Cerebral fat embolism studied by magnetic resonance imaging, transcranial Doppler sonography, and single photon emission computed tomography: case report.

Cerebral fat embolism syndrome is an uncommon complication of trauma. We present a patient who developed cerebral fat embolism syndrome secondary to long-bone fractures. Although computed tomography of the brain failed to show any intracranial lesion, magnetic resonance imaging (MRI) detected scattered, high-signal-intensity lesions on T2-weighted images. 99mTc-d, 1-hexamethyl-propylene amine oxine single photon emission computed tomography (99mTc-HMPAO SPECT) and transcranial Doppler sonography (TCD) demonstrated low cerebral blood flow in the acute stage. MRI, 99mTc-HMPAO SPECT, and TCD correlated well with the clinical course of cerebral fat embolism syndrome.

[A case of an adult patient of tetralogy of Fallot having malignant hyperthermia during surgery].

An episode of malignant hyperthermia occurring in a 42-year-old man undergoing hypothermic cardiopulmonary bypass is reported. Malignant hyperthermia is a syndrome initiated by a hypermetabolic state of skeletal muscle. A patient presented for correction of an acyanotic tetralogy of Fallot. The coincidental usage of hypothermic cardiopulmonary bypass obscured the classical presenting sings and symptoms of the malignant hyperthermia. And the disease of tetralogy of Fallot made the syndrome difficult to manage. Although the clinical diagnosis of malignant hyperthermia is difficult to be confirmed, when it is suspected, it is prudent for the case to be initially treated as malignant hyperthermia.

[Case report of successful treatment of patent ductus arteriosus with severe pulmonary hypertension in an adult].

A 40-year-old man was diagnosed as having PDA with severe PH by cardiac catheterization. We decided that surgery was indicated on the basis of 100% oxygen inhalation test. After 100% oxygen inhalation, left to right shunt changed from 37.8% to 61.5%, Qp/Qs changed from 1.43 to 2.60, and PVR changed from 1,199 dyne/sec/cm5 to 237 dyne/sec/cm5. We decided to perform surgery. Under extracorporeal circulation, PDA was safely closed by triple ligation and was directly closed after pulmonary arteriotomy. The postoperative course was uneventful. Severe PH was greatly improved, pulmonary artery pressure recovering to the normal range, by administering PGE1, prostacyclin (PGI2), and nitroglycerin.

[Surgical treatment of infective endocarditis : an analysis of the risk factor for operative mortality].

From January 1980, through December 1995, 60 patients underwent surgical treatment of infective endocarditis (IE) at Sakakibara Hospital. Of 60 patients, 40 showed active endocarditis and 20 healed endocarditis at the time of operation. The over-all hospital mortality was 10% (6/60). The complication of cerebral accident (embolism and bleeding) was related to a higher incidence of operative mortality. The reconstructions of destruction and disruption of ventricular-aortic or mitral-aortic continity in the presence of acute infection of the annular tissue were in need of radical and complex surgical techniques. It is recommended to follow up these patients thoughtfully and to perform reoperation or re-reoperation before a patient develops severe heart failure or multiple organ failure.

Long-distance PCR of VNTR at the D17S74 (CMM86) locus.

We have successfully amplified D17S74 (CMM86) alleles by a long-distance polymerase chain reaction (PCR) using TaKaRa Ex Taq (a Taq DNA polymerase with a 3'-exonuclease activity) and Perfect Match Polymerase Enhancer (a special polymerase enhancer). We adopted a hot-start technique with TaqStart antibody. Because of the high guanine content (60%) in D17S74 alleles, removal of K+ from the buffers was quite effective. The use of K(+)-free buffers reduces premature chain termination in G-rich regions, thereby facilitating amplification of targets containing such sequences. The 17 alleles amplified from DNA samples of 72 unrelated Japanese subjects ranged from 1.05 to 3.5 kb, with a heterozygosity of 92%. PCR amplification of D17S74 alleles makes their detection simpler than by conventional Southern blotting, and increases the practical utility of the locus.

[Measurement of airborne fumes in welding factories].

