The authors report a case of cystic meningioma in a 10-month-old boy diagnosed by metrizamide ventriculography and computerized tomography. Intracranial meningioma in infants under 1 year old is extremely rare. This case is only the 16th case reported in the world literature; the other 15 cases are reviewed.

This report is concerned with a 55-day-old female hospitalized because of intraventricular hemorrhage from a tumor in the pineal region. Examination of serum alpha fetoprotein showed 7400 ng/ml. The patient expired because of frequent intraventricular hemorrhage and progressive hydrocephalus. Autopsy demonstrated the tumor was ependymoblastoma. No teratomatous component was noticed in the histological examination. High serum alpha fetoprotein in relation to tumor histology was briefly discussed.
Tumor tissues of glioblastoma multiforme, astrocytoma and medulloblastoma, maintained up to 21--28 days by gelfoam organ culture technique, were examined by scanning electron microscopy (SEM). Glioblastoma multiform has irregular cell surface and many cytoplasmic folds. Astrocytoma has many fibrils. The fibrils have smooth surface and are coiling. Fibrils of piloid astrocytoma are smooth and cylindrical. The focal thickness of fibrils are associated with so-called Rosenthal fiber. Capillary of astrocytoma has irregular surface and marked tortuosity. Medulloblastoma is composed of non-fibrillated round tumor cells. The tumor cells touch each other with short cell processes. These findings seemed to correspond to the malignancy of original tumor. Comparative observation of medulloblastoma maintained by monolayer cell culture with one maintained by organ culture, using light microscopy and scanning electron microscopy, was done. And medulloblastoma in monolayer culture was proliferated to two types of cells. One is epitheloid cell with taper cell processes, and the other is stellate cell with fine processes. In most organ culture, feature of cells corresponded to those observed in original surgical material by light microscopy.
Using RIA technique, response patterns of ACTH-adrenocortical system to short term large dose corticosteroid therapy for neurosurgical patients were investigated. The subjects were consisted of 20 males and 13 females, ageing ranging from 10 to 60 years old. The subjects were divided into three groups according to mode of steroid administration. In the first group, 12 mg/day of betamethasone were administered for 5 days and cancelled without gradual reduction. In the second group, 12 mg/day of betamethasone were administered for 4 days and 6 mg/day of betamethasone were given for following 2 days. In the third group, 12 mg/day of betamethasone were administered for 3 days and gradual reduction of betamethasone was given for following 4-5 days. Response of plasma cortisol level to synthetic 18-ACTH were measured in all patients for 3 days after betamethasone administration cancelled. The results led to the following conclusion. 1) The second and third groups had not any clinical symptom after steroid cancelled, but one half cases of the first group had mild headache, nausea for 2 days after cancelled. 2) Recovery process of adrenocortical function in the second group is most rapidly, compared with that in other two groups. 3) In the postoperative steroid therapy in neurosurgery, it could be concluded that 4 days massive administration of steroid followed by stepwise reduction in 2 days would be functionally well tolerated by adrenocortical system.

A case of solitary plasmacytoma of the skull base growing into the cerebellopontine angle is reported. Myeloma protein of the lambda type light chain was identified in the patient's cerebrospinal fluid. The difficulty in differentiating solitary plasmacytomas of the skull base from other tumors in this location, such as chordoma and meningioma, is discussed.
In 168 patients with ruptured intracranial aneurysms, the pathology of intracranial hemorrhage visualized on CT was analyzed. Blood in the subarachnoid space could be visualized in 95% of cases within three days after SAH and 75% of 106 cases within two weeks after SAH. In one case blood clot in the subarachnoid space visible up to 13 days after SAH. Concerning the cases within two weeks after the bleeding, intracerebral hematomas were observed in 36% of anterior cerebral aneurysms and middle cerebral aneurysms, 16% of internal carotid aneurysms and none of vetebro-basilar aneurysms. The incidence of the intraventricular hemorrhage was as follows; vertebro-basilar, 44%; anterior cerebral, 38%; internal carotid, 28%; middle cerebral, 12%. On the basis of the pattern of distribution of extravasated blood the location of the ruptured aneurysm was properly predicted in 58% of anterior cerebral, 81% of middle cerebral, 58% of internal carotid and 30% of vertebro-basilar. Especially CT could contribute to predict which aneurysm has ruptured in patients with multiple aneurysms. It was possible to localize the site of bleeding in 11 out of 12 CT positive cases. The development of intracranial hemorrhage demonstrated by CT well correlated with the clinical grading of the patients and the clinical outcome. Patients merely showing subarachnoid hemorrhage were more likely to have good neurological grades, but ones showing complicated intracerebral hematomas and intraventricular hemorrhage had poor neurological grades at the time of the scan. The findings of extensive subarachnoid clot, which were followed by severe vasospasm, and marked intraventricular hemorrhage, usually correlated with poor prognosis. These pathology recognizable on CT was very helpful in determination of the timing of surgery and management of such patients. In conclusion CT is of great value in the examination of SAH when performed in the acute stage and should be the initial examination followed by angiography.
A hugh intracranial angioblastic meningioma responded well to preoperative irradiation. The indication for preoperative irradiation of hypervascular and inaccessible meningiomas is discussed.
A polypectomy snare has been used for the transurethral resection of bladder tumors. The method is easy and resection of a main tumor en bloc is rapid in cases with a 3 cm. or less bladder tumor and tumors that are difficult to resect with customary methods because of location.

Changes in lectin activity during development of embryonic chick skin were studied. In the dorsal skin of the chick embryo in which feathers were formed, lectin activity first increased, during the period of dermal condensation, and then it decreased during the development of feathers. A similar change in lectin activity was also found in the anterior shank skin, the prospective scale region of the chick embryo. The embryonic cornea, in which no mesenchymal condensation took place, had lectin activity and did not show any developmental changes in lectin activity. Apteria regions of the dorsal skin, experimentally formed by treatment with hydrocortisone, gave low lectin activity. The lectin found in the embryonic skin showed specificity for lactose. The relationship found between lectin activity and dermal condensation in the embryonic chick skin is discussed.

A case of paraventricular cerebral cyst was reported. A 48-day-old infant was admitted to the Department of Neurosurgery with an enlargement of head circumference. Cerebral angiography, pneumoventriculography, and computed tomography showed a cyst in the trigon of the right lateral ventricle and unilateral hydrocephalus. Right-parieto-occipital craniotomy was done. The trigone of the right lateral ventricle was opened and the wall of the cyst was subtotally removed. A partial defect of the corpus callosum and other additional cysts at the quadrigeminal region were identified. On light microscopy, the inner layer of the cyst, protruding into the lateral ventricle, consisted of columnar cells and outer part of the wall was formed of loose connective tissue and glial tissue. On electron microscopy, the cells of the cyst wall was formed of loose connective tissue and glial tissue. On electron microscopy, the cells of the cyst wall had microvilli and abnormal cilia, and desmosome was recognized. And the cells of the cyst were bordered by a continuous basement membrane. According to pathological finding, the cyst was considered to be classified as a transitional type between chroidal epithelial cyst and ependymal cyst.
