two compartment model with doses less than or equal to 0.15 mg/kg, but not with 0.30 mg/kg. This finding matched the above estimated with pharmacologic data. The active metabolite was not found in plasma and urine. The extent of d-TC plasma protein binding was independent of the wide range of plasma levels and its mean (+/- SD) value was 30.5 (+/- 3.8). Plasma d-TC levels and pharmacologic response intensity were well correlated by Hill's equation and a three compartment model (the general two and the biophase compartments) in the dose range less than or equal to 0.15 mg/kg.

Plasma lipids and apolipoproteins as discriminators for presence and severity of angiographically defined coronary artery disease.

The relationship of coronary artery disease to with plasma lipids and apolipoproteins was examined in 100 male patients aged 32-69 years undergoing coronary angiography for suspected myocardial infarction. Patients with angiographically defined coronary artery disease had significantly lower plasma levels of high density lipoprotein (HDL) cholesterol, apolipoproteins A-I and A-II, and significantly higher values of low density lipoprotein (LDL) cholesterol and apolipoprotein B than those in patients without coronary artery disease. The ratios derived from the measurements as LDL cholesterol/HDL cholesterol, apo B/apo A-I and apo B/apo A-II were highly significantly increased in the patients with coronary artery disease. Coronary score values, by which the severity of coronary artery disease was quantified, were not related to plasma levels of the HDL components, while they were positively correlated with those of the LDL components. These results suggest that, in single measurements, plasma levels of the HDL components, HDL-cholesterol and apo A-I, contribute strongly to the discrimination between patients with coronary artery disease and those without this disease, whereas the LDL components, LDL-cholesterol and apo B are more suitable parameters for the severity of the disease than are the HDL components. The ratios of LDL cholesterol/HDL cholesterol, apo B/apo A-I and apo B/apo A-II were powerful discriminators for either presence or severity of coronary artery disease.

The restorative action of coenzyme Q10 in 2,4-dinitrophenol-depressed electrical and contractile activities of guinea-pig heart.

With electrophysiological methods, mechanisms of the restorative action of coenzyme Q10 (CoQ10) in 2,4-dinitrophenol-depressed electrical and mechanical activities of isolated guinea-pig hearts were studied. Isoproterenol (3 X 10(-8) M)-induced action potential and contraction of the heart in 27 mM KCl Tyrode solution were abolished by 6 X 10(-6) M 2,4-dinitrophenol, while the additional application of CoQ10 (50 micrograms/ml) restored both these activities of the heart. This restorative action of CoQ10 was dose related (2 to 50 micrograms/ml) and was inhibited by 10 micrograms/ml 15-hydroperoxyarachidonic acid (15-HPAA) or pretreatment of the animals with indomethacin (5 mg/kg i.v.). When exogenously applied, 200 ng/ml prostacyclin also restored the 2,4-dinitrophenol-depressed electrical and contractile activities of the heart, but this restorative action of prostacyclin was affected neither by 15-HPAA nor by indomethacin pretreatment. These results suggest the possible participation of prostacyclin in the restorative action of CoQ10 in 2,4-dinitrophenol-depressed electrical and mechanical activities of the heart.

Supernumerary lactate dehydrogenase isozymes in human gliomas.

Supernumerary bands in agarose gel electrophoresis of lactate dehydrogenase (LDH) were frequently observed in extracts of human gliomas. The supernumerary fractions which migrated cathodic to LDH-2 and/or cathodic to LDH-3 were designated LDH-2' and LDH-3'. These extra bands were clearly seen in certain gliomas and less distinctly in others, being more frequent in primitive or undifferentiated tumours. The extra bands seen in gliomas differ from the LDH-X of the testis, but LDH-2' seemed correspond to LDH-Z in placenta, hydatidiform mole, and choriocarcinoma. These sub-bands are interpreted as being produced by gliomas and as oncofetal enzymes.

Cavernous hemangioma in the pineal region.

A cavernous hemangioma in the pineal region of a 22-year-old man is reported. Because the patient had diabetes insipidus and a contrast-medium-enhanced mass in his pineal region on computed tomography, a germinoma was suspected. A ventricular shunt was made and the tumor was irradiated, but without a response. After irradiation the tumor was entirely removed, and postoperatively the diabetes insipidus improved.

Suppressor cells in levamisole-treated mice: a possible role of T-cell-mediated feedback suppression in the drug-induced suppression of the humoral immune response.

The suppressive effect of levamisole (LMS) on the primary IgM plaque-forming cell (PFC) response to sheep erythrocytes in mice was studied. The suppressive effect of LMS on day 4 PFC response was found to be modified by dose, the timing of drug administration, and the amount of antigen injected. The experiments, using an inhibitory dose of 10 mg/kg LMS, showed that: 1) T cells play an essential role in LMS-induced suppression; 2) antigen-specific suppressor cells are induced in the spleen of LMS-treated mice; 3) the spleen cells from LMS-treated donors either suppress or enhance the PFC response in the recipients, depending on the amount of antigen injected and the number of spleen cells transferred; and 4) a dose of 10 mg/kg LMS, which is inhibitory when assessed on day 4 PFC response, actually enhances the response three and a half days after the immunization. These results suggest that LMS induces precursors of both helper and suppressor cells, and their differentiation and/or maturation to LMS-primed antigen-specific suppressor T cells are modulated, at least in part, by T-cell-mediated feedback suppression.
