[Removal of melanocytic nevi as a prophylaxis and early recognition of cutaneous and mucous malignant melanoma].

In article 1240 melanocytic nevi were described in which 26 melanomas were found. The authors present necessity of early removal (excision) of dysplastic melanocytic nevi, pigmented lesions with high risk of malignant transformation, giant congenital melanocytic nevi before adolescence and nevi which are irritated constantly. Particularly dysplastic nevi and pigmented lesions with high risk often contain melanoma "in situ" or invasive melanoma. Such a procedure early done, prevents developing of tumor and makes possible early recognition of melanoma but the procedure depends on a conscience of patients, members of their families, nurses and doctors.

[Breast dysplasia in women].

The current knowledge concerning the fibrocystic disease of the breast is presented in this work. The authors outlined the theoretical principles of the problem and the most important diagnostic methods as well as the prophylaxis and prognostic in the fibrocystic disease of the breast.

[Risk factors in breast cancer].

This assay analyses the risk factors which have great importance in breast cancer pathogenesis. Having excluded the group of higher risk in women's population we can early detect this disease and treat it as well. It may extremely improve the results of breast cancer treatment.

[Diagnostic value of mucin-like associated antigen (MCA) in neoplasms of the mammary gland].

The aim of the present study was to evaluate the diagnostic value of the new tumour marker MCA in serum from women with breast cancer. Relationship between presence MCA in serum and cER, cPgR, nER, nPgR in aqueous extracts from cancer tissues was subject of our investigation, too. Immunoenzymatic method (Roche) which employed monoclonal antibody MAb b-12 was used for determination of MCA in serum 49 operable patients (preoperative determination). In a group of 26 cancer patients MCA and cER, cPgR, nER, nPgR were tested simultaneously. Steroid hormone receptors were measured by immunoenzymatic method (Abbott). Mean concentration of MCA in women with breast cancer (n = 36) was higher as compared with control (n = 21, p < 0.001). When 6.86 U/ml was took as mean level of MCA in control group it was found that 46% women with benign tumours, 64% with carcinoma ductal and 62% with carcinoma lobular have had concentrations of MCA below this value. Also it was found that differences between mean levels of MCA and cER, cPgR, nER, nPgR were statistical significance only in women with carcinoma ductal. On the basis of our own investigations we can suggest that MCA has limited value in the diagnosis of breast cancer.

[Paget's cancer of the breast, clinical problem].

From October 1977 to October 1992 eighteen patients with Paget's cancer of the breast underwent surgical treatment in the Surgery Department, Oncological Clinic in Poznań. Mean age of patients was 60.4 years, mean duration of symptoms 12.6 month, mean time of dermatological treatment-7.2 month, more often we observed Paget's disease without palpable masses in the breast (T0)-72.2%, axillary lymph nodes free of metastases in 55.5% cases. Only one patient died because of breast cancer during follow up. None of radical operated patients developed cancer recurrence or distant metastases. Prolonged dermatological treatment delay the effective oncological therapy.

[Surgical procedure in an extensive un-operable decaying breast cancer].

The article indicates necessity of palliative surgical treatment of ulcerating and decaying breast cancers. Ulceration and decay of tumour of the breast make worse the quality of life of the afflicted. The operation like simple mastectomy liquidates the threat of haemorrhage and prevent intoxication from decaying tumour. In the period from 1977 to 1991 year the group of 1075 women with breast cancer were operated. In this number there were 44 women with decaying breast cancer. Sometimes there are difficulties in closing of the operative wound, when women with very extensive tumour ulceration are treated and then reconstructive methods are needed (transposition of a simple skin flap from the opposite breast, a latissimus dorsi myocutaneous flap or omental transposition from the abdomen to the chest wall). All these operated women survived the operation and left surgical ward.

[Prognostic factors in breast cancer].

This treatise dealt with a particular prognosis factors analysis carried out on a large group of breast cancer patients. Creating analysis is the main task to establish both the way of adjuvant treatment and the treatment results foresight.

[Evaluation of an immunoenzymatic assay for determining estrogen and progesterone receptors in breast tumors].

The aim of our research was the usability of immunoenzymatic assay for the determination of estrogen and progesterone receptors. We tested 59 women with breast tumours. 20 of these women have had carcinoma ductal, 10 carcinoma lobular and 3 women another histopathological type. 26 women have had benign tumours: 15 mastopathia fibrosocystica, 11 fibroadenoma. It was found that mean concentration of cER, nER and cPgR, nPgR were higher in malignant tumours as compared with benign tumours. Mean concentration of cER in carcinoma ductale (mean = 45.96 fmol/mg protein) and carcinoma lobular (mean = 130.38 fmol/mg protein) were of higher statistical significance than the levels of these receptors in mastopathia fibroso-cystica (mean = 16.82 fmol/mg protein). Mean concentrations nER and nPgR in benign and malignant tumours were not statistically significant. On the basis of our own investigations we can suggest that immunoenzymatic method supplied by Abbott is a simple test to perform and useful to evaluate hormonal dependence of cancer tissue.

[Thyroid carcinoma from material of the surgery department at the oncology clinic, Medical Academy in Poznan].

From among 612 cases of various kinds of thyroid diseases operated in the Surgical Department of the Oncological Clinic, we present here 24 cases (3.92%) of thyroid carcinoma. They are divided according to sex, staged to TNM system and histopathologic diagnosis. Discussed were the range of performed operations which depended on the results of histopathologic examinations. They were compared with the current principles of thyroid carcinoma treatment. Owing to the significance of inter-operational examination, in order to establish the range of the operation, the authors suggest that necessity of surgical treatment in cases of thyroid carcinoma, in centers which have at their disposal a histopathological laboratory.

[Concentration of selected sex hormones and SHBG in the serum of women with breast cancer treated with antiestrogens and (or) cytostatic agents].

The concentration of sex hormones prolactin, LH, FSH, estradiol, testosterone and SHGB has been estimated in 34 premenopausal women with breast cancer, including 10 treated by radiologic castration, 22 treated with cytostatics, 26 treated with tamoxifen and 14 treated with tamoxifen and cytostatics. Concentration of sex hormones has been estimated applying radioimmunological methods, and of SHBG with filtration method using DEAE cellulose filters. It has been found that chemotherapy of breast cancer of premenopausal women results in decrease of concentration of estradiol, testosterone, prolactin and SHGB down to the values approaching postcastration values and the application of cytostatics and of tamoxifen in postmenopausal women is not resulting in the decrease of concentration of these hormones and SHGB to the degree justifying a routine application of these drugs simultaneously.
