Experimental studies on carcinogenesis in anomalous arrangement of the pancreaticobiliary ducts.

An experimental model of anomalous arrangement of the pancreaticobiliary ducts (APBD) was produced by pancreaticocholecystostomy performed in 29 mongrel dogs. Our purpose was to study carcinogenesis of the extrahepatic biliary tract by DNA ploidy analysis with cytofluorometry. The amylase level in the bile was elevated in all 25 dogs tested except the controls. The phospholipase A2 level in the bile was elevated in all selected dogs except the controls. Common bile duct (CBD) dilatation was found in 23/29 (79%) of the dogs, and biliary stones were found in 3/29 (10%) dogs. Inflammatory changes were observed microscopically in all specimens except those from the controls. Intramural glandular structures were found in 17/25 (68%) of gallbladder (GB) specimens and 10/25 (40%) of CBD specimens; goblet cells were found in 7/25 (28%) of GB specimens and 2/25 (8%) of CBD specimens. In the controls neither glandular structures nor goblet cells were observed except for two GB specimens showing mild cholecystitis. Cytofluorometry showed 21% GB and 7% CBD diploidy, 69% GB and 65% CBD low ploidy, 10% GB and 28% CBD high ploidy patterns of histograms. These results show that, APBD may be central to high risk condition or play a key role to develop atypical biliary tract epithelium and DNA ploidy abnormality with or without biliary duct dilatation.

Comparison of cytofluorometric nuclear DNA ploidy analysis on fresh and paraffin embedded specimen.

The cytofluorometric nuclear DNA ploidy analysis was performed in gallbladder (GB) epithelium of eight patients of chronic cholecystitis to make a comparison between fresh and paraffin embedded tissue analysis, to make an ideal single cellular suspension, to evaluate the ploidy pattern affected by inflammation and to assess the histological correlations. Six (75%) fresh specimens and eight (100%) paraffin embedded specimens showed diploidy pattern of histogram. Two (25%) fresh specimens showed low ploidy pattern of histogram. There was a good correlation between histological findings and the ploidy patterns of 100% paraffin embedded specimens and 75% fresh specimens. The relative fluorescence intensity was found higher and more stable in paraffin embedded tissues than fresh tissues. Inflammation did not affect the ploidy patterns of paraffin embedded specimens whereas the ploidy patterns of fresh specimens were affected. It can be concluded that the DNA ploidy patterns and histological findings have a good correlation and inflammation does not affect the cytofluorometric DNA ploidy pattern when using the paraffin embedded specimens. Paraffin embedded specimens are superior to fresh specimens for making an ideal single cellular suspension to obtain stable, high, and accurate fluorescence intensity with less background fluorescence which makes the ploidy pattern behave like exact behavior of tissues.

Cytofluorometric determination of nuclear DNA content in gallbladder epithelium of patients with biliary stones.

Cytofluorometric determination of nuclear DNA content in fresh gallbladder epithelium of 11 patients with biliary stones were performed to assess the histological correlations and to evaluate the ploidy pattern affected by inflammation or regenerative proliferating benign epithelium associated with non-neoplastic or neoplastic diseases. 27% of the patients were classified to have pure cholesterol stones, 54% were classified to have mixed stones and 19% were classified to have black stones. All of the specimens show suggestion of chronic cholecystitis without any metaplastic or neoplastic change. The bile culture shows free of bacteria in all of the cases. Cytofluorometric nuclear DNA ploidy analysis shows 64% diploidy and 36% low ploidy patterns of histograms. It can be concluded that the low ploidy pattern which could lead to false positive diagnosis of aneuploidy and may be associated with regenerative proliferating benign epithelium with severe inflammatory changes.

Cystic fibrosis in Japan.

A nation wide survey for cystic fibrosis was carried out in Japan from 1982 to 1989. Cystic fibrosis was determined by extended criteria including high concentration of sweat chloride (more than 60 mEq/L), digestive, and respiratory manifestations, and family history. A total of 25 cases were confirmed as cystic fibrosis during the survey. Patients' characteristics were; age ranged from 0 day to 20 years old at diagnosis, sex ratio was 11:14 (M:F). All cases had respiratory, digestive symptoms and abnormally high sweat chloride excretion. Five cases from 3 families showed familiar involvement. These results show that cystic fibrosis is rare but really exists among Japanese. Further investigation including gene analysis should be carried out.

[Treatment with UFT as surgical adjuvant chemotherapy in carcinoma of large intestine. Cooperative Study Group of Surgical Adjuvant Chemotherapy for Colorectal Cancer in Japan].

