[Focal nodular hyperplasia of the liver].

Among the benign liver tumours particularly the focal nodular hyperplasia (FNH) increased its importance. Within 10 years we could observe 28 patients with the lesion. Macroscopically the nodular changes show extremely characteristic findings. The histological picture reminds us of cirrhosis, the prevailing presence in women and the clinical recognition mostly at reproductive age let us think of a hormonal action in development and growth of the lesion. The same concerns the use of hormonal contraceptive drugs. Differential-diagnostically particularly malignant processes are to be demarcated. Though the danger of a haemorrhage with formation of a haemoperitoneum is insignificant, in the individual case a surgical approach should be taken into consideration.

[Concurrent appearance of stomach polyposis and hepatocellular carcinoma].

An 18 year old woman had been known to have juvenile polyposis since five years with multiple hyperplastic polyps of the stomach and several polyps of the colon. Death occurred from hepatic coma with hepatocellular carcinoma. A lymphangiosis carcinomatosa was observed in some stomach polyps with a circumscribed infiltration into the stroma and into the adjacent glands. Such an unusual combination of diseases readily causes diagnostic confusion.

[Morphology, incidence and clinical significance of changes in the liver parenchyma following use of hormonal contraceptives].

Among 438 liver punctates of women who had used oral contraceptives and who showed a clinical symptomatology or pathological laboratory values in 332 cases (75.8%) morphologically recognizable changes were formed which spoke for a causal connection with the application of hormonal contraceptives. Pathological-anatomically 5 various types of damage may be differed, in which cases frequently a combination of findings is present: uncharacteristic lesions with epithelial and mesenchymal changes, disturbances of the microcirculation in form of sinusoid ectasias up to the development of a peliosis hepatis and intrahepatic venous thrombi, intrahepatic cholestasis without any essential inflammatory reaction, hepatitic changes, liver tumours and tumour-like lesions. The clinical importance and valency of the individual changes is different, whereby exclusive uncharacteristic findings mostly have no pathological value, whereas a cholestasis and hepatic changes as well as tumours demand an end of the medication. Most frequently sinusoid ectasias occur; they represent the most important morphologic indicator of a lesion of the liver parenchyma after application of hormonal contraceptives.

[Rare liver damage caused by an antihypertensive and anti-arrhythmic agents].

It is reported on lesions of the liver parenchyma after the application of dihydralazin and detajmium bitartrate with description of the clinical and morphological findings. By means of the morphological findings got in liver biopsy a clinically supposed acute virus hepatitis could be excluded. After the application of dihydralazin group cell necroses and an inflammation of the periportal fields, after detajmium bitartrate a typical intrahepatic cholestasis was in the front. The knowledge of medicamentous side-effects may avoid diagnostic and therapeutic errors in the individual case.

[Morphological and clinical liver changes after taking oral contraceptives].

In 155 women of the age group 16-45 years after intake of oral contraceptives in 68.4% (106 cases) histologically provable liver changes were found. Sinusoidal ectasias of different size and drug-conditioned hepatitides most frequently occurred. An isolated intrahepatic cholestase only rarely existed. Increases of transaminase are the most frequent reference to a lesion of the liver parenchyma after intake of oral contraceptives. The author adopts a definite attitude to the differential diagnosis and its difficulties between a drug-conditioned hepatitis and the usual virus hepatitis.

[Liver biopsy findings after intake of oral contraceptives (author's transl)].

115 liver biopsies of women who had taken oral contraceptives have been examined. In 106 of these case (68.4%) morphological modifications were found which may be considered as effects of the hormonal contraceptives. Dilations of the sinusoids with differently proliferated Kupffer cells were predominantly evident (73 cases). Medicamentally induced hepatitis (20 cases) and cholestatic hepatosis (5 cases) were considerably rare. The sinusoid ectasias may be understood as a consequence of liver damage by steroid hormones. The pathogenesis of the changes is unknown.

[Endooesophageal prostheses and their complications (author's transl)].

After inserting an oesophageal prosthesis complications could be demonstrated in 53 cases (65%) from a total of 81 autopsies. Age distribution, tumour localisation and its relation to the development of local complications are analysed. Notwithstanding the high incidence of complications endoprostheses should be given preference to gastric fistulae.

[Morphological findings after Busulfan long term therapy (author's transl)].

Autopsy findings from two patients who have been treated for several years with Busulfan because of chronic myelogenous leukemia are reported. Beside a pulmonary fibrosis epithelial dysplasias in the respiratory tract, the liver and kidney were detected. Furthermore, in one case beside a proliferation of reticulate cells in the spleen a distinct plasmocytosis of the bone marrow and a so-called "wasting syndrome" were found. The pathogenesis of these changes is still unkown; it may be that immunological processes play a role.