Concentrations and size distributions of airborne welding fumes in work environments were measured. Samples were collected from 20 workplaces in 13 companies. In all workplaces, the stationary samplings including A-sampling and B-sampling and the personal sampling were carried out simultaneously. The fume concentration in the stationary sampling was measured with a digital dust monitor. A low volume air sampler and an Anderson cascade impactor was also used to determine the conversion factor of the values obtained by the digital dust monitor and the size distributions of the fumes. The personal exposure levels were measured with a TR sampler and a personal cascade impactor. The concentrations by the personal sampling (average: 11.3mg/m3) were much higher than those by the A-sampling (average: 0.4mg/m3 and range: 0.15-0.64mg/m3) but lower than those by the B-sampling (average: 46.8mg/m3, range: 0.66-152.8mg/m3). The mass median aerodynamic diameter (MMAD) of the stationary and the personal samplings were 2.41 microns and 0.80 microns, respectively. Fumes were observed under a scanning electron microscope. Most of the fume particles were sphere in shape but smaller fumes were aggregated and formed larger secondary particles.

gp180, a host cell glycoprotein that binds duck hepatitis B virus particles, is encoded by a member of the carboxypeptidase gene family.

Duck hepatitis B virus particles bearing the L and S envelope proteins bind a cellular glycoprotein of 180 kDa (gp180) with high affinity and specificity. Binding is mediated by the pre-S region of the L protein and is blocked by neutralizing but not by non-neutralizing monoclonal antibodies to the virus. These and other properties have suggested that gp180 may be a component of the viral entry machinery. Here we report the purification of gp180 from duck liver and the isolation and characterization of cDNA encoding it. DNA sequence analysis of this cDNA indicates that gp180 is a novel member of the basic carboxypeptidase gene family.

Spontaneous disappearance of an aneurysmal malformation of the vein of Galen.

We describe spontaneous disappearance of an aneurysmal malformation of the vein of Galen (AMG), an anomaly that occurs most often in children, with a high mortality due to the accompanying severe cardiac failure. Spontaneous thrombosis of an AMG is rare. In this case, the AMG has been closed for about 5 years, without radiological evidence of thrombosis, and no active treatment. Proposed mechanisms of spontaneous thrombosis include slow flow shunts and severe obstruction of the venous outflow. However, in this case, the disappearance of the AMG without evidence of thrombosis indicates a different mechanisms, namely, obstruction of the feeding artery.

Giant cell glioblastoma in the frontal cortex of a dog.

A dark gray mass 3 cm in diameter replacing the right frontal cortex was found in the brain of a 5-year-old male Doberman Pinscher dog at necropsy. Microscopic studies revealed that the mass consisted of a proliferation of pleomorphic tumor cells: large bizarre or plump eosinophilic cells, multinucleated giant cells, and small lymphocytic cells. These neoplastic cells at the margin of the necrotic area had a psuedopalisade arrangement and tended to proliferate around blood vessels. Immunohistochemically, the tumor cells reacted intensely with the antibody for vimentin and moderately with those for S-100 and glial fibrillary acidic protein. This canine tumor is placed in the category of glioblastoma or undifferentiated astrocytoma, which is analogous to human giant cell glioblastoma.

PCR amplification of D2S44 (YNH24) alleles.

We have successfully amplified D2S44 (YNH24) alleles by a method for long-distance PCR using a special polymerase enhancer, Taq Extender PCR Additive. The alleles amplified from DNA samples of 58 Japanese subjects ranged from 0.42 to 3.5 kb and were 1.5 kb shorter than those detected by Southern blotting of Hinf I-digested genomic DNAs. Although alleles longer than 3 kb were barely visible by ethidium bromide staining, we were able to visualize them clearly with SYBR GREEN I NUCLEIC ACIC GEL STAIN. PCR amplification of D2S44 alleles is much simpler than their restriction fragment length polymorphism (RFLP) analysis; therefore, our procedure is well-suited for use in medicolegal practice. With minor modifications, the method described here should be applicable to other loci of variable number of tandem repeats (VNTRs) that have been analyzed by Southern blotting.

Analysis of the binding of a host cell surface glycoprotein to the preS protein of duck hepatitis B virus.

We have previously identified a 180-kDa host cell glycoprotein (gp180) that specifically binds the surface envelope of duck hepatitis B virus (DHBV) and whose binding is inhibited by neutralizing antiviral monoclonal antibodies. Here we map the viral determinants required for gp180 binding to a 66-amino acid region within the preS domain of the envelope coding region. This region includes both major neutralizing preS epitopes previously defined by monoclonal antibodies. Examination of a series of linker-substitution mutations throughout preS indicates that all mutations that block gp180 binding ablate virus infectivity. Interestingly, two mutations that do not prevent binding can also impair infectivity.