The therapeutic results of the method II of the second study of the Cooperative Study Group of Surgical Adjuvant Chemotherapy for Colorectal Cancer in Japan were retrospectively reviewed and the relations of the 3-year survival rate to the dose of UFT per body weight as well as per kg of the patient and discontinuation of the treatment with UFT were investigated. The dose per day of UFT was randomly divided into group C (treated with UFT 600 mg/body/day) and group D (treated with UFT 400 mg/body/day for 12 months) by the envelope method. The 3-year survival rate of the patients whose body weight was 50 kg or more was higher than in the patients weighting less than 50 kg. The 3-survival rate of the patients treated with UFT from 8 to less than 12 mg/kg was the highest, while that of the patients treated with UFT of more than 12 mg/kg was the lowest. However, no statistical difference was observed between the two groups. The same results were observed in carcinoma of the colon and rectum, respectively. The findings indicated that the administration of excess amount of UFT was not effective. The relation between the 3-year survival rate and discontinuation of treatment with UFT was investigated at 6, 9 and 12 months after administration with UFT. The prognostic background factors of the C-group and the D-group were compared, but no difference was observed between the 2 groups at 9 months after administration. The 3-year survival rate of the patients treated continuously with UFT was higher than that of discontinued administration. The difference was especially evident in carcinoma of the colon, and good results were observed in cases treated continuously than in discontinued cases in both C- and D-groups.

[Cooperative study of surgical adjuvant chemotherapy of colorectal carcinoma (second study): 3-year survival after surgery of non-curatively resected patients. Cooperative Study Group of Surgical Adjuvant Chemotherapy of Colorectal Cancer in Japan].

A prospective randomized study was carried out to evaluate the efficacy of surgical adjuvant chemotherapy for non-curatively resected colorectal carcinoma patients as a collaborative study at 428 institutes in Japan from Jan., 1984 to Dec., 1985. A total of 1,138 patients were entered in this study. All patients were divided according to disease location (colon or rectum) and received one of two chemotherapeutic regimens after surgery (Regimen C: MMC 12 mg/m2 i.v. at operation day and 6 mg/m2 in every 2 months +UFT 600 mg/day p.o., D: the same dose of MMC + UFT 400 mg/day p.o.) randomly. 556 cases (colon carcinoma regimen C: 148, D: 185 cases, rectal carcinoma regimen C: 94, D: 129 cases) were evaluable at this presentation. Three-year-survival rate of histologically confirmed non-curatively resected colon carcinoma patients was higher in the group receiving regimen C than in regimen D. But no significant difference was found among the other groups.

Biliary stones and bacteriae in bile study in 211 consecutive cases.

A retrospective study have been performed to investigate the correlation between stone formation and bacteriae in the bile of 211 consecutive biliary stone patients (133 females and 78 males, age ranged 1.3 to 87.0 years old, 196 cholecystolithiasis, 54 choledocholithiasis and 9 hepatolithiasis). They had undergone surgery since January 1980 to December 1987. Bile was collected to perform bacterial culture from the gallbladder and common bile duct during operation and all stones had been classified according to morphology after surgery and confirmed with component analysis if necessary. Totally 5115 stones had been collected and 20.4% had been classified as pure cholesterol stone, 16.8% as combination stone, 17.2% as mixed stone, 23.6% as calcium bilirubinate stone, 17.6% as black stone, 0.8% as calcium fatty acid stone and 3.6% as unknown. In bile culture, 84 from the gallbladder and 31 from the common bile duct were positive. Streptococci were the most frequent and followed by Klebsiella, Escherichia coli and so on. A higher incidence of positive culture was shown in calcium bilirubinate and lower incidence was shown in cholesterol stones. These results suggest that bacteriae in the bile may play a role in the formation of pigment biliary stones.

Nationwide survey of cases of choledochal cyst. Analysis of coexistent anomalies, complications and surgical treatment in 645 cases.

A nationwide survey of cases of choledochal cyst was conducted to obtain the data on cases of choledochal cyst in Japan between 1972 and 1982. A total of 645 cases in 73 institutions, including 52 cases from this hospital, were surveyed. Choledochal cyst was associated with an anomalous arrangement of the pancreaticobiliary ductal system in 90.2% of the cases which were examined extensively. The serum amylase level was elevated in 26.6% of the cases. Calculus disease of the biliary duct was recognized in 28.5% of the cases. Biliary carcinoma developed in 54 (8.4%) of 645 cases. The incidences of biliary carcinoma were 0.3% (1/306) in cases in children and 15.6% (53/339) in adult cases; carcinoma was found in the wall of the choledochus in 30, and in the gallbladder in 16 of these 54 cases. The nationwide survey showed that an anomalous arrangement of the pancreaticobiliary ductal system was closely associated with choledochal cyst and that cases of biliary carcinoma developed with the advance of age. Therefore, early resection of the cyst wall and reconstruction of the biliary duct is proposed as suitable surgical treatment for stopping reflux of pancreatic juice into the biliary duct.

Monoclonal antibodies to hepatitis B surface antigen (HBsAg) as a tool for the automated HBsAg screening.

Hybridomas secreting monoclonal antibodies to hepatitis B surface antigen (HBsAg) were established by fusion of the spleen cells from mice immunized with purified HBsAg with the mouse myeloma cell line P3-NSI/1-Ag4-1. The monoclonal antibodies to the group-specific antigen (a) produced by one of them were used for the automated screening of HBsAg on the Groupamatic 360 (Kontron International). Its sensitivity is almost equal, but slightly inferior, to the system employing polyclonal horse antibodies to HBsAg; it barely detects 6 ng/ml of HBsAg. It is also as highly specific as the system with polyclonal antibodies; the incidence of false-positive reactions is 0.2%. These results indicate that the monoclonal antibodies will become a practical source for the HBsAg screening on the Groupamatic.
