[Entoptic detection of choriocapillaris].

Entopic perception of the central choriocapillaris is documented by personal experience. Seen in this manner, the choriocapillaris appears due to circulatory conditions as a wavy convolute of fresh capillaries whose paths are paralleled by numerous, irregularly distributed, round, black structures. The latter have been presumed to be corpuscles, i.e. erythrocytes, and can be seen entopically as single rows of radial capillaries. Histological studies have shown, however, that these rows of erythrocytes are not always isolated; they have also been found in the choriocapillaris in clusters of rows. Moreover, analogously to the lack of entopic perception of erythrocytes in the retinal vessels in Purkinje's images, the erythrocytes of the choriocapillaris may likewise not be perceived for physical and optical reasons. Therefore the round, black formations are not entoptically perceivable erythrocytes of the choriocapillaries but can only originate in the circular network of the central choriocapillaris. Light directed experimentally into the eye and focussed on the choriocapillaris through a narrow slit positioned nasally leads via reflection to stimulation of the retina from outside to inside and so to entoptic perception of the choriocapillary network. Closer analysis of entoptic perception of the choriocapillaris is significant in view of its heuristic value.

[Macular coloboma and alternating diffuse chorio-retinal dysplasia].

The author reports on two siblings. The elder, a 28-year-old woman has bilateral macular coloboma and a high degree of myopia. Her 27-year-old brother's left eye also has a macular coloboma, while his right eye has diffuse chorioretinal dysplasia with extensive nevoid choroidal pigmentation but no macular coloboma. His right eye is hyperopic, the left myopic. The eyes of the parents and their other, younger daughter are normal in all respects. Congenital syphilis and toxoplasmosis have been excluded as causes of the fundus deterioration of the first two offsprings. The macular colobomas of the two siblings (circumscribed macular dysplasia) and the alternating diffuse chorioretinal dysplasia must be classified as genetically conditioned maldevelopment of the optic cup ectoderm.

Late manifestation of conjunctival malignant melanoma.

The patient, who is now 60, had a nevus and areas of cystic changes in the conjunctiva in her right eye, which had been present from birth to the age of 59. In 1987, rapid tumor growth aroused suspicion that malignant change could be occurring and so the nevus as well as the area of the conjunctiva containing cystic changes were excised. Histological finding: conjunctival malignant melanoma, containing compressed (densely arranged) atypical cells with pleomorphic hyperchromatic nuclei and prominent nucleoli. The biopsy specimen of the conjunctiva showed a typical nevocellular nevus with large inclusion cysts of the conjunctival epithelium.

[Assessment of eye damage in private accident insurance in Austria].

In Austria, eye damage is assessed by private accident insurance underwriters in accordance with the "General Rules for Private Accident Insurance" (AUVB 1965). According to these rules the degree of invalidity is determined on the basis of three types of findings: 100% in the case of total loss of vision, 30% in the case of total loss of vision in one eye, and 60% if vision in the other eye had already been lost. In the case of complete loss of vision in one eye and simultaneous partial loss in the other, or partial deterioration of vision in both eyes, the basis for calculating the degree of invalidity is 50% for each eye. The respective degrees of disability are expressed in thirtieths. The degrees of disability are adjusted to the levels of reduction in earning capacity in accordance with the proposals of the German Ophthalmological Society, which are also recommended in Austria. The difference in the degree of disability according to the AUVB in Austria and the AUB in the Federal Republic of Germany, resulting from different methods of calculation, is pointed out. The degrees of disability for accidental damage to one or both eyes (without previous damage) are presented in tabular form. As regards visual field defects, the degree of disability should be assessed on the basis of the total visual field, taking into account the fact that the eyes are paired sensory organs.

[Origin of mirror writing in children].

In left-handedness it is not the right hemisphere which usually dominates, as previously assumed but rather the left, as recent studies show. The development of right- and left-handedness is considered here in the light of the findings of Eccles, who substantiated the hypothesis of a relationship between the brain and a consciousness which is to be seen as basically independent of this organ. Right- and left-handedness are engendered by the interplay between consciousness and certain liasion areas in the brain. When the subject's will dictates a movement, consciousness exercises a strong influence on the impulse pattern of the modules. In genetically conditioned left-handedness it is not primarily motoric, but rather neuronal factors which are responsible for the reversal in the direction of writing and mirror-image script. Left-handed children who produce mirror-handwriting can be retrained to write normally by exercises and by assimilating conscious experience.

[Genetic aspects of Behçet's disease and juvenile rheumatoid arthritis].

Diagnosis of Behçet's disease in a subject implies that the monosymptomatic aphthae in other members of the family, which are also caused by genetic factors, should be considered as malignant aphthosis. In seronegative juvenile rheumatoid arthritis without common symptoms with anterior uveitis, genetic factors also play an important role.

[Idiovariation of papillogenesis].

Among the great number of idiovariations of papillogenesis, some malformations near the optic disk are of special interest with regard to their formal origin, morphology and differential diagnosis. This matter is discussed on the basis of the author's own observations, which relate to the coloboma spurium of the optic disk, to the ectatic colobomas situated on the disk, the circumscribed pitting of the disk and to Handmann's optic nerve anomaly. With regard to the central retinal changes which occur in coloboma spurium of the disk and in circumscribed pitting of the disk, these are not to be explained by mechanical factors, but as correlation alterations. They are also closely connected with the macular changes in Handmann's optic nerve anomaly. Coloboma spurium of the disk as well as circumscribed pitting of the disk should be distinguished. With regard to their formal origin and morphology, the former malformation should be designated major pitting and the latter minor pitting of the disk. A guide is given to the differential diagnosis of major and minor pitting of the disk, as well as of glaucomatous cupping.

[Bullous dermatoses with special reference to the symptomatology of the eyelids in pemphigus vulgaris].

Among the bullous dermatoses the pemphigus group, the pemphigoids have to be distinguished from the hereditary epidermolyses. Immune-histological examinations are of paramount importance for the differential diagnosis between pemphigus vulgaris and cicatricial pemphigoid. This paper describes symmetrical erosions of the edges of the eyelids and adjacent skin which have their origin in transient vesicle formations, and classifies them as a partial symptom of the overall pemphigus vulgaris complex. High doses of cortison can re-epithelialize these troublesome and cosmetically unpleasant erosions.

[Corneal dyskeratosis].

Isolated dyskeratosis of the cornea is more frequent than is generally recognized. Our observations show that corneal dyskeratosis can originate from insufficient tear secretion. Also, trophic disturbances together with a mucus deficiency of the tear-film may be the cause. With regard to the treatment of dyskeratoses, the etiological factors should be taken into account. Isolated dyskeratosis of the cornea cannot be considered a precancerosis.

[The effect of the sap of Euphorbiaceae on the eye].

Ocular inflammations caused by the latex of euphorbiaceae manifest themselves as lid swelling, severe conjunctivitis, keratitis and iritis with hypopyon. As therapy, the local application of corticosteroids is recommended. In severely affected cases intravenous injections of Sandosten-Calcium and Solu-Dacortin (Merck) are indicated. The caustic effect of euphorbia cyparissias is caused by phorbol acid and phorbol acid ester. On the Canary Islands, the latex of euphorbiaceae is quickly removed by drop application of the sap of aeonium lindleyi, family crassulaceae. In the past, the sap of sempervivum tectorum which also belongs to the crassulaceae, was used as a household remedy for ocular inflammations.

[Clinical course and treatment of vernal conjunctivitis (author's transl)].

Vernal conjunctivitis is caused by a hyperplasia of connective tissue. On the upper eyelid, vernal conjunctivitis may manifest in a mild or serious form. The mild form is characterized by isolated small warts on the upper edge of the tarsus. In such cases the conjunctiva tarsi may exhibit increased vascularization with biomicroscopically demonstrable, subepithelial, reticular density of the connective tissue. In the chronic tarsal form large cobble-stone proliferations (condylomata) develop, which may result in a keratopathy (disk-shaped erosion). The association of vernal conjunctivitis with other symptoms of atopy (neurodermitis constitutionalis, bronchial asthma) is indicative of common etiopathogenetic relationships. The serum antibodies present in atopy are immunoglobulins of the E type. The differential diagnosis of vernal conjunctivitis comprises trachoma, paratrachoma, conjunctival folliculosis, and chalaziosis. Therapy consists of a local application of corticosteroid containing drops and ointments, drops containing vasoconstringents and antihistaminics, and of Opticrom eyedrops. In the inveterate bulbar type, subconjunctival injections of corticosteroids are indicated. Frequently, vernal conjunctivitis ceases after onset of puberty. When large and coarse condylomata on the upper eyelid occur together with long-lasting complaints and corneal complications, combined tarsal extirpations after Kuhnt is the method of choice.

[Hereditary nystagmus].

Hereditary nystagm occurs in latent and manifest form. It can be occur in isolation or together with certain hereditary alterations of the eye or of the central nervous system. The objection to a sharp separation of isolated (extraocular) and ocular hereditary nystagm is that no causal relationship between hereditary bulbar alterations and nystagm need exist. Survey of the hereditary transmission of latent and manifest nystagm in extraocular occurrence as well as in hereditary alterations of the eye and of the central nervous system. Reference to the significance of the brain stem for sight regulation and for the occurrence of hereditary latent and manifest nystagm. In operative treatment of nystagmic torticollis, Kestenbaum's principle must be taken very thoroughly into account to avoid therapeutic failures, as has been demonstrated by some practical examples.

[The significance of vitrectomy after perforating injuries].

This paper presents three cases that show the importance of vitrectomy after perforating injuries of the eye. Whenever a trauma produces a mixing of vitreous and lens material in the anterior chamber, vitrectomy is the method of choice. Chronic posttraumatic uveitis can be prevented through this procedure. The three young patients, who were operated with the Vitrektor, reached a 6/6 vision with contact lenses after 1 1/2, 2 and 3 years respectively of observation. No consent can be given to the opinion recently expressed that the removal of infected vitreous after a cataract extraction nearly by an irrigation of the anterior chamber is considered to be a vitrectomy performed for decades. Priority for vitrectomy should be given to Kasner.

[Evaluation of the degree of disability for the privately insured patient (author's transl)].

There are marked differences between the public health system and the private insurance system. This paper shows how to evaluate the degree of permanent disability according to "The General Rules for Private Insurances (AUVB 1965)". At the beginning the evaluation of the degree of permanent disability after the visus naturalis or the visus corrected and at unilateral and bilateral aphakia will be discussed. The paper gives detailed information on the degree of disability for: one injured eye, one injured eye with preexisting ocular lesion, bilateral injury, one injured eye with preexisting lesion of the other eye, one injured eye with preexisting ocular lesions of both eyes, bilateral injury with preexisting lesions of both eyes. For bilateral lesions, the degree of disability is calculated as a function of the reduction in activity by the AUVB 1965; this is given in one table. The rules applying to Austria are compared to those valid in the Federal Republic of Germany (AUB). It is very important to understand these methods of calculation, if one wants to secure an objective ophthalmological evaluation of the degree of disability for the privately insured patient.

[Conjunctival alterations by sympathomimetic drugs (author's transl)].

This is the report of two patients, who used eye drops containing adrenergic agents for a relatively long period of time because of conjunctival troubles. The eye drops contained phenylephrine or tetryzoline or naphazoline. As a consequence of the vasoconstriction, the treatment led to an epithelial xerosis in the region of the caruncula and of the neighbouring eyelid edges. The keratinisation resulted in a closure and disappearance of the puncta lacrimalia. Therefore, the application of vasoconstrictive eye drops for a longer period of time is discouraged. Moreover, eye drops with a low concentration of the sympathomimetic agent should be preferred.

[The consideration of topographic anatomy in ophthalmic surgery (author's transl)].

The fistulizing operations can be classified with regard to their topography to the Schlemm's canal into the precanalicular Elliot's trephining, the trephining in the region of the Schlemm's canal (intercanalicular) or of its next vicinity with scleral flap (goniotrephining with scleral flap), the sclerectomy in the region of the Schlemm's canal (intercanalicular) or of its next vicinity with scleral flap (trabeculectomy). If the trephining is performed retrocanalicularly, the scleral spur and the ciliary muscle are involved. Iridodialysis and haemorrhage from the circulus arteriosus iridis major may occur in the peripheral iridorhexis, which is recommended for the iridencleisis and for the iridectomy in acute glaucoma. The proof for the decompression of the intrascleral optical canal in central vein thrombosis by the surgical procedure reported by Vasco-Posada cannot be deduced when the topographic-anatomical circumstances are taken into consideration.
